Geoscience & Technology, P A (GeoSci), on behalf of Yarbrough Transfer Company (Yarbrough),
is pleased to submit the enclosed annual storrnwater monitoring data for the above -mentioned
facilities for November 2010, pursuant to general storrnwater certificates of coverage # NCG080195
and #NCG080449 Yarbrough's offices and truck maintenance facility are located at 1500 Doune
Street, Winston-Salem, NC (certificate NCG080195) Their trailer maintenance facility is located at
181 Fayetteville Street, Winston-Salem, NC (certificate NCG080449) The Stormwater outfalI at the
Doltne Street facility is designated as outfall 002 and the outfall at the Fayetteville Street facility is
designated 003 The Environmental Manager at Yarbrough is David Yarbrough (336 725 7552)

Peter H DeVries, Project Geologist, with GeoSci, pertoi tried the storrnwater sample collection and
prepared the Qualitative and Quantitative Monitoring Reports Mr Yarbrough reviewed and signed
the SDO MR Mr Yarbrough maintains a file of the Qualitative and Quantitative reports at the
Doune Street facility

The sampled storm event began in the early morning hours of Tuesday, November 30, 2010, and
continued into the early morning hours of Wednesday, December 1, 2010 Rau} was relatively
steady but light throughout Tuesday and runoff did not start until late afternoon Runoff began at
approximately 1645 on November 30`h at the Doune Street facility and a sample was collected at
1730 The onsite rain gauge indicated that 0 15-inches had fallen up to that point Runoff began at
approximately 1715 on November 30" at the Fayetteville Street facility and a sample was collected
at 1745 The onsite rain gauge indicated that 0 15-inches had fallen up to that point The rain gauge
at the Doune Street facility indicated at total of 0 6-inches on the morning of December 1, 2010
The rain gauge at the Fayetteville Street facility indicated at total of 0 5-inches on the morning of
December 1, 2010 The previous storm event ended on the morning of Friday, November 26, 2010,
4 days or approximately 93 hours prior to the sampled event
The attached Tables 1 and 2 summarize the pervious/impervious and drainage areas for the
designated outfalls Runoff calculations for the November 2010 event are also documented in the
tables The calculations indicate a total runoff of 22,318 gallons (0 02 MG) through outfall 002 and
9,994 gallons (0 01 MG) through outfall 003
The laboratory test results indicate concentrations foi all parameters are below benchmark values
for both locations

On behalf of Yaibrough Transfer Company (YTC), Geoscience and Technology, P A (GeoSci) is
pleased to submit this response to the subject Notices of Violation (NOVs) The NOVs were issued
for the above referenced facilities for the absence of Stormwater Pollution Prevention Plans (SP3s)
and no analytical or qualitative monitoring since 1999 YTC has retained GeoSci to prepare SP3s for
both facilities GeoSci intends to also prepare Spill Prevention and Response Plans (SPRPs) or Spill
Prevention Control and Countermeasures plans (SPCCs), as required GeoSci anticipates completion
and submittal of these plans by July 15, 2008
Attached to this document are copies of the laboratory analytical results for three stormwater samples
collected at the Doune Street facility on May 16, 2008 These results indicate concentrations of the
analytical parameters within (pH) or below (TSS and 0&G) the limits specified in the permits The
samples were collected and analyzed by Research and Analytical Laboratories (R&A) in Kernersville,
NC Attached also are the Qualitative Monitoring reports for the three outfalls prepared by the sample
collector
In conjunction with preparation of the SP3s and SPRPs, GeoSci is reviewing the current and
historical sample collection and analyses to correlate sample collection locations with the analytical
results The results and any recommendations arising from our review will be incorporated into the
plans, as necessary
YTC appreciates the courteous nature of Mr Marc Stokes and his willingness to assist YTC in
responding to the stormwater violations It is YTC's intention to comply with the stormwater
regulations and sincerely apologizes for their oversights Based on the current and historical

analytical results it does not appear that YTC has contributed to stormwater pollution and does not
intend to do so in the future
