Many of the items mentioned in that inspection report have been resolved. Part I Summary: 1. The routine overflows from the lift station near the fuzzy filter appears to have been resolved. The site in this area appeared to be clean with no signs of accumulated wastewater in that vicinity noted. 2. A stormwater pollution prevention plan had been developed and is being implemented. The site appearance was MUCH improved over the previous site visit in 2010. There are a few problem areas that still exist in the vicinity of the loading area to the processing plant and the area where offal trucks are being loaded. A bird carcass was seen on the impervious pavement area as well as some signs of offal on the ground east of "DIP-7 (discharge point 7 or stormwater discharge outfall (SDO) 007). 3. The House of Raeford has implemented analytical monitoring. Some outfalls were monitored beginning in the fall of 2010 while others were monitored beginning in 2012. The requirement to conduct monthly analytical monitoring has been triggered, and should be in effect, at the following outfalls: 012, 011, 010, 008, 004, 003, 002; 001, due to two consecutive exceedances of benchmarks at those outfalls. Outfall 005 exceeded benchmarks in February, 2011 and no data for the last half of 2011 or first half of 2012 was provided. Failure to monitor also results in the requirement to begin monthly monitoring. Therefore, outfall 009 (no data has been received for 009) will need to be monitored on a monthly basis. Monthly monitoring should continue until the end of the current permit cycle or until further notice by the Division of Water Quality. A copy of the monitoring data should be sent to the Wilmington Regional Office 30 days after the receipt of the laboratory results, in addition to the reporting requirements to the Central Office. Please forward that information to Dean Hunkele. The Division of Water Quality (DWQ) requires monitoring at outfalls 006 and 007 be conducted again to determine whether outfall 005 is representative of the other two, especially since the appropriate monitoring frequency was not maintained at outfall 005. Most of the stormwater discharge might be leaving the area via one of the other outfalls. Part II Additional Information Request: 1. DWQ requested in stream monitoring (in Rock Fish Creek) of bacteria during an inspection in August 2010, The Division has not seen the results of the bacteria monitoring in the UT to Rock Fish Creek. Sampling was to occur in the UT downstream of the point where stormwater outfalls DP-5 (SDO 005), DP-6 (SDO 006), and DP-7 (SDO 007), discharge. If this has not been conducted, please implement instream sampling immediately. It should be conducted quarterly. 2. The Division is in receipt of the Wastewater Treatment Plant Calculations from Jimmy Holland. Included in the report was an updated map showing the drainage areas on site and the amount of stormwater generated from a 1" rain. Hydraulic and biological loading for the wwtp was provided. Information still needed are as follows: A. Indicate the rain events (1", 2" etc.) that would result in bypasses from the lift stations; B. Provide the storage capacities in the wet wells for each Lift Station; C. Provide the storage capacity of the emergency storage pond; D. Provide stormwater data that indicates the amount of rain required to provide an adequate first flush so that runoff not captured for treatment would meet benchmarks. E. The following are bacteria concentrations (Fecal Coliform colonies1100 ml) measured in stormwater outfalls from the Wallace facility; 001 - 1.5 million, 002 - 500,000, 003 - 51,000, 004 - 400,000, 005 - 3600, 008 - 260,000, 010 - 510,000, 011 - 16,000, 012 - 450,000. Reporting values of greater than is not acceptable. Please provide the Division with the plans to resolve these bacteria concentrations in stormwater discharges from the facility. Conclusion: The quality of the stormwater discharges from this site are unacceptable. The quantities of bacteria and oxygen consuming pollutants can pose detrimental effects to the receiving stream as well as pose a danger to public health for recreational users downstream of your facility. Your efforts taken to date are commendable, however, the quality of the stormwater discharge continues to be of concern to the Division of Water Quality. Why is the BOD so much lower from the EQ versus the DAF yet the COD shows no change? Any idea why the R TSS is so much greater from the EQ than the DAF? 4. Lift station #3 discharges to the DAF rather than the EQ basin? LeS S. Can you give us the hydraulic talcs on the DAF that also receives stormwater? And is that really a DAF or just a wide spot in the line from the DAF unit? 6. What storage capacities do the wet wells at each lift station provide? 7. Can I get more info on how you come up with the talcs for LS #3? You're showing 22,500 sq ft that serves that LS (I'm assuming that includes the area west of the wood chip boiler bldg and the shop?) but should also take in the discharge from LS #2 (47,991 gallons). I thought LS #3 had the overflow pipe to the emergency basin, but the map shows the overflow from the Drain (#53). How did you determine what amount is diverted to the emergency pond vs the DAF? 8. I'm showing 3 other LS's that aren't numbered. Which is LS#4 that you reference? Can you give us the pump rates and flow from the other two Lift Stations noted on the map? 9. 