SUBJECT: NOTICE OF VIOLATIONS/Compliance Evaluation Inspection
Wilson Trucking Corp. -Greensboro NPDES Permit No. NCGO80322,
Guilford County
Dear Mr. Herndon:
A Compliance Evaluation Inspection was performed on the Wilson Trucking Corp. -Greensboro
stormwater facility on March 8, 2011 by Sue White of the Winston-Salem Regional Office.
Mike Uselman from Wilson Trucking, Greensboro was in attendance. The inspection consisted
of two parts: an on -site inspection of the stormwater facilities and an in -office file review. The
following are the Endings from the subject inspection.
I. Permit
The NPDES permit for Wilson Trucking Corp. -Greensboro became effective on November 1,
2007 and expires on October 31, 2012. Application for permit renewal must be received no later
than 180 days prior to expiration date.
II. Records/Reports
Part II Section A of permit NCGO80322 requires that a Stormwater Pollution Prevention Plan -
(SPPP) be implemented and maintained on site. This is typically reviewed during the inspection.
The Stormwater Pollution Prevention Plan was reviewed and contained the necessary data.
III. Facility Site Review
All stormwater conveyances and outfalls noted in the Stormwater Pollution Prevention Plan were
inspected and found to be satisfactory. It is noted that one outfall listed is gone due to high«ay
construction. It is no longer necessary to monitor that outfall.
IV. Effluent & Receiving Waters
Wilson Trucking Corp. -Greensboro discharges stormwater to Long Branch River (Cape Fear
River Basin).

V. Self -Monitoring Program
Part II, Sections B of NPDES Permit No. NCG080322 requires the Permittee to perform semi-
annual analytical sampling for the parameters found in Table 1 (Section B). There was no
record of any analytical sampling done on this site. Within 30 days of receipt of this letter,
please provide the name of the person/company that will be responsible for sampling the
outfalls_ In addition, after a significant rain event but prior to June 30, 2011, pleaseprovide
sample results from a qualified lab to this office, attention: Sue White. All outfalls excluding
the one under the highway must be sampled and all results provided.
Part 111, Section D, paragraph 5 discusses Representative Outfall Status. This means that if
sampling of all the outfalls show results are substantially identical, that sampling requirements
may be performed at a reduced number of outfalls. For assistance in acquiring Representative
Outfall Status, contact Sue White.
Part 11, Section D requires qualitative monitoring on a semi-annual basis, once in the spring and
once in the fall. Mr. Uselman has been doing the qualitative monitoring but has not been making
a record of the results. Sue White discussed with Mr. Uselman the form in which these results
should be recorded and suggested that a digital photo with a date stamp would be a useful tool in
documenting these inspections, For all future qualitative inspections, please ensure that
proper documentation protocol is followed, Records far both analytical and qualitative
monitoring must be kept on -site, with the Stormwater Pollution Prevention Plan, for a period
of 5 years. Qualitative monitoring can be done at the same time as the analytical monitoring.
A rain gauge must be on site in a permanent location to record rainfall amounts during the
sampling significant rain fall event. At the time of the inspection no rain gauge was on site.
Please note that failure to adhere to permit regulations could -result in an Enforcement with
possible fines of $25,000.00 per day per violation.
Mr. Uselman appears to be a conscientious person who would benefit from proper training in
permit requirements and procedures.

pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between the state of North Carolina and the U.S. Environmental Protection Agency, dated December 6, 1983.

Your coverage under this general permit is not transferable except after notice to DWQ. The Division may require
modification or revocation and reissuance of the Certificate of Coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by DENR or relieve the permittee from responsibility
for compliance with any other applicable federal, state, or local law, rule, standard, ordinance, order, judgment, or
decree.

This letter is to inform you that, as of the date of this letter, the Division of Water Quality has not received a
renewal request for the subject permit certificate of coverage. This is a violation of NCGS § 143.215.1 (c)(1) which
states "All applications shall be filed with the commission at least 180 days in advance of the date on which it is
desired to commence the discharge of wastes or the date on which an existing permit expires, as the case may be".
Any permittee that has not requested renewal at least 180 days prior to expiration or permittee that does not have a
permit after the expiration and has not requested renewal at least 180 days prior to expiration, will be subjected to
enforcement procedures as provided in NCGS §143-215.6 and 33 USC 1251 et. seq.
In order to prevent continued, escalated action, including the assessment of civil penalties you must submit a
completed permit coverage renewal application to the attention of the "Stormwater and General Permits Unit" at
the letterhead address within ten (10) days of your receipt of this letter (renewal application enclosed). If the
subject discharge has been terminated, please complete the enclosed rescission request form. Mailing instructions
are listed on the bottom of the form. You will be notified when the rescission process has been completed.
