This letter is in response to the Notice of Deficiency from the NCDENR dated February
4, 2013. Mr. Ronald Boone inspected our facility. Mr. Boone was very thorough. He was also
very helpful with explaining the requirements of our permit.
We have addressed all of the issues that are listed from the inspection checklist and the
Notice of Deficiency. Below are the corrective actions that we have taken to address these
issues:
• We have updated our General Location Map and/or Site Map to include the name of the
receiving waters and the latitude / longitude coordinates of each stormwater outfall as
specified by NCG060000, Part II, Section A, Paragraph I (a). Additional notes have been
added to the map with reference to the states 303 (d) list and the TMDL established for
watersheds.
• We have updated our General Location Map and/or Site Map to include the percent of
each drainage area that is impervious as specified by NCG060000, Part I1, Section A,
Paragraph 1(c).
• The Stormwater Pollution Prevention Plan has been updated and signed with the specific
responsible party according to NCG060000, Part II, Section A, Paragraph 7.
• The Stormwater Pollution Prevention Plan has been updated, reviewed, and signed with
the specific responsible party according to NCG060000, Part II, Section A, Paragraph
I (e) and 7. An annual reminder has been added in the Microsoft calendar for future.
• The Spill Prevention Control and Counter Measures Plan has been located and placed
with other stormwater material. The Emergency Contact List is in this document. This
includes the Divisions telephone number and other important response and notification
contacts.
• Training has been performed and documented according to NCG060000, Part 1I, Section
A, Paragraph 6. An annual reminder has been added in the Microsoft calendar for future.
• The Stormwater Pollution Prevention Plan has been updated, reviewed, and signed
according to NCG060000, Part 11, Section A, Paragraph 8.
• Two additional SDOs have been added as per the Divisions request. The appropriate
plans and documentation has been updated.
• Appropriate inspections, updates, and reviews have been established to comply with the
current permit.
We feel that the above list has addressed all of the items from the Notice of Deficiency.
We also feel that this will regain our Facility Status as "Compliant".
Again, I appreciate the help and direction provided by Mr. Boone. This has provided us
with a greater understanding of the Stormwater Permit requirements.

Dear Mr. Koonce:
Ron Boone, of the Winston-Salem Regional Office (WSRO) of the NC Division of Water Quality (DWQ or the
Division), conducted'a compliance evaluation inspection (CEI) of subject facility on January 31, 2013. The assistance and
cooperation of Mr. David McCormick, Maintenance Supervisor, and Mr. Scott Sell, Plant Manager, during the inspection
was greatly appreciated. An -inspection checklist is attached for your records and the inspection findings are summarized
below.
The facility is located at 3000 Big Oaks Drive, in King, Forsyth County, North Carolina, at approximate
coordinates 36.254970N,-80.3753820W. Stormwater related to industrial activity is discharged from stormwater discharge
outfalls (SDO) to unnamed tributaries of Crooked Run Creek, which is currently classified as a Class WS-IV (water
supply) water located in the Yadkin Pee -Dee River Basin.
Documentation & Monitoring Review
The Stormwater Pollution Prevention Plan (SP3) and CRES's monitoring activities were reviewed. The following
discrepancies were noted:
• The General Location Map (GLM) does not meet all requirements specified in NCG060000. Part II, Section A,
Paragraph 1(a);
The Site Map (SM) does not meet all requirements specified in NCG060000, Part II, Section A, Paragraph
1(c);
• The SP3 contains a statement that no significant spills have occurred at the facility in the past three years,
however, the SP3 was written in May, 2008, and the list/statement has not been updated since, as required
by NCG060000, Part II, Section A, Paragraph 7;
• The SP3 contains a certification of the SDOs from May, 2008, but it has not been updated since, as required
by NCG060000, Part II, Section A, Paragraphs 1(e) and 7;
• The Spill Prevention and Response Procedures (SPRP) contained a reference to the facility's Spill
Prevention, Control, and Countermeasure Plan (SPCC), but the SPCC could not be located at the time of the
inspection; CRIES should ensure the Division's phone numbers are contained within the plan(s) so we can be
contacted in the event of a spill;
• Employee training documentation could not be located during the inspection as required by NCG060000, Part
II, Section A, Paragraph 6;
• The SP3 is not being reviewed and updated annually as required by NCG060000, Part il, Section A,
Paragraph 8; and,

• Both analytical and qualitative monitoring, as required by NCG060000, Part II, Sections B and D, respectively,
has been accomplished for the currently identified SDOs, however, two more SDOs were observed during the
inspection, as detailed below;
Site Review
The site appeared clean and well maintained. However, Mr. Boone observed two more SDOs during the
inspection, in addition to the SDOs currently identified and monitored by CRIES. The two existing SDOs that CRIES
monitors are located on the northwest and southwest corners of the facility. The two additional SDOs are located:
On the north side of the facility, just outside the fence line, at approximate coordinates: 36.255981 °N,
80.3757690W; and,
On the southeast corner of the facility, at approximate coordinates: 36.254853°N,-80.374516°W.
These additional SDOs must be added to the facility's SP3 and monitored in addition to the first two SDOs.
The Division requires that Alliance/CRES corrects the deficiencies outlined above as soon as'possible. Please .
reply to this Notice of Deficiency (NOD) in writing within 30 business days of receiving it. Alliance's written response
must address each of the deficiencies outlined above and provide corrective actions with implementation schedules for
each.
Alliance is reminded that violations/deficiencies of the NCG060000 general stormwater permit are subject to civil
penalty assessments not to exceed $25,000 per day, per violation. Should you have any questions regarding the permit,

On September 24, 2009, Gary Hudson of this office met with you to perform a General
Stormwater Permit Inspection at the CRIES Tobacco facility located on Big Oaks Drive near
King. This inspection evaluated the areas designated on the attached inspection form.
Observations from each area are addressed below:
1. Permit
CRIES Tobacco holds Certificate of Coverage # NCG060299 for the discharge of stormwater
from the facility. The General Permit became effective February 15, 2007 and expires October
31, 2012.
2. Records/Reports
Part II, Section A of the permit requires the development of a Stormwater Pollution Prevention
Plan (SPPP). A copy of the SPPP was available for review. The plan contained all of the items
required by the stormwater permit. Please note that the permit requires the plan to be reviewed
and updated on an annual basis.
3. Facility Site Review
The site appeared clean and well managed. No concerns or problems were noted.
4. Effluent/Receiving Waters
Stormwater runoff from the facility reaches an unnamed tributary to Run Creek, Class °C
waters in the Yadkin/Pee-Dee River Basin.

5. Monitorinq Program
Part ll, Section B of the permit requires that semi-annual qualitative (visual) monitoring be
performed and documented once in the spring (April - June) and once in the fall (September —
November). A review of the facility files found that the qualitative monitoring has been
conducted as required.
Part II, Section B of the permit requires that analytical monitoring be performed. A review of the
facility files found that the analytical monitoring has been conducted as required.
The analytical monitoring for on -site vehicle maintenance listed in Section C of the permit is not
required since CRIES Tobacco uses less than 55 gallons of new motor oil per month when
averaged over the calendar year.
Our office appreciates the efforts by you and your staff at the CRIES Tobacco facility to
comply with the NPDES General Stormwater Permit. Should you have any questions
