Resources (DEMLR) conducted a compliance evaluation inspection at the subject facility on 7 November
2013 Your assistance, as well as that of Mr Dwayne Lott, was greatly appreciated An inspection
checklist is attached for your records and the inspection findings are summarized below
This facility is located at 100 Ragsdale Road in Jamestown, Guilford County, North Carolina Stormwater
related to industrial activity is discharged from the site to an unnamed tributary to Deep River, this
segment of which is currently classified as Class WS-IV waters, in the Cape Fear River Basin
Documentation & Monitoring Review
The facility's Stormwater Pollution Prevention Plan (SPPP) is very thorough, up to date, and fully
implemented The SPPP was created by Pyramid Environmental and was last updated in 2012 Employee
training has not yet been completed for 2013, but is scheduled for November or December of this year
Qualitative monitoring has been completed as required Analytical monitoring is not required because
this facility does not perform on -site vehicle maintenance
Site Review
The facility is clean and well -maintained Secondary containment is implemented as required The
facility has two Stormwater Discharge Outfalls (SDOs) that discharge stormwater related to industrial
activity Both outfalls were inspected and found to be in good order No evidence of erosion or
instability at the SDOs was observed The receiving stream was free of pollution indicators

Dear Ms Geddle
On December 9, 2008, Jenny Graznak of this office performed a Stormwater Permit
Inspection at Highland Container located at 100 Ragsdale Road in Jamestown, North Carolina
This facility holds General Stormwater Permit No NCGO50258 under the National Pollutant
Discharge Elimination System (NPDES) to discharge Stormwater as activities classified as
Apparel, Printing, Leather, Rubber, etc Specifically, Highland Container manufactures
corrugated cardboard boxes You were present for the Inspection This type of Inspection
consists of two basic parts a review of facility files and self -monitoring data and an inspection of
the facility's discharge points This particular inspection evaluated five (5) areas, and
observations from each area are addressed below
1 Permit
The permit became effective June 1, 2008 and expires May 31, 2013 A copy of the permit was
available during the inspection
2 Records/Reports
As required by the Permit, the facility has a complete Stormwater Pollution Prevention Plan
(SPPP) that was available for review during the inspection The SPPP, which was created by
Pyramid Environmental in September 2000, appeared to be thorough and complete, including a
location map of the outfalls and a site plan listing potential stormwater pollutants and best
management practices used at the site Employee Training of the SPPP occurs on an annual
basis
3 Facility Site Review
The facility has two aboveground storage tanks one 10,000-gallon diesel tank and one 15,000-
gallon heating oil tank They both have secondary containment The site also has one covered
trash compactor and one uncovered dumpster that is owned by D H Griffin There are small
pieces of metal (used in manufacturing) littering the ground surrounding the uncovered
dumpster Please make sure these all make it into the dumpster rather than on the ground to
protect the nearby stream In addition, pallets are stored outside at this facility The permit
requires that while new and un-used pallets may be kept outside, used pallets must be covered
and kept out of the rain However, it was determined during the inspection that the only used
pallets kept outside are food -grade, untreated pallets, which is acceptable
4 Effluent/Receiving Waters
The facility has two stormwater discharge outfalls (SDO) Stormwater from the property
discharges to an unnamed tributary to Deep River (Class WS-IV) in the Cape Fear River Basin

5 Self -Monitoring Program
The stormwater permit requires semi-annual qualitative monitoring at stormwater outfalls An
on -site review of the monitoring data during the inspection showed that all parameters are
evaluated per the specifications of the permit You are exempt from the annual analytical
monitoring requirements since your vehicle maintenance activities occurring on -site do not use
more than 55 gallons of new motor oil per month when averaged over the calendar year
