On March 22, 2017 the City of Greensboro Stormwater Management Division completed an
industrial compliance inspection of USF Holland. Local regulations permit an inspection of
industrial facilities under authority granted by City of Greensboro Code of Ordinance Chapter
30, Article 12, Section 7.1 (C-5). This inspection consisted of two parts: a review of the facility
files/records and an on -site facility inspection.
The inspection was performed by Sherri Scalcione, Ian Dunn and Zack Mackenzie. The following
deficiencies/observations were noted during the inspection.
Stormwater Pollution Prevention Flan/Spill Plait Review
Update the SWP3 plan annually, it is suggested that the review date and signature appear as
either an attachment page after the cover or on the cover page itself. Please refer to the permit
Part 11, Section A, item number 7 for the full description of the information the plan requires
updates to annually.
Site Maps and SWP3
• Once verified with the State all outfalls should be noted on the plan. See notes below
regarding monitoring deficiencies.
Spill Prevention and Response Plan
• No deficiencies observed.

Monitoring
• Monitoring has not been completed since 2013. Mr. Manuel states they have
contracted GEOS Environmental to get in compliance with this portion of the permit.
• The map used for the previous inspection in 2012 has 2 outfalls marked. Mr. Manuel
indicated the plan is to sample outfall #l, please coordinate with the State to ensure all
outfalls are monitored as required by the permit.
Employee Training
• No deficiencies observed
Site Inspection
Preventative Maintenance/Good Housekeeping
• No deficiencies observed.
Nun-Stormwater Discharges
• Mr. Manual stated that the lift station pump went out in January and last week the float
malfunctioned resulting in overflows. In addition to these events, the City has record
of this occurring in 2002 and again in 2010 during routine inspections of the dry
detention basin. Please contact Ron Boone with the State to ensure that USF Holland
is in compliance with the 02T rules (see email dated 3/23/17 for additional details).
Be advised that failure to report overflows wiff result in a notice of violation from both
the City of Greensboro and NCDEQ.
It is recommended that USF Holland take appropriate actions to correct the noted deficiencies. If

Your facility is currently covered for stormwater discharge under General Permit NCGO80000. This permit expires on
10/31/2012. The Division staff is currently in the process of renewing this permit. When a draft is available for
notice and public comment, it will be posted on our website at http://portal.ncdenr.org/web/wq/ws/su/public-notices.
Please continue to check this website to review and comment on proposed changes to the permit. Once the permit
is reissued, your facility would be eligible for continued coverage under the reissued permit.
To assure consideration for continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit. Enclosed you will find a Permit Coverage Renewal Application Form. The
application must be completed and returned by May 4, 2012. Letters confirming our receipt of the completed
application will not be sent.
Failure to request renewal within the time period specified may result in a civil assessment of at least $250.00.
Larger penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from your
facility without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1 and
could result in assessments of civil penalties of up to $25,000 per day.
"No exposure exclusion" is now available to industrial facilities in any of the categories of "storm water discharges
associated with industrial activity," (except construction activities). If you feel your facility can certify a condition of
"no exposure", i.e. the facility industrial materials and operations are not exposed to stormwater, you can apply for
the no exposure exclusion. For additional information contact the Central Office Stormwater Staff member listed
below or check the Stormwater Permitting Unit Web Site at http://portal.ncdenr.org/web/wq/ws/su/npdessw#tab-5.
If the subject stormwater discharge to waters of the state has been terminated, please complete the enclosed
Rescission Request Form. Mailing instructions can be found on the bottom of the form. You will be notified when
the rescission process has been completed.

determination that stormwater discharge outfall (SDO) 001 be granted representative outfall status for
stormwater outfall 002. Based on the information and maps provided and a site visit on Friday, June
27, 2008, we are approving this request. In accordance with 40 CFR §122.21(g)(7); you are authorized to
sample outfall number 001 as a representative outfall. This approval is effective with the next sampling
event. We also want to remind you that the permit still requires Qualitative Monitoring be
performed at all SDOs, regardless of representative status.
Please remember that any actions you initiate in response to benchmark exceedences as directed in the
tiered response provisions of your permit must address all drainage areas represented by SDO 001, where
appropriate.
Please append this letter to your Stormwater Pollution Prevention Plan (SPPP) to document that
representative outfall status has been approved. If changes in drainage areas, structures, processes, .
storage practices, or other activities occur that significantly alter the basis of this approval, you should

