Your assistance was greatly appreciated. The facility was found to be compliant with its stormwater permit requirements. An inspection report is attached for your records and the inspection findings are summarized below. 1. Permit This facility holds General Stormwater Permit No. NCG070030 to discharge Stormwater from activities associated with Stone, Clay, Glass and Concrete Products under the National Pollutant Discharge Elimination System (NPDES). The permit became effective June 1, 2013 and expires May 31, 2018. Your permit renewal package must be received in the Central Office in Raleigh no later than 180 days prior to the expiration date of the permit. 2. Records/Reports Part II, Section A of the permit requires the development of a Stormwater Pollution Prevention Plan (SPPP). The plan must include all 9 sections of Part II, Section A requirements of the permit. This site is not required to complete analytical monitoring but qualitative monitoring is required to be completed and documented twice yearly as defined in Part li, Section C of the permit. The plan must be updated annually and qualitative monitoring must be conducted semi-annually to maintain compliance with the permit. The current SPPP and monitoring records were available for review and are maintained electronically. A printed copy of the most recent monitoring was available, including photos taken of each outfall at the time of monitoring. Facility Site Review Permatech is a refractory manufacturer. 4. Effluent/Receiving Waters Stormwater from the site discharges to Sooky Creek, Class "C waters of the Yadkin Pee Dee River Basin. 5. Self -Monitoring Program Analytical Monitoring: This site is not required to provide analytical monitoring under current conditions of the permit. 6. Qualitative Monitoring: All qualitative monitoring records were available for review at the time of inspection. The latest monitoring is current and records go back several years with monitoring completed twice annually. Records do not need to be submitted to DEMLR,-however, the records must be kept on file at the facility for a period of five years. 7. Outfalls: This site had (7) seven outfalls. The seventh has never produced flow when monitored and therefore has been eliminated. The remaining outfalls were observed at the time of inspection. Two are in a wooded area but are accessible. Overall, the site is very well maintained with exception to the storage area in the chain link fence. Molds, pallets, metal, steel wire, incidentals and refractory waste need to be cleaned up. The entire storage area needs better housekeeping and clean-up. A maintenance schedule for this area should be part of general operating procedures. A contractor, working on construction of a new part of the facility inadvertently left a line that carried thermal fuel open and unchecked for a limited time. Fuel flowed into the (reline summand drained into the ditch along Hwy 150. Measures were taken to limit spill from running downstream and offsite. An environmental clean-up crew was hired and clean-up was completed Quickly. Precedures were followed. NCDENR staff was notified. This facility holds General Stormwater Permit No. NCG070071 under the National Pollutant Discharge Elimination System (NPDES) to discharge Stormwater from activities classified as Stone, Clay, Glass, and Concrete Products. As Director of Operations, you were present for the inspection. This type of inspection consists of two basic parts: a review of facility files and self -monitoring data, and an on -site inspection of the facility and its outfalls. This particular inspection evaluated five (5) areas as designated on the inspection form, and observations from each area are addressed below: 1. Permit The permit became effective June 1, 2008 and expires May 31, 2013. 2. Records/Reeorts As required by the Permit, the facility has a complete Stormwater Pollution Prevention Plan (SPPP) that was available for review during the inspection. A private consultant created the SPPP, but Permatech staff performs a semi-annual review to ensure that the document is current. Stormwater Pollution Prevention Awareness Training is required of all employees once per year. Please maintain a sign -in sheet from the Training with the SPPP. 3. Facility Site Review Permatech is a refractory manufacturer. The main source of potential pollution at the plant exists in the outside storage area behind the building, where Permatech stores molds for the manufacturing process, dumpsters for waste, as well as empty totes, drums, and pallets waiting for recycling. Dumpsters are sealed with silicone to prevent accidental releases. During the inspection, no major problems were noted with respect to housekeeping in this area. Another possible source for Stormwater pollution is in the docking area. Trucks entering the receiving and delivery areas are required to report and clean up any spills that occur during loading or unloading. The facility places a protective cover such as cardboard on the ground prior to chemical transfers to catch any possible drips or small spills. Effluent/Receiving Waters Stormwater discharges from this site drain to an unnamed tributary to Haw River (Class C; NSW) in the Cape Fear River Basin. The site has six (6) stormwater discharge outfalls. Each one appeared to be well maintained. During the inspection, the outfalls were all flowing with a significant amount of Stormwater, due to the heavy rains in the days before. No major problems were observed. 5. Self -Monitoring Program The stormwater permit requires semi-annual qualitative monitoring at stormwater outfalls. An on -site review of the qualitative monitoring data during the inspection showed that all parameters were evaluated per the specifications of the permit. On -site vehicle maintenance monitoring requirements at the site are waived, since these activities are not performed at the site. You are found to be in compliance with General Permit No. NCG070071. Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial facilities in any of the l I categories of "storm water discharges associated with industrial activity," (except construction activities). If you feel your facility can certify a condition of "no exposure", i.e. the facility industrial materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion. NCG070071 STORMWATER DISCHARGES NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM In compliance with the provision of North Carolina General Statute 143-215.1, other lawful standards and regulations promulgated and adopted by the North Carolina Environmental Management Commission, and the Federal Water Pollution Control Act, as amended, Permatech, Inc. is hereby authorized to discharge stormwater from a facility located at Permatech, Inc. 911 East Elm Street Graham Alamance County to receiving waters designated as the Haw River in the Cape Fear River Basin in accordance with the effluent limitations, monitoring requirements, and other conditions set forth in Parts I, II, III and IV of General Permit No. NCG070000 as attached. This Certificate of Coverage shall become effective July 8, 1994. This Certificate of Coverage shall remain in effect for the duration of the General Permit.
