On June 8, 2016 the City of Greensboro Stormwater Management Division completed an
industrial compliance inspection of Bonset America Corporation Local regulations permit an
inspection of industrial facilities under authority granted by City of Greensboro Code of
Ordinance Chapter 30, Article 12, Section 7 1 (C-5)
This inspection consisted of two parts a review of the facility files/records and an on -site facility
inspection Che inspection was performed by Sherri Scalcione and McKenzie Smith I he
following deficiencies/observations were noted during the inspection
Stormwater Pollution Prevention Plan/Spill Plan Review
Please review the most current permit issued and it is recommended that Bonset keep a copy of
the current permit with the SWP3 plan
Site Maps and SWP3
• Update the SWP3 plan annually, it is suggested that the review date and signature
appear as either an attachment page after the cover or on the cover page itself Please
refer to the permit Part 1I, Section A, item number 8 for the full description of the
information the plan requires updates to annually
• Verify during the next annual review of the plan that the site plan has all items shown
as listed in the permit Part II, Section A, item number 1(c)
• Recommend that an entry is made annually in the list of significant spills to document
no spills as well as spills
• Annual certification that stormwater outfalls have been evaluated for the presence of
non-stormwater discharges was not included in the SWP3 Part II Section A item
number 1(e)

Spill Prevention and Response Plan
• Locate the missing spill response contact page, verify all the information is up to date
and add into the SWP3 plan Part I1, section A item number 3
Monitoring
• No deficiencies observed
Employee Training
• Records with the SWP3 plan were dated 2010 Training has occurred since then and it
is believed the records arc with I iR Please be sure to have a records demonstrating
that Bonset is providing annual training available for inspections Part 1I, Section A
item number 6
Site Inspection
Preventative Maintenance/Good Housekeeping
• Recommend that Bonset place a cover (tarp) over the scrap metal bin to prevent
stormwater run off from the metals bin
• Clear the sediment and debris accumulated at Outfall 13
Non-Stormwater Discharges
• Investigate the cause of the white foam substance located at Outfall #1 Report back to
the City of Greensboro in writing by June 30'h, the cause and actions that have been
taken to prevent this from occurring in the future Any future discharges of this type
will result in a Notice of Violation from the City of Greensboro
It is recommended that Bonset America Corporation take appropriate actions to correct the noted

On October 25, 2012, Mike Mickey of this office met with Rick Love and Jason Neuman to
perform a NPDES Stormwater Permit Inspection at the Bonset America operation off Corporate Park
Drive in Browns Summit The facility was found to be compliant with the stormwatei permit The
inspection findings are detailed below and an inspection checklist is attached for your records
1 Permit
Bonset America holds NPDES Stormwater Permit No NCG050319 for the discharge of stormwater from
the facility The permit expires on May 31, 2013 The necessary renewal forms will be sent to you from
the Division in the near future
2 Records/Reports
Part 11, Section A of the permit requires the development of a Stormwater Pollution Prevention Plan
(SPPP) A copy of the SPPP, prepared in March 2003, was available for review during the inspection
Please note that Item 3(7) of Section A of the permit requires that the SPPP be reviewed and updated on
an annual basis The plan and site map will need to be updated to show tankage on the SW side of the
facility not currently on the site map and to include the new building addition and changes in stormwater
flow patterns on the NE side of the facility
3 Facility Site Review
A tour of the property found the site to be clean and well managed Observations did not reveal any
problems or concerns 1he secondary containment structures are pumped to the skimmer and discharged
to municipal sewer Records documenting the removal of stormwater from the containment structure were
available for years 2005-2009 No recent records were available Records should be kept documenting
the date and time of the removal, the individual conducting the removal and a description of the
accumulated stormwater

4 Effluent/Receiving Waters
Stormwater from the site is discharged from three outfalls into an unnamed tributary of Reedy Fork, class
"WS-V, NSW' waters in the Cape Fear River Basin Observations of the outfalls did not reveal any
concerns except for some slight sediment deposits at the outfall located in front of the facility This outfall
should be monitored to assure that the sediment and erosion control measures are functioning
satisfactorily
5 Monitoring Program
Part I1, Section B of the permit requires that qualitative (visual) monitoring be performed and documented
semi-annually the file review showed that this monitoring was performed as required The last two
monitoring events for all three outfalls occurred December 6, 2011 and April 17, 2012
The analytical monitoring for on -site vehicle maintenance referenced in fart 11, Section C of the permit is
not required since the facility uses less than 55 gallons of new motor oil when averaged over the calendar
year

bulk storage tanks (plasticizer, pellets and PVC resin powder) Please note that this
area had more tanks than shown on the SPPP site map The SPPP needs to be
updated to Include all tanks and the new building addition Stormwater from the
secondary contalment structure Is pumped to sanitary sewer