A Compliance Evaluation Inspection was performed on the USF Holland general stormwater permit (NCG080653) on
Friday, June 27, 2008 by Corey Basinger of the Winston-Salem Regional Office. Mr. Darin McClure, PE was also present
for the inspection.. The inspection consisted of two parts: an on -site inspection of the facility and an in -office file review.
The following are the findings from the subject inspection.
1. Permit
The NPDES general stormwater permit (NCG080653) became effective on November 1, 2007 and expires on October 31,
2012.
It. Records/Reports
The Stonnwater Pollution Prevention Plan (SPPP) was prepared by Mid -Atlantic Associates, Inc. and appeared complete
and well organized. A review of the reports revealed no discrepancies. A copy of the data is kept on file. All applicable
data is collected as per the NPDES general permit requirements.
Ili. Facility Site Review
The subject facility discharges stormwater pursuant to NPDES Permit NCG080653. The facility also discharges
wastewater from an oilAvater separator to the City of Greensboro sanitary Sewer system.
IV. Effluent & Receiving Waters
USF Holland discharges to an unnamed tributary to I-lorsepen Creek in the Cape Fear River Basin.
V. Self -Monitoring Program
USF Holland conducted the required qualitative and quantitative monitoring since 2007 when the SPPP was written.
VI. Operations and Maintenance
The following items must be addressed:
1) the oilJwater separator must be unclogged to ensure proper operation and to prevent an illegal discharge to
surface waters;
2) pallets should be stored under roof or be covered at all times;
3) it is suggested that curbs be added at all open bay doors at the maintenance shop and truck wash station. N6"°hCarolina

Inc. requests representative outfall status for the above referenced site. The facility is permitted to
discharge stormwater under the North Carolina Department of Environment and Natural
Resources, Division of Water Quality, General Permit NCG080000. Stormwater currently
discharges off site at two outfalls (See Attached Drawing 2.1). Potential pollutants associated with
stormwater_ discharges at each outfall include oils, greases, petroleum, vehicle fluids, and
suspended solids- Based on our recent evaluation of the site and its stormwater outfalls, it appears
that the stormwater discharge from Outfall 1 is representative of discharges from each outfall at the
site. It also appears that the discharge at Outfall 1 would be the "worst -case" discharge of the two.

Regional Supervisors,
At our last East/West Consistency Meetings, we asked the regions what they thought
about reviewing and approving/denying requests for Representative Outfall Status
(ROS). Up to now, we have handled these in the Central Office. The responses were
very positive, and we felt it was an appropriate transition because usually we rely
on a site visit by the region to verify conditions at the site support representative
outfall status.
We have set up a process for tracking requests and approvals, and with this e-mail we
are providing guidance for your staff to handle these requests. We plan to provide
additional guidance at future Consistency Meetings or other meeting opportunities,
but we wanted to get something out to you in the interim (which is why it's -noted as
"draft.") -- especially since we are already working with regional staff on some ROS
requests.
The tracking process is in the last part of the attached memo, but here's a summary
of what -regional -staff should do:
1. Copy the ROS request letter (and site plan if submitted) to the Central
Office/SPU.
2. Update RIMS under the Events Tab with the date for "Representative Outfall Status
Requested."
3.•A template letter of ROS approval is attached. Please feel free to modify it as
necessary.
4. If you deny the ROS request, list specifically the basis for denial. if the denial
is temporary, note the conditions that need to be corrected before a re -visit and
granting of ROS.
S. Be sure to remind the permittee that qualitative monitoring (visual and odor
characterization of the discharge) is still required _at all outfalls_. Also remind
the permittee that a tiered response (if required by their permit) must be
implemented in the other represented sub -drainage areas.
6. Copy the final ROS approval or denial letter to (1) Central Office/SPU and (2) DWQ
Central Files.
7. Update BIMS under the Events Tab with date for "Representative Outfall Status

At the last East/West Consistency Meetings with the Regional Offices, regional staff indicated their
willingness and preference to review and document approval of representative outfall status (ROS) requests from
stormwater permittees. Before now, these requests were processed by the Central Office (CO). However, in'the
past many requests were neglected or approvalldisapproval was left undocumented. Below is a draft checklist of
review points and steps for approving and tracking representative outfall status in BIMS. We will go over this
topic at either the next Consistency Meetings or other opportunity, but we wanted to provide guidance in the
interim. If a request comes to the regional office in the meantime that you would prefer CO staff to handle, we will
be glad to coordinate review with you until we can provide more training.
1. A word about a Representative Outfall Status request:
a) Our permittees may be eligible for representative outfall status based on the physical facts at their site
and on some other qualifying criteria. However, granting of ROS is at DWQ discretion. We owe the
permittee an evaluation but not a specific outcome. The SPU has routinely denied ROS based on review
of the submittal documents.or report from the regional office (RO).
2. Guidance on evaluating the initial request:
a) Does the permittee's letter identify a physical basis for the request? (in other words, does it state that the
character of the stormwater pollution, or the potential stormmater pollution or exposure is similar in the
sub -drainage areas under consideration?) REQUIRED, unless b. applies.
b) Or, if not very similar, has the permittee indicated that the representative outfall includes the sub -drainage
area with the most stormwater pollution, or the potential for the most pollution or exposure (i.e., the
"worst" sub -drainage area?) REQUIRED if a. does notapply.
c) While the objective of ROS is to reduce the monitoring burden on the permittee, reducing the permittee's
costs is absolutely conditional on establishing the representativeness of the outfall. We cannot grant
ROS simply because we think the permittee deserves.a financial break.
d) Has the permittee provided a site drawing to show which outfalls he/she is proposing to be considered
representative of which other outfalls, and which ones will continue to be sampled as single outfalls?

