This permit is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection Agency dated
October 15, 2007 (or as subsequently amended).
Please take notice that this certificate of coverage is not transferable except after notice to the Division of
Energy, Mineral, and Land Resources. The Division may require modification or revocation and
reissuance of the certificate of coverage.
This permit does not affect the Icgal requirements to obtain other permits which may be required by the
Division of Energy, Mining, and Land Resources, or permits required by the Division of Water
Resources, Coastal Area Management Act, or any other federal or local governmental permit that may be
required.

Ennis -Flint Traffic Safety Solutions - Thomasville Facility
On May 23, 2016, Ennis -Flint Traffic Safety Solutions (Ennis -Flint) received a Notice of Violation (NOV)
due to a stormwater discharge from its facility located at 115 Todd Court in Thomasville, Davidson
County, North Carolina (Thomasville facility). On June 10, 2016, a response letter was submitted to the
North Carolina Division of Energy, Mineral and Land Resources (DEMLR) to detail the facility's
compliance with Stormwater regulations. The response letter indicated that the facility would apply for
coverage under National Pollutant Discharge Elimination System (NPDES) Stormwater General Permit
No.NCG050000.
With this submittal, Ennis -Flint requests coverage under General Permit No. NCG050000 for stormwater
discharges associated with industrial activity at the Thomasville facility. The following attachments
include DEMLR required items:

> Attachment 2 - USGS Map showing the location of the facility and a Site Drainage Map that shows

The facility applied for NCG05 per recommendation and NOV request from the DWR folks. According to the
application, the facility manufactures markers from preformed thermoplastic. The sister plant has an individual permit
that actually mixes the resins and such. Unless you have seen something different.

The initial site visit that lead to the application was done by Mike Mickey with DWR. I've attached a copy of an NOV
they issued for an illicit stormwater discharge.
When I looked up 115 Todd Ct, Thomasville I did notice that the water quality permit (WQ0036649) is listed as being in
Randolph County, the new stormwater permit in review (NCG050431) is listed as being in Davidson County. Not sure
which is correct or if it matters.

On May 20, 2016, Mike Mickey and April Norton of this office met with Joanna Reich and
Rob Mantek at the Ennis=Flint facility located at 115 Todd Court. The site visit was prompted by
a recent water quality complaint concerning a stream downslope of Ennis -Flint that was running
white. The inspection at the facility found that wastewater from bins stored outside and runoff
from the trash compactor area had been discharged illegally to the curbing along Todd Court that
leads to the .storm drain. The storm drain pipe eventually reaches a tributary of North Hamby
Creek, class "C" waters of the Yadkin -Poe Dee River Basin.
Please be advised that the discharge of any type of wastewater onto the lands or into the
waters of the State without a valid permit is a violation of North Carolina General Statute I43-
215.1. The wastewater discharges must be ceased immediately. This violation, or any future
violations, could be subject the Ennis -Flint to a civil penalty assessment of up to $25,000 per day,
per each violation.
Additionally, it was noted that the facility was in violation of the No -Exposure Certification

the stormwater inlet and leaking bins and used pallets were being stored outside. 'Ennis -Flint must
either obtain coverage under NPDES Stormwater Permit NCG050000 or comply with the No -
Exposure Certification which requires that all industrial materials and activities be protected by a
storm resistant shelter or relocated inside the building.
The Division of Water Resources requests that you respond in writing to this office within
twenty (20) days of receipt of this Notice. Your response should outline the actions that have been
taken to eliminate the illegal discharges of wastewater and the actions taken to comply with the
stormwater regulations.

Ennis -Flint Traffic Safety Solutions -- Thomasville Facility
On May 23, 2016, Ennis -Flint Traffic Safety Solutions (Ennis -Flint) received a Notice of Violation (NOV)
due to a stormwater discharge from its facility located at 115 Todd Court in Thomasville, Davidson
County, North Carolina (Thomasville facility). On June 10, 2016, a response letter was submitted to the
North Carolina Division of Energy, Mineral and Land Resources (DEMLR) to detail the facility's
compliance with stormwater regulations, The response letter indicated that the facility would apply for
coverage under National Pollutant Discharge Elimination System (NPDES) Stormwater General Permit
No. NCG050000.
With this submittal, Ennis -Flint requests coverage under General Permit No. NCG050000 for stormwater
discharges associated with industrial activity at the Thomasville facility. The following attachments
include DEMLR required items:
