evaluation ;nspection at the subject facility on October 23, 2015 Your cooperation and assistance with the inspection were greatly appreciated An inspection checklist is attached for your records and the inspection findings are summarized below 1 Permit This facility holds General Storm water Permit No NCO50243 to discharge Storm water from activities associated with Apparel, Printing, Converted Paper Products, Leather, Rubber Products or Miscellaneous Products, under the National Pollutant Discharge Elimination System (NPDES) The permit became effective June 1, 2013 and expires May 31, 2018 Your permit renewal package must be received in the Central Office in Raleigh no later than 180 days prior to the expiration date of the permit 2 Records/Reports Part II, Section A of the permit requires the development of a Stormwater Pollution Prevention Plan (SPPP) The plan must include all nine (9) subsections of Part It, Section A of the permit Qualitative monitoring is required to be completed and documented twice yearly as defined in Part II, Section C of the permit The SPPP plan must be updated annually and qualitative and analytical monitoring must be conducted and properly Paper 3 Facility Site Review Tammy Isaacs, Safety Manage, assisted the inspectors with review of the SP3 documents, monitoring records and a review of the site and outfalls The facility has a fenced outside storage yard for materials and pallets and a bay door for bringing materials in and out These locattions are the areas where pollutants could potentially get into the stormwater system In 2013, while an environmental firm hired to clean -out a tank was performing the task, they created a spill NCDEQ was notified and environmental clean-up began immediately Facility now stores a sand pile in the vicinity to aid with emergency clean-up 4 Effluent/Receiving Waters Stormwater outfalls the property and into an unnamed tributary of the Haw River (Class C, NSW) of the Cape Fear River Basin 5 Seif-Monitoring Program Analytical Monitoring This site is required to provide analytical monitoring under current operating procedures Monitoring records for 2015 were available for review Results were within parameters 6 Qualitative Monitoring All qualitative records do not need to be submitted to DEMLR, however, the records must be kept on file at the facility for a period of five NSW) in the Cape Fear River Basin via the two-abovementioned stormwater discharge outfalls 5 Self -Monitoring Program The stormwater permit requires semi-annual qualitative monitoring at both
