Laura, thanks so much for sending along the information, and the "permit owner affiliation form." As I mentioned in a
voice mail to you this morning, I know that Waste Management (Shawn Carroll) submitted a "permit name/ownership
change request" form for our Fayetteville Hauling Company in a letter addressed to you, dated March 27, 2015. Our
intent in that correspondence was to simply change the responsible contact within our organization. I believe that the
form'we submitted will accomplish that, but please let me know if you would prefer that we submit the "permit owner

Aana Taylor -Smith of the Winston-Salem Regional Office of the NC Division of Water Quality (DWQ) conducted a
compliance evaluation inspection at the subject facility on 27 June 2013. Your assistance and cooperation, as
well as that of Mr. Jason McRee and Ms. Teresa McGee, were greatly appreciated. An inspection checklist is
attached for your records and the inspection findings are summarized below.
The facility is located at 2991 Highway 770 East in Stoneville, Rockingham County, North Carolina. Stormwater
related to industrial activity is discharged to the Mayo River, this section of which is currently classified as Class
WS-IV (Water Supply) waters in the Roanoke River Basin.
Documentation and Monitoring Review
The Stormwater Pollution Prevention Plan (SPPP) is complete, current, and very thorough. It was last reviewed
in May 2013. Facility inspections are conducted monthly, which exceeds the requirements of the permit.
Employee training is also conducted monthly, with stormwater and spill prevention/cleanup components. Waste
Management's dedication to compliance is highly appreciated.
Qualitative monitoring has been conducted and documented as required. The Waste Management facility has
one Stormwater Discharge Outfall (SDO) at which qualitative monitoring is performed. No analytical monitoring
is required, as less than 55 gallons of new motor oil are used per month on average, and there is no oil/water
separator or petroleum bulk station/terminal on site.
Facility Site Review
The facility is orderly and well -maintained. All necessary secondary containment is implemented as required.
Outfall 001 was inspected. The SDO was clean and receiving waters were free of pollution indicators. No '
evidence of erosion or instability at the SDO was observed.

In reference to your letter dated August 151h, 2008 regarding the earlier compliance
evaluation inspection conducted by Mr. Ron Boone on August 121h, 2008 at our
Stoneville Hauling facility, we submit the following responses, which will also serve as
our corrective action plan, and implementation schedule to resolve all of the identified
issues:
1. Item 3: As a correction to your letter, we do conduct analytical monitoring at this
site. Mr. Boone reviewed the results of our previous analytical monitoring events
during his inspection, as well as the qualitative monitoring results. He
commented, during our walk around, how well he liked our Tier I improvements
as a result of the initial TSS benchmark exceedance of the analytical monitoring
results. Therefore we believe item 3 in your August 151h letter should be
corrected; analytical monitoring is conducted as required.
2. Item 4a: The general location map required by Part II, Section A, Paragraph 1(a)
has been completed and added to the stormwater pollution prevention plan.
3. Item 4b: The plan site map has been updated to reflect the percentage of each
drainage area that is impervious as required by Part II, Section A, paragraph 1 (c).
4. Item 4c: Regarding the comment on the annual review, the SP3 was only
completed in January 2008, so we should have until January 2009 to perform the
review. When Mr. Boone reviewed our previous SP3, the annual reviews had
been conducted and documented, so item 4c in your August 15`h letter is incorrect.
We will conduct an annual review in January 2009 and document the review with
either a review page or annotations to the plan, as suggested.
Waste Management diligently pursues environmental excellence. This was the first
NCDENR inspection for any of our NC facilities under the General Permit that was just
issued in November 2007, so we are proud that these minor areas of concern had no
impact on the natural environment.
We trust that these responses adequately address those concerns expressed in your letter
of August 151h, 2008. As noted, all concerns and responsive actions have been
completed.

Your assistance and cooperation during the CEI was greatly appreciated. An'inspection checklist is
attached for your records and the inspection findings are summarized below.
Site Review
2. This facility is located at 2991 NC Highway 770 West, approximately one mile west of US Highway
220. Stormwater related to industrial activity is discharged from stormwater discharge outfali (SDO)
001, which is located in the grassed swale area in the front of the building. Stormwater is
discharged to the Mayo River, this section of which is currently classified as Class WS-IV (water
supply) waters in the Roanoke River basin.
3. Qualitative monitoring has been conducted as required at SDO 001. No analytical monitoring is
required as new motor oil usage is well under the 55 gallon per month threshold for vehicle
maintenance activities and there is neither an oil/water separator nor petroleum bulk storage at the
site.
Documentation Review
4. The facility does have a stormwater pollution prevention plan (SP3). The following SP3 deficiencies
were noted during the inspection:
a. There is no general location map as required by Part II, Section A, Paragraph 1(a).
b. Part II, Section A, Paragraph 1(c) requires the site map to reflect the percentage of each
drainage area that is impervious. Please update the site map with this information.
c. Please be sure to annotate the annual review and up date of the SP3 as required in Part II,
Section A, Paragraph 7.
5. Please be aware that the Division may assess civil penalties of up to $25,000 per day, per
violation for failure to comply with the requirements of NCG080000. Please reply to this letter
within 30 days of receiving it. Your response should include a corrective action plan and

In accordance with your application for a discharge permit received on August 20, 2004
we are forwarding herewith the subject certificate of coverage to discharge under the subject
state — NPDES general permit. This permit is issued pursuant to the requirements of North
Carolina General Statute 143-215 .1 and the Memorandum of Agreement between North
Carolina and the US Environmental Protection Agency dated May 9, 1994 (or as subsequently
amended).
Please take notice that this certificate of coverage is not transferable except after notice to
the Division 01' Water Quality. The Division of Water Quality may require modification or
revocation and reissuance of the certificate of coverage.
This permit flocs not affect the legal requirements to obtain other permits which may be
required by the Division of Water Quality or permits required by the Division of Land
Resources, Coastal Area Management Act or any other Federal or Local governmental pen -nit
that may be required.
