On January 24, 2017 Glen White of the North Carolina Department of Environmental Quality —
Division of Energy, Minerals and Land Resources met with you at the facility located at 3255
Maple Avenue in Burlington, to conduct a compliance inspection. You recently became
responsible for maintaining the facilities environmental permits. This visit was requested by you
to help you determine what would be needed to ensure compliance with the facilities industrial
stormwater permit. Inspection consists of review of the the SPPP (Stormwater Pollution
Prevention Plan), facility outfalls, and any possible exposures. Your assistance with inspection
was greatly appreciated.
Permit:
This facility holds General Stormwater Permit NCG080431 to discharge stormwater from
industrial activity associated with Vehicle Maintenance Areas under the National Pollutant -
Discharge Elimination System (NPDES). The permit became effective November 01, 2012 and
expires on October 31, 2017. The current permit was available for review and was included in
the SPPP.
Records/Reports:
This facility was required to develop and maintain a Stormwater Pollution Prevention Plan
(SPPP) in accordance with Part II, Section A of the permit. All qualitative and analytical
monitoring records are required to be maintained with the SPPP for a minimum of five (5) years.
Record review revealed that the SPPP had not been fully implemented by the prior responsible
party. Each Item of Part II, Section A needs to be addressed and included in the new SPPP.
Responsible parties list, stormwater training, spills records, operating procedures, changes to the
facility or the site itself all need to be updated at least once each year. (A new copy of the current
Permit is included with this submission.)

Facility Site Review:
This facility performs maintenance on tractor trailers. Mr. Goras walked the facility with the
inspector and the outfalI and possible exposures were observed. There was no obvious
deficiency with the condition of the site itself
Effluent Receiving Waters:
Effluent from this facility drains to a small pond behind the property and discharge reaches an
unnamed tributary of Little Lick Creek, Class "C" waters of the Cape Fear River Basin.
Self -Monitoring Prooyram:
This facility is required to provide Qualitative and Analytical Monitoring twice annually.
Collection of analytical samples should be taken at the same time qualitative monitoring is
performed. Collect samples during a qualifying rain event during the first 1/2 hr of flow from the
outfall.
The facility is not currently compliant with its industrial stormwater permit. As we discussed,
please begin working on the SPPP and make sure each required component is included. If you

Inspection Summary:
NCDEQ - DEMLR Inspector, Glen White, met with Mr. Ron Goras at his request on January 24, 2017. Mr. Goras has
taken over EHS (Environmental Health & Safety) and is trying to bring the facility into compliance with its pdrmits. The last
documented inspections at Carolina Tankk Lines received NOV's (Notices of Violations). No enforcement fines or fees were
assessed.
The former EHS Director never prepared a SPPP and only began monitoring as per the permit after NOV's. The facility is
currently conducting Analytical monitoring bi-annually as required but is lacking qualitative records. Mr. Goras will begin
keeping Qualitative records. Both can be done at the same time.
Mr. Goras will contact the inspector to visit the site and review the SPPP when he feels he has it ready for review. Full site
inspection likely to occur again at that time.

Comment: Since the last NOV. analytical monitoring has been conducted as required. Samples appear to
test within the requiredparameters consistently although the collection location is not ideal. It is
a plunge pool at the lowest location. Inspector told Mr. Goras it would be acceptable to collect at
that location as long as it was done during the first 30 minutes of rainfall event flow at the
location.

records have been conducted as required and result within required parameters.

This letter is in response to the Notice of violation which was the result of a visit by Ms. Jenny
Graznak on December 14, 2010. First let me express my apparition to Ms Graznak for reviewing
the permit and grounds and helping me understand what I need to do to get this program current.
On January 3, 2011 I submitted water samples taken from the holding pond to Microbac
Laboratories to satisfy the Analytical monitoring Requirements. To date I have not received the
results.
On' 12-18-2010 the Spill prevention plan was revised and is now up to date and current.