e) Consider the type of industrial activity. For example, landfills (NCG12) and mines (NCG02) have
changing footprints with perhaps higher potential for activities in a sub -drainage area to change, which
might affect our discretion. Also, ready -mix concrete (NCG14) and mines may have wastewater
discharges on site with the potential for the two waters to commingle, which might affect our discretion.
f) If the request is for an individual permit, are the monitoring parameters the same across all the outfalls
being considered as representative/represented?
g) Vehicle Maintenance Activity (VMA) outfalls may not be represented by non-VMA outfalls, and vice -versa.
3. Review the file materials and BIMS:
a) Receiving water classification? This may affect our discretion. For example, approving ROS for
discharges to ORW/HQW receiving waters may not be appropriate.
b) Receiving water special circumstances? Impaired? For what? Is there a TMDL? For what? Are there
threatened or endangered species? All could affect our.discretion.
c) Has the permittee paid up? Could withhold ROS until he/she has paid past -due amounts.
d) Past compliance history should affect our discretion. Check file and/or BIMS for violations, enforcement,
comments, history, etc. and consider how this should weigh into a ROS approval.
e) Has past monitoring been complete? If the permittee has ignored past monitoring, may consider denying
ROS approval until there is enough monitoring data to support the assertion of similar or less stormwater-
pollution potential in the sub -drainage areas to be represented. How much is "enough"? Ideally a
minimum of eight (8) to 10 samples, but may consider fewer if other evidence supports ROS request.
4. Visit the site:
a) Do the physical facts verify the characterization that one outfall should be considered representative of
another?
b) Check out the Stormwater Pollution Prevention Plan (SPPP) — is it descriptive of site conditions? Is the
permittee using the SPPP as a management tool and living record? Does it demonstrate that the
permittee is actively aware of and managing stormwater discharges from the site? If you think it is
warranted, consider requiring an updated/sufficient SPPP before continuing review of the ROS request.
c) How is general housekeeping? if poor, consider whether the operator can confidently assert that one
outfall is representative of others.
d) Try to pre-empt any misunderstanding about ROS: it allows the analytical monitoring at one outfall to be
considered as representative of other outfalls. ROS does not change the monitoring frequency required;
nor does it change the required monitoring parameters; nor does it reduce the qualitative monitoring
required at all outfalls.
e) You may also need to explain that the final determination is based on the physical facts and DWQ's
discretion. We are not obligated to approve a ROS request.
_ f) Emphasize with the permittee that if the permit requires a tiered response to benchmark exceedences,
then an exceedence at the representative outfall requires a response action in ALL of the sub -drainage
areas represented by the one outfall.

On January 11, 2007, the City of Greensboro Stormwater Management Division completed an industrial
compliance inspection of the USF Holland terminal. Local regulations permit an inspection of industrial facilities
under authority granted by City of Greensboro Code of Ordinance Chapter 27, Article 1, Section 27-9(e). This
inspection consisted of two parts: a review of the facility files/records and an on -site facility inspection.
The inspection was performed by Michael Borchers and the following deficiencies/observations were noted during
the inspection.
Site Plan and Stormwater Pollution Prevention Plan
♦ The facility does not have a Stormwater Pollution Prevention Plan as required by NCDENR DWQ
General Permit No. NCG080000.
♦ The facility inspection program is inadequate. The last facility inspection was performed on January 3,
2005. The facility is required to inspect all Stormwater systems on at least a semi-annual schedule.
Spill Prevention and Response Plan
♦ The facility does not have a Spill Prevention and Response Plan as required by NCDENR DWQ General
Permit No. NCG080000.
♦ The facility does have some spill cleanup materials however the materials are inadequate for
containment and not readily accessible.
Preventative Maintenance and Good Housekeeping
♦ A program should be implemented to provide for routine trash pick up around the perimeter of the site.
♦ Numerous vehicle fluid leaks evident, particularly around the vehicle maintenance building. A good
housekeeping program should be promptly implemented to contain vehicle fluid leaks/spills.
Employee Training
♦ Since the permit was issued in September 2000, no employee training has occurred on spill response
