Based on the information and maps provided as well as a site visit by Ron Boone, of this office, on February 17th, 2011, we are approving this request. In accordance with 40 CFR §122.21(g)(7), you are authorized to sample outfall number 007 as a representative outfall. This approval is effective with the next sampling event. We also want to remind you that the permit still requires Qualitative Monitoring be performed at all SDOs, regardless of representative status. Also, please remember that any actions you initiate in response to benchmark exceedences as directed in the tiered response provisions of your permit must address all drainage areas represented by SDO 007, where appropriate. Please append this letter to your Stormwater Pollution Prevention Plan (SPPP) or permit to document that representative outfall status has been approved. Ron Boone of the Winston-Salem Regional Office of the NC Division of Water Quality (DWQ) conducted a compliance evaluation inspection (CEI) at the subject facility on February 17tn, 2011. The assistance and cooperation of Pete Navarro, Cliff Evans and James Frei during the inspection, was greatly appreciated. An inspection checklist is attached for your records and the inspection findings are summarized below. 2. The facility is located at 1255 NC Hwy 66 South, in Kernersville, Forsyth County, North Carolina. Stormwater related to industrial activity is discharged from the site to the West Fork Deep River, which is currently classified as Class WS-IV (water supply) waters in the Cape Fear River basin. Documentation & Monitoring Review 3. The Stormwater Pollution Prevention Plan (SP3) is complete, current and has been implemented. You are reminded of the annual SP3 reviews and everything the permit requires as part of the annual reviews. Please review that section of the general permit in detail. Also, the site plan only reflects four oil/water separators (OWS), but in actuality there are six. Please update the site,.plan to show all six OWSs. Lastly, please begin keeping records of the maintenance of the O11VSs, including documentation of when you have the units pumped out. Training records were excellent and the actual SP3 was very well written. 4. The facility has performed analytical monitoring (AM) at stormwater discharge outfall (SDO) 007 only. This is the result of a request for Representative Outfall Status (ROS) submitted to the Division in 2008. A letter from the Division approving the ROS request was not available at the time of the inspection and is not in our files. However, Mr. Boone has agreed, in accordance with the 2008 request, that the sampling of SDO 007 is representative of the other SDOs (003, 005 and 006). Site Review 6. The site was very clean and well maintained. Mr. Boone noted no violations or discrepancies. It is believed that you have minimized exposure of industrial activities to the maximum extent practical. 7. You are reminded that violations of the NCGO80000 general permit are subject to civil penalty assessments not to exceed $25,000 per day, per violation. The following are our reasons for choosing SDO-007: SDO-007 has a variety of industrial activity within its drainage area, consisting of a portion of the terminal docks, the truck garage, covered service/ wash bay, fueling station, and tractor parking. Its drainage area is 939,444 sf and is 95% impervious. The end of the pipe is easily and safely accessible. We want to monitor this outfall because it gives a good representation of BMP effectiveness in the vicinity of the Garage. SDO-003 drainage area is limited to the terminal docks and trailer parking. SDO-006 drainage area, the smallest at this facility, is limited to trailer parking. SDO-005 drainage area is limited to terminal docks, trailer parking, and a portion of the facility maintenance shop. This outfall discharges from a wet detention pond, and usually has a flow throughout the year due to groundwater infiltration into the outfall's internal pipes. We have sampled the discharge from SDO-005 twice during the past Permit term; both times the parameters were significantly less than the benchmark values. We believe qualitative monitoring will suffice to detect any pollutant problems at SDO-005. In our opinion, SDO-007 is the preferable choice for analytical monitoring. There are no outfalls labeled SDO-001, SDO-002, or SDO-004. An old map indicated these points, but our field investigation showed that they were not separate point source discharges. We kept the same nomenclature for continuity.