1 used google to figure out the dimensions of the emergency stormwater/ww basin.. I'm guessing the depth at 2' (conservative estimate on depth) and the width at 300' x length of 350' for a volume of roughly 210,000 tuft. Do you agree with that? if not, what's your figures on that basin? 10. The map also doesn't show any flow to the emergency stormwater basin from the wwtp, is that correct? ` 11. 1 see you calculate that 1.05 MGD is the tar flow for an allowable BOD (you back calculated that).. where did the allowable BOD loading come from? iteratu ? This permit is reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as subsequently amended). The following information is included with your permit package: • A new Certificate of Coverage (COC) • A copy of General Permit NCGO60000 • A copy of the Technical Bulletin for the General Permit • Two copies of the Discharge Monitoring Report (DMR) Form • Two copies of the Qualitative Monitoring Report Form The General Permit authorizes discharges of stormwater, and it specifies your obligations for discharge controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself with all the changes in the reissued permit. Your facility has six (6) months from the time of receipt of the permit to update your current SPPP to reflect all new permit requirements. The first sample period of your permit begins January 1, 2013. Your facility must sample a "measureable storm event" beginning during the periods beginning January 1 and July 1 of every year (or, if applicable, report "No Flow," as outlined in Part III, Section E). Also, please note that Tier 3 Actions in Part II of your permit are triggered by benchmark exceedances on four occasions beginning on the effective date of this permit and do not count prior exceedances. Additional conditions for specific industry sectors have been added to the SPPP requirements in some cases. • Sections B, C: Failure to perform analytical Stormwater monitoring may result in the Division requiring that the permittee begin a monthly sampling scheme. • Sections B, C: A lower TSS benchmark of 50 mg/I for HQW, ORW, PNA and Tr Waters applies to these more sensitive waters. • Sections B, C: The monitoring parameter Oil & Grease (O&G) has been replaced by the parameter Total Petroleum Hydrocarbons (TPH) for vehicle maintenance areas, and in some cases, other analytical monitoring requirements. • Sections B, C, D: Inability to sample due to adverse weather must be recorded in the SPPP, or in separate on -site records if your General Permit does not require an SPPP. A measurable storm event is defined in the permit. • Section D: If the permittee fails to respond effectively to problems identified by qualitative monitoring, DWQ may require the permittee to perform corrective action. The focus of the site inspection was to provide technical and compliance based recommendations to resolve deficiencies/issues on site pertaining to NPDES Permits NCO003344 and NCG060177. DWQ compliance inspector, Kipp Glazier, requested that DWQ inspectors not pursue any enforcement actions concerning deficiencies related to the NCGO60000 permit or any surface water issues found, since the company is working diligently to resolve issues via of the Judicial Order by Consent. Provided the issues herein are incorporated into the resolutions, no further compliance action will be taken. 1. There is evidence of routine overflows at the lift station located by the fuzzy filter. This lift station receives overflows from the mudwell. Evaluate, report and resolve this overflow issue. Require daily inspections of this area and report all overflows to DWQ. The mudwell should not be overflowing to the ground, it should be pumped to an appropriate treatment process (headworks). 2. Evaluate the pumping capacities for all pump stations conveying stormwater to the ww treatment system. Evaluate what size rain event (1", 2" etc.) would result in bypasses from the collection system. Plan to upgrade any deficiencies in this system. 3. Begin monitoring all discharges from each of the twelve stormwater outfalls during every rain event that results in stormwater discharges until further notice. The reason is to determine stormwater characteristics. Analyze for TSS, Oil and Grease, Fecal Coliform, and COD. When enough data to demonstrate a trend has been collected, House of Raeford can return to monitoring at frequencies specified in the permit (semi-annually). Should the characteristics of the stormwater warrant actions for remedial measures, DWQ wants to be able to add further action items to the list. All the data must be recorded on a daily monitoring report (see attached) for review by DWQ and in accordance with reporting requirements specified in NCG060000. There are 3 stormwater outfalls in the same vicinity (DP-5, DP-6 and DP-7), with some improvements, these can be reduced to one outfali, for a total of 10 outfalls for monitoring. 4. A written Stormwater Pollution Prevention Plan (SWP3) is required for this facility in accordance with the NPDES General Stormwater Permit NCG060177. House of Raeford has owned this facility since 2003. A SWP3 was due in 2004 (12 months after House of Raeford purchased this facility). 5. Immediately begin in stream sampling in the UT to Rock Fish Creek on a biweekly basis for fecal Coliform bacteria until further notice.
