This letter is in follow-up to the NPDES Stormwater Permit Compliance Inspection conducted on
November 8, 2010 1 appreciated the opportunity to meet with you, Ms Lipe, and Mr Stevens
The facility was found to be in compliance with most aspects of the stormwater permit
(NCG080682) However, the facility was not in compliance with the benchmark value for Total
Suspended Solids Due to the exceedances (at both outfalls) of Total Suspended Solids, the City is
under Tier One response, specified in Part I1, Section B of the permit
Enclosed is a copy of the Compliance Inspection Report, which contains additional observations
and comments for your reference (in the Summary section on p 2)

Inspection Summary
Susan Wilson (AROIDWQ) met with City staff members Mark Stevens, Keisha Lipe and McCray Coates at the Fleet
Maintenance site
After the initial site visit March 2010, the City hired Stormwater Services Group to evaluate the site and provide
monitoring/sampling data Sampling was conducted May 2010 (after purchase of bag filters for drop inlets) A review of
the data indicated that the TSS benchmark values were exceeded for the two outfalls that are sampled (002 and 006,
outfall 002 is near the upstream' portion of the site and outfall 006 is near the 'downstream' portion of the site other drop
inlets are directed to these outfalls -which go directly to a culverted stream under the property) Since the benchmark
values for TSS were exceeded, the City Fleet Maintenance is now under Tier 1 response The City was reminded to
review all data after results are received and possibly sample more frequently and/or evaluate the area to determine the
source of the exceedance Another exceedance will cause the City to be in a Tier 2 response (where monthly sampling
will have to be conducted as well as other elements specified in the permit) DWQ suggests that analytical and qualitative
monitoring be conducted at the same time (It may be that some sediment runoff from the adjacent hillside is an issue)
The City's investigative actions should be documented and kept with the Stormwater Pollution Prevention Plan (SWPPP)
The City has already notified DWQ via e-mail of their pending response to Tier One
Vehicle maintenance takes place in the building with floor drains that ultimately go to an oillwater separator, then to the
sanitary sewer system
Most elements of the SWPPP are in place, but should be easily referenced and accessible It was suggested that the City
document (and have forms for) facility inspections when parking lot is cleaned etc

McCray — Below is the comments from our inspection I thought you had said the sweeping would be done weekly but
when I looked at the e-mail you sent to James on Wednesday it read every 2 weeks? If that is the case we will need to
change number three to read every two weeks and the same with number four I did not know when the sweeping is
going to begin so that will need to be inserted Let me know if you need me to do anything else before Mark sends to

1 Conduct a Stormwater management inspection of the facility within two weeks of receiving sampling results
2 Identify and evaluate possible causes of the benchmark value exceedence
• Loose material in parking lot
• Debris fallen off trucks in parking lot
• Steep bank along the back of the facility
3 Identify potential and select the specific source controls operational controls, or physical improvements to
reduce concentrations of the parameters of concern, or to bring concentrations within the benchmark range
• Weekly sweeping of parking lot
• Use a leaf blower weekly on parking lot
• Meeting with manufacture to see what could have caused the high total suspended solids reading at the
May 14 2010 testing
4 Implement the selected actions within two months of the inspection
• Sweeping will begin 11/18/10 and be performed each Thursday thereafter
• Leaf blowing will begin 11/18/10 and be performed each Thursday prior to the parking lot being swept
with the sweeper
• Meeting scheduled for November 17 2010
5 Record each instance of a Tier One response in the Stormwater Pollution Prevention Plan Include the date and
value of the benchmark exceedence the inspection date the personnel conducting the inspection, the selected
actions and the date the selected actions were implemented

Subject Report of Non -Compliance Incident at the Transit Center Facility
Dear Mr Edwards,
This letter is in reference to non-compliance of our NPDES requirements located at the Transit
Center The act noted was water carrying what appears to be an oil/petroleum product onto Park
Avenue from the Transit Center
On July 9, 2008, I received a call from. Wanda Fraizer NCDWQ in reference to a citizen s
complaint Upon calling the citizen I was informed that he smelled what he took to be a
oil/petroleum based product and that he saw an oil sheen on top of the water On July 10, 2008
visited the site with the general manager of the transit facility and noticed what appeared to be
residue from the oil sheen
In order to correct these issues, the City will determine if the oil/water separators are functioning
correctly On July 11, 2008 the Transit Center had the oil/water separators cleaned out
Currently, City staff is working to find a product to place in the flow line of the street to remove
any possible contaminates

In response to your renewal application for continued coverage under stormwater General Permit NCGO80000 the Division of
Water Quality (DWO) is forwarding herewith the reissued stormwater General Permit This permit is reissued pursuant to the
requirements of North Carolina General Statute 143 215 1 and the Memorandum of Agreement between the state of North
Carolina and the U S Environmental Protection Agency dated December 6 1583
The following information is included with your permit package
• A new Certificate of Coverage
• A copy of stormwater General Permit NCGO80000
• A copy of a Technical Bulletin for the General Permit
• Five copies of the Discharge Monitoring Report (DMR) Form
• Five copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater only and it specifies your obligations with respect to stormwater
discharge controls management monitoring and record keeping Please review the new permit to familiarize yourself with all
the changes in the reissued permit
The more significant changes in the General Permit since your last Certificate of Coverage include the following
• Part I Section A — A new notation that the No Exposure Exclusion from permitting option may be available to the
permittee
• Part I Section A — A new provision that facilities draining to 303(d) ftsted waters or in watersheds with an approved TMDL
may not be eligible for renewed coverage under the General Permit
• Part 11 Section B Table 1 — An increased sampling frequency from once per year to twice per year
• Part II Section B Table 3 — Benchmark concentrations now replace cutoff concentrations the elimination of the option to
avoid sampling based on cutoff concentrations
• Part li Section B — New provisions requiring the permittee to execute Tier One and Tier Two response actions based on
the first benchmark exceedence (Tier One) and the second consecutive benchmark exceedence (Tier Two) Tier Two
requires that the permittee institute monthly monitoring instead of twice per year monitoring until three consecutive
monitoring events show no benchmark exceedences
• Part II Section B — A new provision that four exceedences of any particular benchmark will trigger increased DWO
involvement in the permittee s stormwater management and control actions DWO may direct the permittee to apply for
an individual permit or may direct the implementation or installation of specific stormwater control measures
DWQ received a number of comments on a series of general permits similar to this one A full response to comments can be
