Your facility holds a general permit from the North Carolina Division of Water Quality to
discharge stormwater associated with industrial activities performed at your facility. Please be
advised that the Asheville Regional Office will be performing NPDES stormwater inspections
sometime in the near future.
If this office has not previously inspected your facility, you should be prepared to demonstrate
compliance with all terms and conditions included in the permit. Specifically, we will be
evaluating; the following: your stormwater pollution prevention plan, stormwater outfall
locations, qualitative and analytical monitoring data and any other activities required by your
permit. Please note that any data reported to the State must be analyzed by a facility that has a
North Carolina laboratory certification, either as a full laboratory or as a facility certified to
perform on -site field testing. If you have any questions regarding the generation of your
facility's data, please feel free to call Gary Francies at (828) 296-4677.

During rain:periods, myself and" a - contractor. were: flushing. drain lines outside on the east. side.:of our
plant. These drainsnave not been-used`€or some time and we were flushing-them-inpreparation-for our
kettle project that is- in .progress: Upon flushing the drains, :a cleanout 100: f . downstieam of-VA=e we
had = the high pressure hose -began to-, 8k . up and overflow_ It 'appeared to ' be a mixture of grater and
black grit and --dirt fiom the -old drain lines. Some grease p ugs.came out but -They, did not -run -to the
stormdrain- The- stormdrain is located about. 50 $ from. the cleanout_ I let the hose run. for 30 -minutes
hoping:the line cleared, but -when -I saw it wasn't: I closed the storm drain at wastewater and' -we began
flushing-. into the- line that was overflowingand. were able. to- unclog the- line. wi#1 be installing new
plugs into the cleanout lines -to prevent -any future incidents_ -Please feel free to contact -me •if there are

Enclosed please find a copy of the Compliance Evaluation Inspection form from
the inspection conducted on August 5 2005. Ms. Linda Wiggs and Mr. Larry Frost of the
Asheville Regional Office conducted the Compliance Evaluation Inspection. The
inspection was a follow-up to a reported spill. The facility was found to be in Compliance
with permit NCG060063.

The following items wiH be added at the -oven pit area -to help prevent any further spills:
1. A daily pit inspection on each shift for the wastewater pits on the plant side has started
2 A louder audible alarm has been installed on the control pans} for the pumps, a second alarm and beacon will be
installed in the blend and grind area.
3. Addition to spill policy that includes spiWhandHrtg procedure on the oven end is in place
4. A back flow devive will be installed in the outfall line from the receiving dock trench drain to eliminate any backup to
the trench area, by next weekend.
5 Drainage matswill-be placed at the truck scale stormdrainsat the parking -lot and will be removed -only. at a storm
event
6. Berm height will be increased going -to -the parking -lot sump -pump to help prevent any flow going across berm to storm
drain
7. A basket will be installed at the pit outW Une from the plant -to help elir4wte any debris getting in pump suction line

S.F.I. SPILL CONTAINMENT POLICY
In order to reduce the chance of a wastewater upset, a spill containment policy will
he implemented. The procedure will be as follows:
All water in the affected area will be shut off (production will continue if possible).
All pit pumps will be shut off.
Maintenance will notify the wastewater operator of the situation, type of spill and
the amount. (In the event there is no operator on site it will be the mechanic or his
supervisor who is responsible to complete the folio -tiring.)
'The wastewater operator will turn the appropriate valves at the hydraseive unit
(these valves have been painted and labeled).
The wastewater operator will notify maintenance that the valves have been changed.
At this time the pit pumps will be turned back'on and the spilled material will be
flushed through the system. After entering the storage tank it will be determined
what type of treatment is necessary.
The operator will turn the valves to their normal position but only after he is certain
that all materials are contained.
The following is the amounts of chemicals or oils that will be used as a guideline to
activate this policy.

SH 4VEN PIT SPILL POLICY
In order to prevent any spilis to stprmwater drains and surface waters, the
followin is the vrocedure for handling any s ill to the Parking lot
storm drains.
CONTACT YOUR SUPERVISOR 1M11 EDIIATIMY
All cleanup will cease immediately in the event of a high pit level.
In the event of a pit high level, alarms inside blend and grind and at the
pit area will sound.
In case of flow going to -the storm drains, mats must be in place on -the
storm d rains on each side -of the -truck scale to- stop or reduce any
amount going to surface waters.
Insure that the sump pump at the -parking lot steps is pumping if
needed.
Correct the pump problem -as -soon as possible in order to resume
normal cleanup. operations.
Onee the cause of the problem is resolved, cleanup of the affected area
should begin
Document the event and the time it took place and what steps were
taken- to resolve the problem
REMEMBER: The main- objective is to -eliminate -the possibility of any
spill going tor the river
All- mechanics must know how to implement this policy.

permit. This form must be completed and returned to the Division within 30 days of receipt of
this letter. DO NOT send the SPIPP with the signed form.
■ Five copies of Analytical Monitoring forms.
■ Five copies of Qualitative Monitoring forms.
A copy of a Technical Bulletin on the stormwater program with outlines program components
and addresses frequently asked questions.
■ A corrected Certificate of Coverage if you indicated a name or address change on the Renewal
Form returned to the Division.
Your certificate of coverage is not transferable except after notice to DWQ. The Division of Water
Quality may require modification or revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by
DWQ or permits required by the Division of Land Resources, Division of Air Quality, Coastal Area
Management Act or any other Federal or Local governmental permits that may be required.
