On 9/12/2013 at 1:34pm Chris styles of Consolidated Metco (NCG050249) called to report a
small unintended release of an elastomeric roof coating. An independent contractor was
applying the roof coating at the facility when a rain storm popped up. The coating did not have
enough time to dry and less than five gallons of the product was washed off of the roof. The
runoff went down a storm drain aid into the creek. Mr. Styles noted a white milky sheen in the
storm drain. I asked Mr. Styles to review the MSDS sheet for the product and to survey the
creek downstream of the release. I also instructed Mr. Styles to document the occurrence in
their SPPP. Mr. Styles will call back if he notes any further problems in the creek related to this
event.

If you need to change the Owner (organization) Information or Owner Affiliation (the section right below the box that
says **Legally Responsible for Permit** and has red text), please fill out the Name/Ownership Change Form for each
p�rmit and submit it to our Unit. It can go to the Attention of Sheri Montalvo.
That is the easiest way for us to keep an official record of that Affiliation change request.
If you need to change the other contacts, i.e., Owner Contact Person, Permit Contact Person, and/or Facility Contact
Person, you can either (1) just email the new contact information directly to Sheri, or (2) strike through the information
on the attachments and fax or scan/email them back to us. You could also strike through and make corrections to the
contact summaries, and then attach those to the Name/Ownership form when you send those in (Option 3).
I think you said you were making these changes because you are retiring at the end of the year. If there's a date in the

CHRONIC HEALTH HAZARDS
Crystalline Silica (Quartz, Cristobalite) is listed by IARC and NTP. Long term exposure to high levels of silica dust, which can occur only when
sanding or abrading the dry film, may cause lung damage (silicosis) and possibly cancer.
IARC's Monograph No. 93 reports there is sufficient evidence of carcinogenicity in experimental rats exposed to titanium dioxide but
inadequate evidence for carcinogenicity in humans and has assigned a Group 26 rating. In addition, the IARC summary concludes, "No
significant exposure to titanium dioxide is thought to occur during the use of products in which titanium is bound to other materials, such
as paint."

The subject facility holds a general permit from the North Carolina Division of Water Quality to
discharge stonnwater associated with industrial activities performed at your facility. Please be
advised that staff of the Asheville Regional Office will be performing a stormwater inspection at
the subject facility sometime in the near future.
Some of the items you need to be sure are available for our review during the inspection are as
follows, a stortriwater pollution prevention plan, spill prevention and containment plan,
monitoring data/records for the site, including a copy of your Stormwater Permit, and records of
any training of your personnel as required by your permit. The inspection will also include a
visual inspection of the site, which includes the stormwater outfall location(s). Please note that if
you are required to collect samples, the samples must be analyzed by a North Carolina certified
laboratory or the facility must be certified by North.Carolina to perform on -site field testing. If

In response to your renewal application for continued coverage under the subject permit, the Division
of Water Quality (DWQ) is forwarding herewith the reissued stormwater general permit. This permit
is reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection agency
dated December 6, 1983.
The following information is included with your permit package:
■ A copy of the stormwater general permit.
■ A Stormwater Pollution Prevention Plan Certification Form. This form certifies that you have -
developed and implemented the Stormwater Pollution Prevention Plan (SPPP) required in your
permit. This form must be completed and returned to the Division within 30 days of receipt of
this letter. DO NOT send the SPPP with the signed form.
■ Five copies of Analytical Monitoring forms.
Five copies of Qualitative Monitoring forms.
■ A-copyof a Technical Bulletin on the stormwater program which outlines program components
and addresses frequently asked questions.
■ A corrected Certificate of Coverage if you indicated a name or address change on the Renewal
Form returned to the Division.
Your certificate of coverage is not transferable except after notice to DWQ. The Division of Water
Quality may require modification or revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by
DWQ or pernuts required by the Division of Land Resources, Division of Air Quality, Coastal Area
Management Act or any other Federal or Local governmental permits that may be required.
