On January 31, 2013, Jenifer Carter, of the Division of Water Quality, performed a stormwater inspection at
Rex Oil Company located on Lexington Avenue in Thomasville, North Carolina. You were present during the
inspection. The following observations were noted during the inspection (Action Items are Underlined):
Permit
The General Permit became effective on November 1, 2012, and expires on October 31, 2017. A signed
Certificate of Coverage is maintained on site. An outside contractor is hired to perform all duties as they relate
to the general stormwater permit monitoring and records requirements. Maintenance activities are performed
by Rex Oil staff.
Stormwater Pollution Prevention Plan SPPP
• Any changes to the SPPP, or the lack of anv changes needed, must be documented annually. Currentiv. the
SPPP needs to be updated to include new above- ound storage tanks ASTs .
• Annual staff training is implemented during safety training events.
• Secondary Containment has been added around all ASTs since the last inspection. Amend the SPPP to
ensure that secondary containment is evaluated for the presence of contaminates prior to release of
stormwater. Contaminated stormwater should not be released to the storm drain system. but disposed of in
accordance to Federal, State and Local laws.
Analytical Monitoring
• Analytical monitoring of stormwater was performed at both stormwater outfalls as required by the permit.
• Results from both outfalls since the last inspection revealed several exceedences of permit limits for TSS, as
well some exceedences of the Oil and Grease limit.
➢ All results must be submitted to the Division, within 30 days of receiving results from the lab. to the
address noted on the Discharge Monitoring Form (DMR).
➢ And -compliance with the limits should be noted on the DMR and in the SPPP, with causes and
corrective actions noted.

➢ Any non-compliance with the permit that endangers health or the environment requires you to contact
DWQ within 24 hours of knowledge of the non-compliance.
o January 14, 2013 monitoring did not incorporate the change from Oil and Grease to Non -Polar Oil & Grease
as implemented during the last permit renewal. Please arrange to have another sample pulled and tested
using the correct test method (EPA Method 1664) prior to the end of June 2013.
Qualitative Monitoring
o Qualitative Monitoring of both stor nwater outfalls was performed at the frequency required by the permit,
though monitoring for both outfalls was recorded on one form. Use a separate form for each outfall.
o On July 25, 2011, the monitoring form for outfall #2 noted no problems; however, the analytical monitoring
form showed substantial limits exceedences for Total Suspended Solids (TSS). Make sure qualitative
monitoring is performed at the same time as anal3jical monitoring, and during a rain event.
The Division acknowledges improvements since the ' last inspection, most notably, the addition of secondary
containment around all ASTs. Your continued efforts to obtain and maintain compliance with your stormwater

Dear Mr. Tedder:
In response to your letter of October b, 2008, item V. Other Observations, please find
attached our plans for retainment wall expansion to provide secondary containment for
the industrial lubricant AST's located at our facility on Lexington Ave., Thomasville,
NC. The secondary containment construction for the AST's was in progress, but was
det-erred due to engineering plans from our- main supplier, Exxon, for repackaging
considerations. Continuation of construction will resume with 30 days and should be
completed this quarter. We hope to be able to comply with secondary containment
regulations and our suppliers' requirements without unnecessary duplication or possibly
having to go back to the beginning.
Please let me know if there are further considerations we should make on these plans. We
try to maintain compliance at our facility and appreciate your cooperation in this mutual
pursuit.

On September 24, 2008, Jenifer Carter, of the Division of Water Quality, performed a stormwater inspection
at Rex Oil Company located on Lexington Avenue in Thomasville, North Carolina. You and Mr. Harold
Hawkins were present during the inspection. The following observations and/or permit conditions were
noted during the inspection:
I. Permit
The General Permit became effective on November 1, 2007, and expires on October 31, 2012. A signed
Certificate of Coverage is maintained on site. An outside contractor, Ms. Lynn Hayes, is hired to perform all
duties as they relate to the general. stormwater permit monitoring and records requirements. She does not
perform required maintenance activities.
II. Storm'+eater Pollution Prevention Plan (SPPP)
A SPPP has been developed for this facility.
• An up-to-date Site Plan was available for review.
• A Stormwater Management Plan is included and has been carried out as described.
• A Spill Prevention and Response Plan has been implemented.
• Preventative Maintenance and Good Housekeeping practices are included and implemented.
I13. Qualitative Monitoring
Qualitative Monitoring of both stormwater outfalls is performed as required by the permit. There is a third
stormwater outfall on the property; however, since it drains mostly an offsite DOT maintained road and only
a small portion of the front lawn, it does not need to be included in the monitoring program.
IV. Analytical Rlonitorin
There is not vehicle maintenance performed or coordinated from this site, therefore a tiered response to any
lab results surpassing the benchmark values in the permit does not apply.
Analytical monitoring of stormwater was performed at both stormwater outfalls as required by the permit for
outfalls that drain areas containing oil -,eater separators, petroleum bulk stations and/or petroleum terminals.
Note: Please refer to Part III Section E for Discharge monitoring*Reporting Requirements.

V. Other Observations:
There were several single -hulled Above -ground Storage Tanks (ASTs) with capacities ranging from 2,000 to
15,000 gallons each for which no secondary containment was provided. Each of these-ASTs-contains
hydraulic/lubrication oils. These tanks were installed prior to Rex Oil Company taking ownership of the
property. Spill kits are available.
Action Items_- Response required within 14 business days of receipt of this letter
• Describe actions that have been or will be taken, including a timeline, to provide secondary containment
for all ASTs. Whether or not enforcement action will be taken by the Division will be depend on the .
submitted plan of action. as well as the timeline.
• Update your SPPP to account for any new secondary containment or other BMPs put in place as a result
of this inspection.

The tanks are suitable for the product stored and labeled UL-142/58 or certified by industry standards at the time
of installation. Emergency relief venting consists of a manufactured emergency vent to relieve at approximately 2.5
psig. pressure (or refer to liftable manhole covers, large diameter vents or weak roof to shell welds if either of these
alternatives is used). Each tank must have a lockable valve on its main flow connection. Overfill prevention must be
installed and engineered to nearly a fail-safe status. Overfill prevention must be tested at regular intervals.
The tank configuration is constructed to drain by a secondary containment system as described under
"Containment and Drainage Control Structures," page 8. Tanks are manually or taped gauged or electronically gauged,
for product level, before delivery of petroleum products. All underground tanks meet Federal and State leak detection
regulations.
Visible oil leaks from sources such as tank seams, gaskets, rivets, and bolts and sufficiently large enough to
cause oil accumulations, will be removed within 24 hours from the time the accumulation occurs, in accordance with the
SPCC Regulations. All leaks must be corrected as soon as possible.
Tank and piping will be visibly inspected. Monthly records will be kept using the form "Inspection, Tests &
Records" on page 10. Integrity testing must be done In accordance with standard engineering practices. In accordance
with API standards 650 and 653, petroleum tanks over 50,000 gallons in size are required to be integrity tested every ten
years or when repairs are made. Aggregate storage capacity of warehouse is 100,000 gallons. Total aggregate tank
capacity is 279,000 gallons.
