This facility is located at 6008 High Point Road in Greensboro, Guilford County, North Carolina. Stormwater related to industrial activity is discharged from the site to Bull Run Creek, which is currently classified as Class WS- IV waters, in the Cape Fear River Basin. Documentation & Monitoring Review The facility's Stormwater Pollution Prevention Plan (SPPP) is thorough, up to date, and fully implemented. However, please be sure to update the list of significant spills within the SPPP. Qualitative and analytical monitoring have been completed as required. This facility does not perform on -site vehicle maintenance. The facility has five Stormwater Discharge Outfalls (SDOs) that discharge stormwater related to industrial activity. Alberdingk Boley was granted Representative Outfall Status (ROS) in a letter from our Department in 1997 and two outfalls are currently monitored for stormwater compliance. Site Review Overall, the facility is clean and well -maintained. Secondary containment is implemented as required. The outfalls were inspected and found to be in good order. No evidence of erosion or instability at the SDOs was observed. The receiving stream was free of pollution indicators. Please note that DEMLR strongly recommends hard -piping treated wastewater from the on -site treatment facility to the sanitary sewer rather than allowing it to trickle over pavement to the open grate. This will reduce the potential for stormwater contamination. Skvarla, III Secretary On Thursday, March 20, 2014 representatives of the Division of Water Resources, George Smith and Deborah Gore, along with representatives of the City of High Point, Carrie Boyd and Bill Frazier met with Mr. Kay Jacobs of Alberdingk Boley, Inc. The purpose of the meeting was to tour the facility where wastewater is generated and the pretreatment system and to discuss a proposed change to the method of discharge and Mr. Jacobs' concern over the way monthly average violations are calculated. I contacted the Region IV Pretreatment Program on March 26 and requested guidance on the calculation and compliance judgment for monthly averages. Flo responded back on April 8 that since there had been other similar questions he had requested an opinion from Headquarters. I will follow tip with you on this issue once I have received an answer. Alberdingk Boley, Inc. has made a proposal to allow them to discharge from the wastewater holding tanks directly to the sampling point without pretreatment if the wastewater in the tank meets permit limits for those parameters the industry is responsible for monitoring for: BOD, TSS, pH, cyanide, lead and zinc. The Pretreatment regulations mention representative samples in many places. In particular, 40 CI~ R 403.12 Reporting requirements for POTWs and industrial users, (g) Monitoring and analysis to demonstrate continued compliance, (3) states: "... For all other pollutants, 24-hour composite samples must be obtained through flow -proportional composite techniques, unless time -proportional composite or grab sampling is authorized by the Control Authority. Where time -proportional or grab sampling is authorized by the Control Authority, the samples must be representative of the discharge and the decision to allow the alternative sampling must be documented in the Industrial User file for the facility or facilities..." Mr. Jacobs has an in -line portable flow meter that can be used to accurately measure the flow leaving the tank. The in -line flow meter can display flow rate and total flow. Of greater concern is the ability of a single grab sample from a 6,000 gallon unmixed tank to represent the entire contents of the tank. During the tour it was noted that the wastewater generated from cleaning the reaction vessels has gelatinous looking solids. There is a screen prior to the wastewater holding tanks and it is cleaned monthly, but it is not clear how much of the solids may pass through the screen and into the holding tanks. The wastewater holding tanks can contain as much as 6,000 gallons. The treatment system handles only 2,000 gallons at a time. Up to 6 batches can be treated in a day. Since the wastewater holding tank is not mixed prior to emptying to the pretreatment system and treatment batches can be made up of wastewater from more than one tank , the wastewater entering the pretreatment system is not necessarily of consistent quality and therefore a single grab sample of the effluent cannot be termed a representative sample. There were other areas of concern noted during the facility tom'. There are 16 wastewater holding tanks with capacity of approximately 6,000 gallons each. 'fire filling hose must be manually moved from one tank to the next as the first one becomes filled. There is no high water level alarm on the tanks, or a site glass to determine the level of wastewater in the tank. Instead a hammer is used to tap the side of the tank to determine by the change in the sound where the level of wastewater is. The tanks are approximately 12' — 15' tall and would require a person to climb a ladder to determine the water level once it got above that person's reach. This method also depends on a person being able to distinguish tite sound difference that would indicate the level in the tank. This may be especially difficult in an industrial setting due to background machinery noise, ht addition, this method requires an operator to be in the area to check the level. Over filling of the wastewater tanks was cited as a cause for a discharge of approximately 300 gallons of untreated wastewater to the creek in August of 2013.The wastewater is transferred to the treatment system via a ]lose that is manually moved from tank to tank . The treated wastewater Ieaves the treatment system through a hole in the wall and trickles down the pavement to a grated, approximate 2' x 1', opening in the pavement to the sewer line. Any residual untreated wastewater on the pavement from connecting and disconnecting the hoses for filling and emptying the tanks, as well as rain water, also makes its way to the discharge point. In the event of a tank or transfer hose failure the entire volume of untreated wastewater could enter the collection system. The open pavement is a source of inflow and infiltration to the City's collection system and in most cases it is desirable to remove as much VI as possible to minimize wet weather issues. The Division has the following recommendations: 1. The industry investigate a method of discharge to the sewer that does not involve the treated . wastewater running across the pavement. 