The facility was previously inspected by Mr. Davis Murphy on June 2, 2015 and found to be operating in compliance with Air Quality rules and regulations at that time. The facility manufactures precast septic tanks and junction boxes. The plant is open ten hours a day, five days a week but operates on a demand basis with some seasonal variability. Mr. Shoaf confirmed that the IBEAM contact information was accurate. Safety It is recommended that inspectors wear safety shoes, safety glasses, hearing protection, and a hard hat. Applicable Regulations According to permit Condition A.1 the facility is subject to the following regulations: Title 15A North Carolina Administrative Code (NCAC), Subchapter 2D .0202, 2D .0515, 2D .0521, 2D .0535, 2D .0540, 2D .0611, 2D .1806, 2Q .0309 and 2Q .07! I. This facility is not subject to RMP requirements of the 112(r) program since it does not use or store any of the regulated chemicals in quantities above the threshold levels in the rule. The facility is only subject to the General Duty requirements contained in the General Duty Clause. Operating Status/Discussion The facility is a precast concrete septic tank plant that receives cement, aggregate, sand, water, and admixtures and loads them into trucks to be poured into molds and dried. The facility also manufactures junction boxes, catch basins, and other concrete fittings. None of the permitted equipment was being operated during this inspection. According to Mr. Shoaf, the facility receives cement deliveries about once per week. The facility is permitted for two concrete plants. Plant No. I consists of a cement storage silo (ES-1) controlled by a bagfilter (CD-1). Plant No. I is no longer at the site and has been removed. As noted in the previous inspection report, this plant was removed in 2012. Mr. Shoaf explained that there are currently no plans to rebuild this plant. Plant No. 2 consists of a cement storage silo (ES-2), Fly ash silo (ES-3), weigh hatcher (ES-4), and truck loadout operation. Each silo has a bagfilter (CD-2 & CD-3) located on top of each respective silo to control emissions during loading. Cement is gravity fed into the weigh batcher directly below the cement storage silo. Mr. Shoaf confirmed that the fly ash silo is no longer in use, and is currently empty. The fly ash silo is located next to the cement silo, and when in use, it can auger feed to the weigh batcher. A small bagfilter on top of the weigh batcher controls emissions when product is added to it. Material is gravity fed from the weigh batcher into the truck loadout, which is uncontrolled. Aggregate and sand is conveyed from a storage bin to the truck loadout. Wet concrete is poured from the truck into molds that are allowed to dry for 24-48 hours before being stockpiled in the yard. The facility also has three concrete admixture tanks located in a shipping container (IES-1 through 3). The admixture tanks contain an accelerator, water reducer, and an air entrainer. Mr. Shoaf confirmed that there had been no changes to facility equipment since the previous inspection. Permit Conditions/Applicable Regulations Condition A.2 contains the 15A NCAC 2Q .0304 and I SA NCAC 2D .0202 permit renewal and emissions inventory requirements. The permit expires June 30, 2023 and the facility will be required to submit a renewal request and air pollution emission inventory report for the 2022 calendar year 90 days prior to the expiration of the permit. The renewal process and emissions inventor were discussed with Mr. Shoaf. Compliance with this condition is indicated. Condition A.3 contains the 15A NCAC 2D .0515 particulate control requirement. This condition sets the maximum allowable particulate emissions using the following two equations: E — 4.10 * (P) °67 for P <= 30 tons/hr., or E = 55 * (P) 01- 40 for P >30 tons/hr. Where P is the process weight in tons/hour and E is the maximum allowable emissions rate for particulate matter in lbs./hour. Bagfilter control efficiencies indicate that this condition will be met provided that the bagfilters are working properly. Also, the . emission sources, baghouses, and associated ductwork appeared to be in good condition during this inspection. Compliance with this condition is expected but could not be verified as the equipment was not operating at the time of inspection. Condition A.4 contains the 15A NCAC 2D .0521 visible emissions control requirement. This facility is limited to a 20 percent opacity limit when averaged over a six -minute period. Compliance with 213.0521 could not be verified as the facility equipment was not in operation, but is expected if the bagfilters are functioning properly. Condition A.5 contains the I SA NCAC 2D .0535 rule which requires the permittee to notify the director of any excess emissions lasting longer than four hours resulting from a malfunction, a breakdown of process or control equipment or any other abnormal conditions. Mr. Shoaf stated that no such incidents since the previous inspection. Compliance with this condition is indicated. Condition A.6 contains the 15A NCAC 2D .0540 fugitive dust control