Randolph County Dear Mr. Sykes: CENTRAL FILES DWQIBOG Aana Taylor -Smith of the Winston-Salem Regional Office of the NC Division of Energy, Mineral, and Land Resources (DEMLR) conducted a compliance evaluation inspection at the subject facility on 6 August 2014. Your assistance was greatly appreciated. An inspection checklist is attached for your records and the inspection findings are summarized below. This facility is located at 1660 Blair Street in Thomasville, Randolph County, North Carolina. Stormwater related to industrial activity is discharged from the site to an unnamed tributary of the Uwharrie River, which is currently classified as Class WS- III waters, in the Yadkin -Pee Dee River Basin. Documentation Review The Stormwater Pollution Prevention Plan (SPPP) that was provided on site was out of date and did not meet permit requirements. The following problems were noted with the SPPP: 1. Employee training has not been documented as required by Part II, Section A-5 of the permit. Stormwater training is required at least annually and training must be documented with the date and signature of each employee in attendance. These records must be kept with the SPPP. 2. The SPPP has not been updated or amended since 1998. Part II, Section A-7 of the permit requires the SPPP to be reviewed and updated annually, along with a re -certification that outfalls have been evaluated for the presence of non-stormwater discharges. Monitoring Review Neither analytical monitoring (AM) nor qualitative monitoring (QM) have been conducted or documented as required. Part 11, Sections B and D of the permit require semi-annual AM and QM at the stormwater discharge outfall (SDO) during a measurable storm event. Murrows Transfer Inc. must immediately correct these violations by beginning monitoring as required by the permit. Please be sure to keep all AM and QM records with the SPPP.
