Inspection Summary:
At the request of Derrick Boone, City of High Point Public Services Manager, Glen White and Sue White of NCDEQ met
with him and Fleet Maintenance Facility staff to discuss the issue of low pH readings at the outfall (Test Site A) located
nearest the parking area of the solid waste vehicles. Officials have tested the Solid waste trucks several times and have
found low pH readings in them many times. They believe heavy organics may be a contributing factor.
This letter is to serve as a record of the meeting held on 11/06/2015, the issue and response as discussed and may not be
a complete record.
Public Services and Fleet Maintenance Facility staff have taken a proactive approach at trying to resolve the issue and are
in the process of implementation of the following:
- Facility will be in "Tier 2 Response" until 3 consecutive samples result within permit parameters.
- COHP will provide an "Action Plan" detailing a timeline and steps they'll take to resolve the issue.
- Trucks are to go through wash before being parked for the night. They will try to ensure this is done by each driver.
-Investigate the possibility of washing solid waste vehicles at the landfill. COHP believes this may eliminate the issue.
- In an effort to reduce or eliminate the issue, COHP has requested and already has approval for a project to install a garage
or covered area for the solid waste vehicles. This may in itself eliminate the problem.
Since COHP is working hard to resolve the pH issue, NCDEQ will work with the City to prevent going into a "Tier 3
Response" and will allow them to continue with the "Tier 2 Response" as currently implemented.

Dear Mr. White:
The purpose of this letter is to follow-up on the meeting that was held on site at the Fleet
Maintenance Facility (3524 Triangle Lake Road) on November 6, 2015 with staff from the
NCDENR Winston-Salem Regional Office. The meeting was requested by the City of High Point
for guidance from NCDENR in addressing the low pH readings that have routinely been
observed at outfall SDO-001A which is located downstream of the parking area for solid waste
trucks. The outcome of the meeting was that NCDEQ will work with the City of High Point to
prevent from going into a Tier 3 response and will allow the CHP Fleet Maintenance Facility to
continue with the Tier 2 response as currently implemented.
As requested, I have enclosed an Action Plan for addressing the low pH readings at the CHP
Fleet Maintenance Facility. The CHP Fleet Maintenance Facility is making every effort to
comply with the requirements of its stormwater permit and address the pH concerns at outfall
SDO-001A. Please feel free to contact me if you have any questions.

Summary: The City of High Point Fleet Maintenance Facility is a 30.3 acre site comprised of the
Fleet Maintenance Building, the Signs and Signals Building, two cold storage buildings, a vehicle
wash building, fueling station, the Environmental Services Building, and associated
equipment/vehicle parking lots. There are 2 stormwater outfalls (SDO-001A and SDO-001B)
that must be analytically monitored for the parameters of pH, Oil & Grease, and Total
Suspended Solids. Over the last 2 years, there has been an issue of low pH readings at outfall
SDO-001A, which is downstream of the Environmental Services Building, sanitation truck
parking lot, and employee parking lot. The collection of pH samples throughout the facility has
traced the low pH readings back to the liquid that is collected in the garbage trucks. The Fleet
Maintenance Facility will implement the following "Action Plan" for addressing the low pH
readings that have been observed at stormwater outfall SDO-001A.
Action Plan: The Fleet Maintenance Department and Public Services Department will
implement the following measures to address the low pH readings at the Fleet Maintenance

The Winston-Salem Regional Office of the Division of Water Quality performed a
stormwater inspection of High Point Fleet Services on September 2, 2009. This
inspection falls under the provisions of North Carolina General Statute 143-215.1.
The aforementioned inspection was performed by Marc Stokes of this office.
PERIMIT
High Point received a Certificate of Coverage (COG), NCG080683, under the
NPDES General Permit NCGO80000 in June 2003, renewed in November 2007 and
will expire in October of 2012. The permit is associated with SIC 41.
FACILITY
_- The facility consists of-1-1 acres,-2-major buildings with, a"80%impervious surface. ---
The facility has 23 employees and operates on a 2417 basis. The wash pit area is
covered and confined with the sump being pumped on a routine basis to an oil water
separator. There exist 2 above ground tanks, which have secondary containment,
that contain various petroleum products. Three dumpsters are present of which one
is for recycling. The facility has 2 outfalls that end in the Deep River.
STORtNTWATER POLLUTION PREVENTION PLAN (SPPP)
A Stormwater Pollution Prevention Plan has been prepared for the facility that
contains the components as required by your permit. The facility does have a Spill
Prevention and Response Plan that incorporates components of the SPPP plan.

CERTIFICATION OF NON-STORMWATER DISCHARGES
I certify, under penalty of law, that the stormwater outfalls covered by this SWPPP have
been tested or evaluated for the presence of non-stormwater discharges in accordance
with a system designed to assure that qualified personnel properly gather and evaluate
the information submitted. Based on my inquiry of those persons, to the best of- my
knowledge and belief, except as indicated on completed Form 3 in Appendix D, no
non -permitted discharges of non-stormwater enter the stormwater outfalls covered by
this SWPPP because of activities occurring at this Facility [as specified in Part 11, Section
A, Paragraph 1(e) of General Permit No. NCG080000].

RECORD OF PLAN REVIEWS
The SWPPP will be reviewed and evaluated at least once a year. Blank lines are provided
on this form for each SWPPT member's review. The SWPPT Leaders are assigned the
responsibility of ensuring that this Plan will be reviewed and amended in accordance with
NPDES requirements. The SWPPT Leaders may amend this Plan to include more
effective pollution prevention technology and BMPs, if such technology is field proven and
if implementation will significantly reduce the likelihood of the contamination of stormwater.

SWPPP AMENDMENT RECORDS
The SWPPP will be amended whenever there is a change in Facility design, construction,
operation, or maintenance that has a significant effect on the potential for the discharge
of pollutants to surface waters of the state. The SWPPP will also be changed if the
SWPPP proves to be ineffective in eliminating or significantly minimizing pollutants from
sources identified in the SWPPP, or in otherwise achieving the general objectives of
controlling pollutants in stormwater discharges associated with industrial activity. Any
amendments will be implemented to the maximum extent practical after such change
occurs. This record sheet is provided to summarize amendments to the Plan.