requirement. Mr. Shoaf indicated that the facility does not typically have problems with dust and that they make sure their haul roads have plenty of gravel on them during dry periods. No fugitive dust was observed traveling beyond the property boundaries at the time of inspection. No complaints regarding this facility were found in the DAQ-IBEAM database. Compliance with this condition is indicated. Condition A.7 contains the 15A NCAC 2D .0611 Fabric Filter Inspection, maintenance, and recordkeeping requirements for particulate matter emissions control. The facility is required to perform, at a minimum, an annual (for each 12-month period following the initial inspection) internal inspection of each bagfilter system and perform periodic inspection and maintenance as recommended by the equipment manufacturer. The facility is required to record the results of all inspection and maintenance activities in a logbook. The bagfilter logbook was reviewed back to the last compliance inspection and found to be complete. According to the logbook, internal inspections are typically performed every 4 months. Both CD-2 and CD-4 were last inspected on April 17, 2017. Mr. Shoaf was reminded that if the facility decides to operate the fly ash silo (ES-3) in the future, inspection and maintenance requirements for the associated bagfilter (CD-3) will be necessary. Compliance with this condition is indicated. Condition A:8 contains the 15A NCAC 2D .1806 control and prohibition of odorous emissions requirements. No objectionable odors were observed during this inspection. The facility does not have any previous odor complaints in the IBEAM database. Compliance with this condition is indicated. Condition A.10 contains the 15A NCAC 2Q .07t I Toxic Air Pollutant Emissions Limitation Requirement. The condition states, in part, that the facility shall be operated and maintained in such a manner that emissions of any listed TAPS from the facility, including fugitive emissions, will not exceed TPERs listed in 15A NCAC 2Q .0711. Calculations from permit review R06 demonstrate that the facility will not exceed any TPERS when producing about 2800 cubic yards/year. Mr. Shoaf estimated that in CY 2015 and CY 2016, this facility's production was approximately half of that rate. Compliance with 2Q .0711 is anticipated. NSPS/NESHAP Mr. Shoaf stated that the facility does not currently have any boilers, hot water heaters, or emergency generators on site. The facility does not appear to be subject to any NSPS or NESHAP programs at this time. However, no deposits were observed within the culvert that leads under US Hwy 64 off of the property. Mr. Shoaf explained that material was washed into the drainiage ditch as a result of recent heavy rains. The excavated pits at the truck washout area had been recently cleaned out. _ Please ensure that all material deposited into the drainage ditch is removed to prevent it from being washed off of the property. Measures should be provided beyond the excavated pits at the truth washout area to prevent solids from being washed off site. This can include but not be limited to, installation of rip rap and gravel check dams. VAN DER VAART Secretary On June 16, 2016 Glen White of the North Carolina Department of Environmental Quality; Department of Energy, Minerals, and Land Resources met with you at the facility located at 4130 West U.S. Hwy 64, in Lexington, to conduct a compliance inspection per,the Industrial Stormwater Permit requirements. This inspection consists of review of the Stormwater Pollution Prevention Plan, training, monitoring and spills records as well as observation of the general condition of the site, the outfalls and any possible exposures to downstream .stormwater. Your assistance with the inspection was greatly appreciated. Permit: This facility holds General Stormwater Permit NCG070032 to discharge stormwater from industrial activity associated with Stone, Clay, Glass and Concrete Products [SIC 321 under the National Pollutant Discharge Elimination System (NPDES). The permit became effective June 01, 2013 and expires on May 31, 2018. A copy of the current permit -is required to be maintained with the Stormwater Pollution Prevention Plan (SPPP) documents. The current permit was available for review and was included in the SPPP. Records/Reports- This facility was required to develop and maintain a Stormwater Pollution Prevention Plan (SPPP) in accordance with Part II, Section A of the permit. The SPPP has been updated annually as required and contained all components as required. No deficiencies noted. Facility Site Review: Shoaf Precast, Inc. opened for business at the current location in 1966. It's founder James Shoaf, father of Dennis and Jimmy, current owners, still delivers product to clients. This facility manufactures septic tanks of many sizes from use at private 3 & 4 bedroom homes to private commercial facilities and even public parks where municipal sewer is unavailable. The plant also makes other concrete products such as heavy duty valve collars and septic field junction boxes. The plant makes one batch per day on average.
