On May 20, 2016 Glen White of the North Carolina Department of Environmental Quality;
Department of Minerals and Land Resources met with you at the facility located at 1241 Buck
Shoals Road, Hamptonville, to conduct a compliance inspection per the Industrial Stormwater
Permit requirements. Inspection consist of review of the Stormwater Pollution Prevention Plan,
training and monitoring records and observation of the general condition of the site, outfalls and
any possible exposures. Your assistance with the inspection was greatly appreciated.
Permit:
This facility holds General Stormwater Permit NCG050419 to discharge stormwater from
industrial activity associated with Apparel, Printing, Leather, Rubber & Miscellaneous
Manufacturing under the National Pollutant Discharge Elimination System (NPDES). The
permit became effective June 01, 2013 and expires on May 31, 2018. A copy of the current
permit is required to be maintained with the Stormwater Pollution Prevention Plan (SPPP)
documents. The current permit was available for review and was included in the SPPP.
Records/Reports:
This facility was required to develop and maintain a Stormwater Pollution Prevention Plan
(SPPP) in accordance with Part II, Section A of the permit. The SPPP along with monitoring
and training records need to be updated annually. The SPPP has been developed and is updated
annually. It is maintained in an electronic file but no hard copy is available. All required
components of the SPPP including the siteplan, training records, spill and monitoring records
need to be maintained in one 3-zing binder that is within easy reach for maintenance, review and
inspections by both your staff and by the inspector. Please provide one complete hard copy. All
records must remain with the file for a minimum of five (5) years.

Facilitv Site Review:
This facility produces thermal/acoustical metal and fiber engineered products that assist in heat
and noise abatement and thermal insulation within the transportation sector. Lydall's products
are mainly found in under body and under hood applications within vehicles.
The facilities and the site were clean and well maintained. The plant has been in
addition/renovation mode for the past few years and currently has an active land disturbance
permit. The condition of the grounds and outfall is good. Everything appears to be well
maintained.
Effluent Receiving Waters:
Effluent from this facility drains to Rocky Branch, Class WS-III waters of the Yadkin Pee Dee
River Basin.
Self -Monitoring Program:
Only qualitative monitoring is required under current permit requirements and current operating
procedures. Monitoring has been performed as required. No deficiencies noted.

Agreement between North Carolina and the US Environmental Protection Agency dated
October 15, 2007 (or as subsequently amended).
Please take notice that this certificate of coverage is not transferable except upon
approval of the Division of Energy, Mineral, and Land Resources. The Division of Energy,
Mineral, and Land Resources may require modification or revocation and reissuance of the
certificate of coverage.
This pdrmit does not affect the legal requirements to obtain other permits which may
be required by the Division of Energy, Mineral, and Land Resources, the Coastal Area

Aana Taylor -Smith of the Winston-Salem Regional Office of the NC Division of Energy, Mineral, and Land
Resources (DEMLR) conducted a compliance evaluation inspection at the subject facility on 10 December 2013.
Your assistance and cooperation during the inspection were greatly appreciated. An inspection checklist is
attached for your records and the inspection findings are summarized below.
This facility is located at 1241 Buck Shoals Road in Hamptonville, Yadkin County, North Carolina. Stormwater
related to industrial activity is discharged from the site to Rocky Branch, which is currently classified as WS-III
waters, in the Yadkin -Pee Dee River Basin.
Documentation & Monitoring Review
The facility's Stormwater Pollution Prevention Plan (SPPP) is thorough, up to date, and fully implemented.
Qualitative and analytical monitoring have been completed as required. Analytical monitoring results indicated
exceedances of benchmark values, which triggered the tiered response as required by the permit. Lydall
Thermal/Acoustical has initiated tiered response actions as required. Actions taken to identify pollutant sources
include contracting an engineer consultant, installing measures, taking soil samples, and resampling at outfalls.
This facility is in compliance and your proactive response is appreciated.
Lydall Thermal/Acoustical does not perform on -site vehicle maintenance. The facility currently has two
Stormwater Discharge Outfalls (SDOs) that discharge stormwater related to industrial activity, with plans for a
third SDO upon expansion.
Site Review
The facility is clean and well -maintained. Secondary containment has been implemented as required. The outfalls
were inspected and found to be in good order. No evidence of erosion or instability at the SDOs was observed.
The receiving stream was free of pollution indicators.

I just got a copy of Lydall's NCG05 NOI for their 1241 Buckshoals Rd, Hamptonville facility.
Whenever that one gets through you guys, it can be approved and their NCG030595 can be
rescinded so that they're covered under NCG05 rather than NCG03 at the Hamptonville site.

Hey there,
Just wanted to let you all know that Lydall Thermal/Acoustical submitted a rescission request
for their NCG03 permit at the Hamptonville facility (NCGO30595) before submitting an NOI for
a new NCGO5 for that facility. They want to switch from an NCGO3 to an NCGO5 to reflect their
primary industry (insulation materials). So if you get a rescission request for NCG030595 for
the Lydall Thermal/Acoustical facility on Buckshoals Road in Hamptonville, just hold on to it
until the NOI for an NCG05 at that facility comes in. This is totally separate from their
newly permitted NCG05 facility on Main Street in Yadkinville.
Sorry for the confusion! Hope that makes sense. If not, just let me know!

Based on a recent Email from Ms. Aana Taylor Smith representing NCDENR, we are submitting
a copy of a SW Permit Rescission Request Form to this office for the above mentioned facility
located at 601 East Main Street in Yadkinville, NC. This Lydall facility is covered under an
existing Stormwater Certificate of Coverage NCG030595. We have recently submitted a
Stormwater NOI and Ms. Taylor responded that the facility should be covered under a NCG05
permit requirements since this is the predominate manufacturing activity at the facility. To allow
the NOI to process, she requested that we submit the attached NCDENR Rescission Request
Form.

state — NPDES general permit. This permit is issued pursuant to the requirements of North
Carolina General Statute 143-215.1 and the Memorandum of Agreement. between North Carolina
and the US Environmental Protection Agency dated October 15, 2007 (or as subsequently
amended).
Please take notice that this certificate of coverage is not transferable except after notice to
the Division of Water Quality. The Division of Water Quality may require modification or
revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be
required by the Division of Water Quality or permits required by the Division of Land Resources.
Coastal Area Management Act or any other federal or local governmental permit that may be
required.

attached.
This certificate of coverage shall become effective September 23, 2010.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.
