Winston-Salem terminal on March 28, 2013 Your assistance and cooperation, during the inspection,

findings are summarized below
The facility is located at 199 Park Plaza Drive, in Winston-Salem, Forsyth County, North
Carolina, at approximate coordinates 36 156333°N, -80 241373°W Stormwater related to industrial
activity is discharged from Stormwater discharge outfalls (SDO) 001 and 002 to Salem Creek, which
is currently classified as a Class C water located in the Yadkin Pee -Dee River Basin

1) UPS's revised site map denotes the sampling location at Outfall #1
2) A screening mechanism has been placed in the stormwater collection system (at the trash
dum ster area) in order to eliminate floatables from entering surface waters
3) The Stormwater Division compared photographs of Outfall #1, which were taken 11/12/08 and
10/22/09 and observed no further degradation of the downstream area Dense natural vegetation
and channel armoring placement of rip -rap) at the discharge point have prevented additional erosion
from occurring In accordance with General Permit NCG080196, the Stormwater Division is requiring
United Parcel Service to conduct continued monitoring_of Outfall #1 for downstream erosion
Please note if obvious indicators of soil erosion are observed, UPS will need to implement
supplemental stormwater best management practices in order to abate any future outfall
degradation
4) UPS has implemented the current version of NCDWQ's stormwater discharge outfall monitoring
report, which directly reflects sampling requirements
5 Total precipation data is obtained from Smith -Reynolds Airport and attached to the stormwater
discharge outfall monitoring report
6) All floatables have been removed from the southern corner of the property
7) United Parcel Service has successfully completed all outstanding issues from the City's 11/12/2008

2 The facility is located at 199 Park Plaza Drive, in Winston-Salem, Forsyth County, North Carolina
Although the receiving stream is not reflected on the current Certificate of Coverage (COC), it
appears that stormwater related to industrial activity is discharged to an unnamed tributary to Leak
Fork, which is currently classified as Class C waters in the Yadkin River basin Please verify this
information upon next permit renewal

United Parcel Service
199 Park Plaza Drive
Winston-Salem
Forsyth County
to receiving waters designated as the Winston-Salem Storm Sewer System and Salem Creek in the Yadkin -Pee Dee
River Basin
in accordance with the effluent limitations, monitoring requirements, and other conditions set forth in Parts I,11, I1I
and IV of General Permit No NCGO80000 as attached
This Certificate of Coverage shall become effective August 12, 1994
This Certificate of Coverage shall remain in effect for the duration of the General Permit
