BEEN NO PHYSICAL LOCATION CHANGE. THE FACILITY PHYSICAL LOCATION AND COORDINATES REMAIN UNCHANGED.

effective June 15`, we have issued the Certificate of Coverage (COC) to Pittsburgh Glass today. Please see the attached
letter and COC that is being mailed out to the Plant Manager.
You can download a complete copy of the General Permit package, which includes the permit itself, the technical
bulletin, and monitoring forms here:

Hi Corey,
Attached is an NOI application from Pittsburg Glass Company, LLC for a new facility in Surry County. The company is
requesting coverage under NCG07 stormwater general permit for a site in Elkin.
They will manufacture automatic glass parts and will discharge stormwater to Turkey Creek (Class C) and to a UT to that
creek in the Yadkin -Pee Dee Basin. Operation is scheduled to begin later this year, June 1, 2013. Does the Winston-
Salem Regional Office have any concerns with issuing a COC for this site? If we don't hear from you in 30 days, the
Central Office will plan to move ahead with a COC for this facility (by April 19, 2013).
