On May 20, 2016 Glen White of the North Carolina Department of Environmental Quality;
Department of Minerals and Land Resources met with you at the facility located at 601 E Main
Street, Yadkinville, to conduct a compliance inspection per the Industrial Stormwater Permit
requirements. Inspection consist of review of the Stormwater Pollution Prevention Plan, training
and monitoring records and observation of the general condition of the site, outfalls and any
possible exposures. Your assistance with the inspection was greatly appreciated.
Permit:
This facility holds General Stormwater Permit NCGO50418 to discharge stormwater from
industrial activity associated with Apparel, Printing, Leather, Rubber & Miscellaneous
Manufacturing under the National Pollutant Discharge Elimination System (NPDES). The
permit became effective June 01, 2013 and expires on May 31, 2018. A copy of the current
permit is required to be maintained with the Stormwater Pollution Prevention Plan (SPPP)
documents. The current permit was available for review and was included in the SPPP.
Records/ReRorts.
This facility was required to develop and maintain a Stormwater Pollution Prevention Plan
(SPPP) in accordance with Part II, Section A of the permit. The SPPP along with monitoring
and training records need to be updated annually. The SPPP has been developed and is updated
annually. It is maintained in an electronic _file but no hard copy is available. All required
components of the SPPP including the siteplan, training records, spill and monitoring records
need to be maintained in one 3-ring binder that is within easy reach for maintenance, review and
inspections by both your staff and by the inspector. Please provide one complete hard copy, All
records must remain v"qth the file for a minimum of five (5) years.

Facility Site Review:
This facility produces thermal/acoustical fiber engineered products that assist in heat and noise
abatement and thermal insulation within the transportation sector. Lydall's products are mainly
found in under body and under hood applications within vehicles.
Mr. Smith walked the site with the inspector. The only exposures noted were the dumpster area
and the back docks where a significant number of totes are being stored. Product waste needs to
be cleaned up in the area of dumpsters. The backs docks area appears unkempt with waste
product on the ground, Please provide better housekeeping in these areas. Maintenance of these
areas should be scheduled to occur on a regular basis.
Effluent Receiviniz Waters:
Effluent from this facility drains to an unnamed tributary of Town Branch, Class C waters of the
Yadkin Pee Dee River Basin.
Self -Monitoring _Program:
Only qualitative monitoring is required under current permit requirements and current operating
procedures. Monitoring has been performed as required. No deficiencies noted.

Hi Larry,
I actually recently inspected this facility and I can confirm that they mainly make thermal/acoustical insulating products
for the automotive industry. If they receive the new NCG05 permit, wouldn't they be able to rescind the NCG03 permit,
since they should only have one permit for their primary industrial activity? I can talk to the engineer early next week
and see what he thinks about that. Other than the issue of rescission of their NCG03 permit, this N01 is good to go.

Hi Bradley,
I'll bring this N01 back to you momentarily. I think we should consider returning it, and getting them to resubmit for
NCG05,
I've looked at the NOI; I've looked at the company website; I've looked at Google Maps aerial photo of the site. Here's
what I see, and a record of the rationale for returning the application.
• The NOI is for NCG03, metals finishing. But the SIC code reported is SIC 3089, which we would normally cover
under NCG05, mist manufacturing. Rubber, sheet goods, etc. So, this contradiction catches our eye.
• From the website it appears they make automotive insulation products for both thermal and sound insulation- -
needle punched polyester carpet backing layers, foam carpet hacking layers, composite (rubber, I think) sheet
insulation. Some automotive exterior products include metal parts — like driveshaft tunnel insulation products.
Judging from the website's listing of products, the majority of the company's offerings are for interior purposes
and are sheet goods, and do not include metal.
• The cover letter implies to me that the engineer is unclear on whether to apply for NCG05 or NCG03 so he took
a guess. And since NCG03 is more stringent, he picked that one.
• NCG03 requires analytical testing for TSS, pH, TPH, Cu, Pb, Zn, and TTO for qualifying facilities.

Hi Larry,
i actually recently inspected this facility and I can confirm that they mainly make thermal/acoustical insulating products
for the automotive industry. If they receive the new NCG05 permit, wouldn't they be able to rescind the NCG03 permit,
since they should only have one permit for their primary industrial activity? I can talk to the engineer early next week
and see what he thinks about that. Other than the issue of rescission of their NCG03 permit, this NOI is good to go.

I'll brine this NOl back to -you momentarily. I think we should consider returnine it, and getting them to resubmit for
NCG05.
I've looked at the N01; I've looked at the company website; I've looked at Google Maps aerial photo of the site. Here's
what I see, and a record of the rationale for returning the application.
• The N01 is for NCG03, metals finishing. But the SIC code reported is SIC 3089, which we would normally cover
under NCG05, mist manufacturing. Rubber, sheet goods, etc. So, this contradiction catches our eye.
• From the website it appears they make automotive insulation products for both thermal and sound insulation- -
needle punched polyester carpet backing layers, foam carpet backing layers, composite (rubber, I think) sheet
insulation. Some automotive exterior products include metal parts — like driveshaft tunnel insulation products.
Judging from the website's listing of products, the majority of the company's offerings are for interior purposes
and are sheet goods, and do not include metal.
• The cover letter implies to me that the engineer is unclear on whether to apply for NCG05 or NCG03 so he took
a guess. And since NCG03 is more stringent, he picked that one.
• NCG03 requires analytical testing for TSS, pH, TPH, Cu, Pb, Zn, and TTO for qualifying facilities.
• NCG05 only requires visual monitoring, plus TPH and TSS for VMA, if the facility qualifies for VMA.

Period 1 (January -June 2014) 2014 forms for the January 10, 2014 site inspections and discharge
sampling at the Lydall facility located in Yadkinville, North Carolina (see Figures 1-2). The

Based on the permit conditions, we conducted a Tier l response and determined that there was
considerable galvanized materials exposed to precipitation (process -related materials, storage, and
scrap piles). We will take n3easure to better cover these materials in an effort to minimize
exposed surfaces. This Tier 1 response was recorded in the SPPP including the date and value of
the benchmark exceedance, the inspection date, personnel conducted the inspection, the selected
actions and the date the selected actions were implemented.

Facilities that incorporate a solvent management plan into the Stormwater Pollution Prevention Plan may so certify, and the requirement for TTO monitoring
may be waived. The solvent management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General
Permit. For those facilities electing to employ the TTO monitoring waiver, the discharger shall sign the following certification Statement:
"Based upon my inquiry of the person or persons directly responsible for managing compliance with the permit monitoring requirement for total toxic organics
(70), 1 certify that to the best of my knowledge and belief, no dumping of concentrated toxic organics into the stormwater or areas which are exposed to rainfall or
stormwater runoff has occurred since filing the last discharge monitoring report. I further certify that this facility is implementing the all the provisions of the