2. Close the open drain in the pavement to the collection system. 3. Unless there is a method for mixing the wastewater holding tanks prior to collecting a sample the Division does not support the discharge of the contents of the wastewater holding tanks without pretreatment. Aana sent me some stormwater information that was very helpful. I looked over the inspection report and it says that they have all necessary containment. But just a month later they had a spill to the creek due to over filling one of the wastewater tanks, so I'm a little confused. I only commented that it happened sort to demonstrate that is it a real concern. I didn't add much. And I didn't really cite the stormwater permit, but it does say that they should have secondary containment around bulk storage of liquid materials. My recommendations stayed the same, but I threw in a comment about i/1. I haven't had a chance to talk to Jeff about it and probably won't today and he goes on vacation next week, so I might ask for Matt's opinion (but he is so critical!) Sorry for the delay with this. Around 300 gallons ran off into the creek via outfall one. The spill was discovered at 7:20 am on 12 Aug 13 by an operator conducting the BMP checklist. The production manager was immediately notified and the extent of the spill was accessed. Dilute waste water was visible in the creek about 100 feet down from the outfall drainage pipe. A dike was constructed to prevent the spill from flowing further down the creek. Zebra Environmental was immediately notified and arrived onsite about 9:30 am. A vacuum truck was used to drain the creek of all standing water. The outfall was flushed with city water and drained again. A total of 6000 gallons of water was drained from the creek. At 5:00 pm I reported the spill via phone to the NCDENR division of water quality. You were present for the inspection. The purpose of the inspection was to review compliance with the National Pollutant Discharge Elimination System (NPDES) General Stormwater permit. The following are the findings from the subject inspection: • A Stormwater Pollution Prevention Plan (SPPP) has been developed and implemented. • There were no residuals from spills or leaks observed on the ground, nor in or near surface waters. • Qualitative monitoring has been conducted and recorded in accordance with permit requirements. • Alberdingk Boley took over operations of this facility in 2004, A 1997 letter from our Department granted Representative Outfall Status (ROS) & allowed one outfall to be ignored, with two additional outfalls allowed to be ignored should follow-up data support it. There are currently two outfalls being monitored, though it was not possible to match current outfall numbers with historical numbers due to the subsequent renumbering of the outfalls. The outfalls being monitored do adequately represent conditions on the ground. DATE OF INCIDENT: DATE OF REPORT: STORM WATER PERMIT: LOCATION: March 12, 2010 March 15, 2010 NPDES Stormwatcr Permit Coverage General Permit No. NCG060000 COC Number NCG060104 Guilford County Alberdingk Boley Inc. 6008 High Point Road Greensboro, NC 27407 Site Outfall Number 1 SUAVdARY• At approximately 11 AM on Friday March 12, 2010, mostly rain water plus some highly diluted water based acrylic was seen entering the storm water Outfall Number 1 at Alberdingk Boley Inc (ABI). Alberdingk Boley personnel diverted the storm water discharge into the site emergency basin on the company premises. in parallel, clean up of the creek began. The slightly white material had, extended about 50-60 yards down the unnamed storm water creek. The amount of water based paint was estimated at 10-20 pounds in an about 1,000 gallons of water. At 2 PK the creek was completely cleaned up of all white material, rinse with more clean water. Regulatory authorities were contacted as follows: Guilford County Environmental Water and the North Carolina Department of Water Quality. DETAILS: The white material in the creek was noticed at 11 AM by Alexander Snage who was making routine rounds of the site. The creek is designated on the storm water permit as "to receiving water designated as a UT to Ball Run, a Class C water in the Cape Fear River Basin." The Maintenance, Manufacturing and Shipping Department personnel immediately responded by diverting the storm water sewer into an emergency collection basin. Next, clean up began immediately. One vacuum truck and one service truck arrived. Simultaneously, site personnel walked the creek to determine the extent of the contamination. It was determined the extent was well short of the low lying marshy area. Further action conducted at Alberdingk Boley was completed to assure no further water left the site. This included double checking and securing the closure of storm water drains in this area. CAUSE: The source of this problem is an on -site sewer plugged. The sewer is not used very much. During a rain event, the sewer not known to be plugged backed up rain water up onto an interior and ran down interior ABI concrete road and into the storm sewer. It was detected just as it started raining. Because the sewer had some residual water based acrylic paint -type material, the rain water had a slight white color. SHORT & LONG CORRECTIVE ACTION: The short term corrective action is to unplug the sewer. This was completed within a couple hours of the above incident. The long term corrective action has two parts to ensure it does not happen again. First, the sewer will be clean out per annual maintenance schedule. A high pressure wash system should be used to ensure no periodic build up occurs. Second, modifications to the sewer will be reviewed to reduce or avoid such over flows or are more automatically diverted during these periods. These may include installation of a screen section in the large opening in the pipe. Having a large cross-section (few feet), this reduces any opportunity to pplug the remaining 8-12 inch sewer sections. Another idea considered is the installation of a pipe section to allow filling water to overflow into an adjacent containment basin. All will be evaluated for implementation.
