Resources (DEMLR) conducted a compliance evaluation inspection at the subject facility on 28July 2014.
Your assistance and cooperation, as well as that of Mr. Jay Easter and Mr. Andy Stewart, were greatly
appreciated. An inspection checklist is attached for your records and the inspection findings are
summarized below.
This facility is located at 9698 Old US Highway 52 in Lexington, Davidson County, North Carolina.
Stormwater related to industrial activity is discharged from the site to Leonard Creek, which is currently
classified as Class WS-III waters, in the Yadkin -Pee Dee River Basin.
Documentation & Monitoring Review
The facility's Stormwater Pollution Prevention Plan (SPPP) is very thorough, up to date, and fully
implemented. The SPPP was last updated in June 2014. All documents were well organized and available
for inspection.
Qualitative monitoring has been completed as required. No discrepancies were noted. Owens -Brockway
performs limited vehicle maintenance on forklifts, but does not use more than 55 gallons of new motor
oil per month, so no analytical monitoring is required. The facility has three Stormwater Discharge
Outfalls (SDOs) that discharge Stormwater related to industrial activity.
Site Review
The site is orderly and well maintained. Secondary containment is implemented as required. All outfalls
were inspected and found to be in excellent condition. Best Management Practices (BMPs) are installed
at the outfalls to reduce potential contaminant loss from the site. No evidence of erosion or instability at
the SDOs was observed. The receiving stream was free of pollution indicators in all cases.

On September 2, 2009, Gary Hudson of this office met with Jeffrey Beckelhimer and you
to perform an Individual Stormwater Permit Inspection at the Owens - Brockway Glass
Container Inc facility located on Old Highway 52 in Lexington. This inspection evaluated the
areas as designated on the attached inspection form. Observations from each area are
addressed below:
1. Permit
Owens - Brockway Glass Container Inc holds an NPDES permit # NCG070064 for the
discharge of stormwater from the facility. The General Permit became effective June 1, 2008
and will expire May 31, 2013.
2. Records/Reports
Part II, Section A of the permit requires the development of a Stormwater Pollution Prevention
Plan (SPPP). A copy of the SPPP was available for review. The plan contained all of the items
required by the stormwater permit. Please note that the permit requires the plan to be reviewed
and updated on an annual basis.
3. Facility Site Review
Observations of the outfalls at the facility did not reveal any concerns. The site appeared very
clean and well managed
4. Effluent/Receiving Waters
Stormwater runoff from the facility reaches Leanord Creek, Class "C" waters in the Yadkin/Pee
Dee River Basin.

5. Monitoring Program
Part 11, Section C of the permit requires that semi-annual qualitative (visual) monitoring be
performed and documented once in the spring (April - June) and once in the fall (September —
November). A review of the facility files found that the qualitative monitoring has been
conducted as required.
The analytical monitoring for on -site vehicle maintenance listed in Section B of the permit is not
required since the Owens - Brockway Glass Container Inc plant uses less than 55 gallons of
new motor oil per month when averaged over the calendar year.

Your facility is currently covered for Stormwater discharge under General Permit NCG070000. This permit expires
on March 31. 2003, The Division staff is currently in the process of rewriting this permit and is scheduled to have
the permit reissued by early spring of 2003. Once the permit is reissued, your facility would be eligible for
continued coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage. To nnake this renewal process easier, we are informing you,in,
advance that your permit coverage will be expiring. Enclosed you will find a Permit Coverage Renewal
Application Form. The application muss be completed and returned by October 2, 2002 in order to assure
continued coverage under the general permit. Due to staff and budget constraints,. letters confirming our receipt of
the completed application will not be sent.
Failure to request renewal within the time period specified, may result in a civil assessment of at least $250,00.
Larger penalties may be assessed depending on the delinquency of the request. Discharge of Stormwater from your
facility without coverage under a valid Stormwater NPDES permit would constitute a violation of NCGS 143-215.1
and could result in assessments of civil penalties of up to $10,000 per day.
Please note that recent federal legislation has extended the "no exposure exclusion" to ail operators of industrial
facilities in any of the 1 1 categories of "storm water discharges associated with industrial activity," (except
construction activities). If you feel your facility can certify a condition of "no exposure", i.e. the facility industrial
materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion. For additional

If the subject stormwater discharge to waters of the state has been terminated, please complete the enclosed
Rescission Request Form. Mailing instructions are listed on the bottom of the form. You will be notified when the
rescission process has been completed.

This Certificate of Coverage shall become effective June 24, 1994.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.
