On January 31, 2019 this facility was inspected for complaince. I met with Collin Breheny on site. No major issues were
observed during the inspection. however, it was noted that the site map was dated and did not include all information
required by the NCG050000 General Permit. The permittee is directed to review and update the site plan as soon as
possible. It was also noted that there is an excesive amount of erosion at an outfall observed and discussed on site. The
permittee is directed to evaluate and attempt to obtain permission to address the eroded areas as soon as possible.

I really appreciate that you are taking the time to help Jonathan and I with this. You have made this a
wonderful learning experience on both our parts, that is rare and greatly appreciated.
As for what you have asked me in the below e-mail. I checked with our Corporate Office - - Todd Weiderhold
(Director of EHS) is the person who is legally responsible for the permit. The Penn it/Own ershi p change form
does reflect this. Terry Nicolas is the Plant Manager and I am the contact person for both the facility and

permit. When I changed the contact information I just put my name in all the places Sheila had her name. I did
not read the form as carefully as I should have, thank you for catching that oversight so I did not create more of
a confusion than there already is.

We can only change Owner Affiliation with the Name/Ownership change, which it sounds like you've sent us today. Be

corporate officer of the company. The Owner Affiliation will be the person we consider the actual permittee (and fully

Maybe you should be listed as the Owner Contact person, rather than Owner Affiliation? You will be essentially
considered the "Owner" of the entire permit if we put you down as the Owner Affiliation. Is that how Printpack wants
to have the permit set up, and do you qualify under the above criteria, or is there an executive officer we should have
there?

I see Sheila signed the original NOI, even though the Plant Manager Terry Nicolas was printed under the signature
block. We should have requested a corrected signature page for the application when we reviewed it, but it wasn't
caught — she should not have signed for Terry.

discovered that the original map sent in with the permit is incorrect. Additionally the receiving streams and the latitude/
longitude for the outfalls are incorrect (this is what I got using Google Earth: 35.47.42,-82.38.57). I have corrected the
lat/long in BIMS and Tisa is going to submit a new map. Tisa asked me where to send the map and I told her to send it
to the same address that she sends the permit renewal to. Should she send it with the renewal or separately? Do you

prefer paper or digital format? (I have also attached a couple maps that I downloaded from ArcGiS online ... not the best
quality but the correct location ... the correct receiving stream is an UT to Profit Branch, a class C water in the French
Broad basin -index# 6-98-2) Your help in getting this corrected is greatly appreciated.

How are you? Hope you are well! Nancy (the neighbor) asked after you the other day — said she hoped you
are well!
Anyway, We've received an N01 from PrintPack Inc. for their printing facility in Marshall (Madison County) for
coverage under NCG05 (apparel & printing permit). The site discharges stormwater into what looks like both
' Hickroylog Branch and Shelton Laurel Creek — C, ORW and C, Tr, ORW, respectively. The NOI is attached.
There are several questions I had about this site and if you haven't done a site visit I think one might be in
' order before issuing this permit. They had an NCGNE. She is going to be requesting a recession for it because
they have industrial materials stored outside. She also said they have 10 silos with resin beads in them.
Additionally, I can't tell very well where they discharge to. She said they are on top of a mountain and its difficult to see
where their flow ends up — either in Hickorylog Branch or Shelton Laurel Creek. If you could provide some feedback on
ethat I'd appreciate it.
Does the Asheville Regional Office have any concerns about issuing this facility a COC for this general permit?

How are you? Hope you are well! Nancy (the neighbor) asked after you the other day — said she hoped you
are well!
Anyway, We've received an NOI from PrintPack Inc. for their printing facility in Marshall (Madison County) for
coverage under NCG05 (apparel & printing permit). The site discharges stormwater into what looks like both
Hickroylog Branch and Shelton Laurel Creek — C, ORW and C, Tr, ORW, respectively. The NOI is attached.
There are several questions I had about this site and if you haven't done a site visit I think one might be in
order before issuing this permit. They had an NCGNE. She is going to be requesting a recession for it because
they have industrial materials stored outside. She also said they have 10 silos with resin beads in them.
Additionally, I can't tell very well where they discharge to. She said they are on top of a mountain and its difficult to see
where their flow ends up — either in Hickorylog Branch or Shelton Laurel Creek. If you could provide some feedback on
that I'd appreciate it.
Does the Asheville Regional Office have any concerns about issuing this facility a COC for this general permit?
