Dear Mr. Adkins:
On April 26, 2016 the City of Greensboro Stormwater Management Division completed an
industrial compliance inspection of Averitt Express. Local regulations permit an inspection of
industrial facilities under authority granted by City of Greensboro Code of Ordinance Chapter
30, Article 12, Section 7.1 (C-5). This inspection consisted of two parts: a review of the facility
files/records and an on -site facility inspection.
The inspection was performed by Sherri Scalcione and McKenzie Smith, and the following
deficiencies/observations were noted during the inspection.
Stormwater Pollution Prevention Plan/Spill Plan Review
Site Maps and SWP3
• Update the SIAT3 plan annually, it is suggested that the review date and signature
appear as either an attachment page after the cover or on the cover page itself. Please
refer to the permit Part II, Section A, item number 7 for the full description of the
information the plan requires updates to annually.
• All contact information should be kept up to date.
• Verify during the next annual review of the plan that the site plan has all items shown
as listed in the permit Part II, Section A, item number 1(c). Specifically the receiving
waters (unnamed tributary of the East Ford Deep River) should be listed on the site
plan.

• Establish a procedure for the release and documentation of those releases from the
secondary containment around the diesel fuel tanks. Please refer to Part II, Section
A, item number 2 (b) for the details and record keeping that goes along with this
procedure. This section requires a locking mechanism for the valve, verify that the
valve does have a locking mechanism
Non-Stormwater Discharges
♦ No deficiencies observed.

Aana Taylor -Smith of the Winston-Salem Regional Office of the NC Division of Energy, Mineral, and Land
Resources (DEMLR) conducted a compliance evaluation inspection at the subject facility on 17 April 2014. This
inspection was conducted as a result of an anonymous complaint DEMLR received regarding a suspected illicit
discharge from the site. Your assistance and cooperation were greatly appreciated during the inspection. An
inspection checklist is attached for your records and the findings are summarized below.
This facility is located at 8814 West Market Street in Colfax, Guilford County, North Carolina. Stormwater related
to industrial activity is discharged from the site to an unnamed tributary of the East Fork Deep River, this segment
of which is currently classified as Class WS-IV waters, in the Cape Fear River Basin.
Documentation & Monitoring Review
The facility's Stormwater Pollution Prevention Plan (SPPP) is thorough, up to date, and fully implemented.
Qualitative and analytical monitoring have been completed as required. Analytical monitoring data is kept at the
corporate headquarters, but is readily accessible when needed. The facility has one Stormwater Discharge Outfall
(SDO) located at a small pond at the rear of the property.
Site Review
During the inspection, no evidence of illicit discharge could be found. Averitt Express Inc. has implemented many
measures to avoid stormwater contamination, including moving all vehicle maintenance indoors and berming the
building to prevent potential spills from reaching the parking lot. The facility emphasizes safety and good
management practices to prevent loss of fuel or other substances from the site. The secondary containment area
was found to be sufficiently sized and in good order. The outfall was inspected and found to be in excellent
condition. No evidence of erosion or instability at the SDO was observed, and the receiving stream was free of
pollution indicators. Averitt Express Inc. is considered in compliance at this time.

A Compliance Evaluation Inspection was performed on the Averitt Express, Inc. stormwater
facility on March 8, 2011 by Sue White of the Winston-Salem Regional Office. You were in
attendance. The inspection consisted of two parts: an on -site inspection of the stormwater
facilities and an in -office file review. The following are the findings from the subject inspection.
I. Permit
The NFPDES permit for Averitt Express, Inc. became effective on November 1, 2007 and expires
on October 31, 2012. Application for permit renewal must be received no later than 180 days
prior to expiration date.
II. Records/Reports
The Stormwater Pollution Prevention Plan (SPPP) was reviewed during the inspection. The plan
contained the necessary information required by NIPDES Permit No. NCG080392.
III. Facility Site Review
All stormwater conveyances and outfalls noted in the Stormwater Pollution Prevention Plan were
inspected and found to be satisfactory.
IV. Effluent & Receiving Waters
Averitt Express, Inc. discharges stormwater to an unnamed tributary East Fork Deep River (Cape
Fear River Basin). All stormwater conveyances discharge into a small pond at the rear of the
property. The discharge from this pond is the only stormwater outfall for the site. Please ensure
that all sampling take place at the outfall pipe and not directly front the pond.

V. Self -Monitoring Program
Part 11, Sections B of NPDES Permit No. NCGO80392 requires the Permittee to perform annual
analytical sampling for the parameters found in Table 1 (Section B). You indicated that
analytical sampling had been conducted but did not have the records on site. Permit
requirements are to retain all monitoring records on site for a period of.5 years. Within 30 days
of receipt of this letter, please provide copies of your monitoring records for the last 5 years
and send them to this office, attention: Sue White. Part II, Section D requires qualitative
monitoring on a semi annual basis, once in the spring and once in the fall. Your records showed

This letter is to inform you that, as of the date of this letter, the Division of Water Quality has not received a
renewal request for the subject permit certificate of coverage. This is a violation of NCGS § 143.215.1 (c)(1) which
states "All applications shall be filed with the commission at least 180 days in advance of the date on which it is
desired to commence the discharge of wastes or the date on which an existing permit expires, as the case may be".
Any permittee that has not requested renewal at least 180 days prior to expiration or permittee that does not have a
permit after the expiration and has not requested renewal at least 180 days prior to expiration, will be subjected to
enforcement procedures as provided in NCGS § 143-215.6 and 33 USC 1251 et. seq.
In order to prevent continued, escalated action, including the assessment of civil penalties you must submit a
completed permit coverage renewal application to the attention of the "Stormwater and General Permits Unit" at
the letterhead address within ten (10) days of your receipt of this letter (renewal application enclosed). If the
subject discharge has been terminated, please complete the enclosed rescission request form_ Mailing instructions
are listed on the bottom of the form. You will be notified when the rescission process has been completed.
