Water Quality (DWQ) is forwarding herewith the reissued stormwater General Permit This permit is reissued pursuant to the
requirements of North Carolina General Statute 143-215 1 and the Memorandum of Agreement between the state of North
Carolina and the U S Environmental Protection Agency dated October 15 2007 (or as subsequently amended )
The following information is included with your permit package
• A new Certificate of Coverage
• A copy of stormwater General Permit NGG050000
• A copy of a Technical Bulletin for the General Permit
+ Five copies of the Discharge Monitoring Report (DMR) Form
• Five copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater only and it specifies your obligations with respect to stormwater
discharge controls, management monitoring and record keeping Please review the new permit to familiarize yourself with all
the changes in the reissued permit
The more significant changes in the General Permit since your last Certificate of Coverage include the following
• Permit cover page — Coverage is expanded to include industrial activities similar to the categorically identified industries
• Part I Section A — This part explains that the No Exposure Exclusion from permitting option may be available to the
permittee
• Part 11 Section A — Several minor expansions and clarifications of the required content of the Stormwater Pollution
Prevention Plan have been added Please be sure that your plan is updated to comply with the new requirements
• Part II Section B — Qualitative monitoring must now be conducted during a representative storm event
• Part 11 Section B — The permittee is required to keep a written record of his response to problems identified by his
qualitative monitoring observations
• Part If Section C - For facilities with vehicle maintenance activities using more than 55 gallons of new motor oil per month
new provisions require that discharges from the vehicle maintenance areas be sampled twice per year and that the
sampling results be compared to new numerical benchmark values (The previous cutoff concentrations have been
eliminated )
• Part 11 Section C — For facilities with vehicle maintenance activities using more than 55 gallons of new motor oil per month
new provisions require the permittee to execute Tier One and Tier Two response actions, based on the first benchmark
exceedence (Tier One) and the second consecutive benchmark exceedence (Tier Two) Tier Two requires that the
permittee institute monthly monitoring instead of twice -per -year monitoring until three consecutive monitoring events
show no benchmark exceedences

• Hearing protection is Mandatory in LM06/81 coating room, and mixing rooms, hearing protection will be provided to by your contact They are

Steel Toe Shoes are required in all manufacturing areas, use appropriate protective shoe for tasks you are performing outside -

• Our LockOut-TagOut Program is available for review and is located in the Safety Folder on all KTP computers See your contact if you require
our written LOTO procedures, you are responsible for supplying your own LOTO equipment `
• Confined Spaces are marked appropriately If you have questions refer them to you site contact
• Safety Data Sheets are available through our SDS database These can be accessed in the Safety Folder on all KTP computers Ask your contact
to print any SDS you require If you bring chemicals onto our site we require you to keep msds with you at all times the chemical is present on our

• No chemicals can be disposed of on -site other than household trash Remove all debris before leaving the site You are responsible for carrying
away all waste conduit, piping, pallets, and other packaging unless you have made specific arrangements with your contact ahead of time
• Before digging or excavation you are required to request a utility location marking service to mark the area you will dig in In NC, this can be
accomplished by dialing 811 Coordinate this with your site contact so it will not interfere with production
• No cell phones can be used inside the coating rooms or mix rooms No pictures are allowed to be taken inside the facility If photos are necessary
you must get them from your contact Flash from a camera will activate CO2 fire suppression systems located throughout the facility Do Not
Enter Areas marked with Orange CO2 suppression systems Warning Signs
• First Aid Supplies are located near the front offices Your contact can direct you to the first aid box
• The North Driveway is an exit only driveway It is also a right turn only driveway
• In case of fire you will hear an alarm and see strobe lights flashing Proceed to the nearest exit and make your way to the evacuation rally point
marked on the map above Leave immediately, do not re-enter until you have been instructed to by plant personnel We have CO2 extinguishing
systems If you are in a room where a fire occurs and the CO2 system activates leave immediately CO2 will cause suffocation if you do not leave
the room immediately you will die Re-entry into a room filled with CO2 after a fire may cause death Do not re-enter until you are told it is safe
• If you require a forklift to perform your job you your employees must be OSI IA certified This certification must be updated every 3 years It is
specific to the type lift you are operating You are responsible for obtaining this certification
• You and your KTP contact are responsible for creating a Safety Plan before you begin work I his may include reviewing some or all of the