Dear Mr. Isley:
On December 14, 2010, Jenny Graznak of this office performed a Stormwater Permit
Inspection at Carolina Tank Lines, Inc. located at 3255 -Maple Avenue in Burlington, North
Carolina. This facility holds General Stormwater Permit No. NCG080431 under the National
Pollutant Discharge Elimination System (NPDES) to discharge Stormwater from facilities with
Vehicle Maintenance Areas. As Safety Director for the facility, you were present for the
inspection. This type of inspection consists of two basic parts: an in -office review of facility files
and self -monitoring data and -an on -site inspection of the facility's discharge. This particular
inspection evaluated five (5) areas, and observations from each area are addressed below:
1. Permit
The permit became effective November 1, 2007 and expires October 31, 2012. A copy of the
permit was available for review during the inspection.
2. Records/Reports
A consultant prepared a Stormwater Pollution Prevention Plan (SPPP) dated 2005 that was
available during the inspection for review. However, your SPPP should be updated to reflect
changes to the site. In particular, you will need an accurate site map that includes new
buildings and other changes on the site. Please read Part II, Section A of the Permit, which
details all of the required sections of the SPPP.
3. Facility Site Review ,
The site has two underground diesel storage tanks of 10,000 gallons each. The facility also has
a 1,000-gallon aboveground tank for used motor oil generated at the site. Noble Oil Services,
Inc. owns the tank and routinely picks up used oil for the facility during the summer, but it is
burned in the shop furnace during the winter months. The tank has secondary containment. All
55-gallon barrels are maintained inside the shop. Also stored in the back lot are used pallets, a
covered dumpster, and a open metal scrap dumpster.

4. Effluent/Receiving Waters
Stormwater from the site drains to a small pond behind the property and discharges from there.
Stormwater from the property discharges to an unnamed tributary to Little Lick Creek in the
Cape Fear River Basin.
5. Self -Monitoring Program
It was determined during the inspection that you have not been performing the self -
monitoring requirements of the General Permit. Permit text Part II Sections B and D
describe the monitoring requirements that are applicable to your facility. Section B describes
semi-annual analytical monitoring requirements. Section D describes semi-annual qualitative
monitoring requirements. Qualitative monitoring records should be maintained at the facility and
made available to Division of Water Quality staff upon request. However, analytical monitoring
reports should be forwarded to the Division of Water Quality.
The failure to comply with the abovementioned requirements is a violation of the
General Permit. Therefore, this inspection report is being sent as a Notice of Violation.
According to permit text Part Ill Section A, "the permittee must comply with all conditions of this
permit. Any permit noncompliance constitutes a violation of the Clean Water Act and is grounds
for enforcement action; for permit termination, revocation and reissuance, or modification; or
denial of a permit upon renewal application." Under state law, a daily civil penalty of not more
than twenty-five thousand dollars ($25,000) per violation may be assessed against any person
who violates or fails to act in accordance with the terms, conditions, or requirements of a permit.

You must respond in writing to this office within 15 days of receipt of this letter,
advising us of the actions taken to abate the violations. Failure to do so will result in a
recommendation by this office in favor of the pursuit of formal enforcement actions against you.
Please include in your response a detailed description of why these violations occurred and how
they will be prevented in the future.. If you have any questions concerning this letter or the

2. Records/Reports
A consultant prepared a Stormwater Pollution Prevention Plan (SPPP) dated 2005 that was available during the
inspection for review. However, your SPPP should be updated to reflect changes to the site. In particular, you will need
an accurate site map that includes new buildings and other changes on the site. Please read Part 11, Section A of the
Permit, which details all of the required sections of the SPPP.
3. Facility Site Review
The site has two underground diesel storage tanks of 10,000 gallons each. The facility also has a 1,000-gallon
aboveground tank for used motor oil generated at the site. Noble Oil Services, Inc. owns the tank and routinely picks up
used oil for the facility during the summer, but it is burned in the shop furnace during the winter months. The tank has
secondary containment. All 55-gallon barrels are maintained inside the shop. Also stored in the back lot are used pallets,
a covered dumpster, and a open metal scrap dumpster.
4, Effluent/Receiving Waters
Stormwater from the site drains to a small pond behind the property and discharges from there. Stormwater from the
property discharges to an unnamed tributary to Little Lick Creek in the Cape Fear River Basin.
5. Self -Monitoring Program
It was determined during the inspection that you have not been performing the self -monitoring requirements of the
General Permit. Permit text Part II Sections B and D describe the monitoring requirements that are applicable to your
facility. Section B describes semi-annual analytical monitoring requirements. Section D describes semi-annual qualitative -
monitoring requirements. Qualitative monitoring records should be maintained at the facility and made available to
Division of Water Quality staff upon request. However, analytical monitoring reports should be forwarded to the Division
of Water Quality.
