VAART Secretary On June 03, 2016 Glen White of the North Carolina Department of Environmental Quality, Department of Energy, Minerals, and Land Resources met with you at the facility located at 1431 Progress Avenue in High Point, to conduct a compliance inspection per the Industrial Stormwater Permit requirements Inspection consist of review of the Stormwater Pollution Prevention Plan, training and monitoring records and observation of the general condition of the site, outfalls and any possible exposures Your assistance with the inspection was gre,ltly appreciated Permit 1 his facility holds General Stormwater Permit NCG050174 to discharge stormwater from industrial activity associated with Manufacture of Paperboard Containers and Boxes [SIC 2651 under the National Pollutant Discharge Elimination System (NPDES) The permit became effective June 01, 2013 and expires on May 31, 2018 A copy of the current permit is required to be maintained with the Stormwater Pollution Prevention Plan (SPPP) documents The current permit was available for review and was included in the SPPP Records/Reports This facility was required to develop and maintain a Stormwater Pollution Prevention Plan (SPPP) in accordance with Part II, Section A of the permit The SPPP along with monitoring and training records have been updated annually Training records and pen/ink signatures are up to date through 2015 Training is conducted during the summer months Mr Houser also conducts a comprehensive monthly inspection Those records were provided for review as well The SPPP is well prepared the facility has remained in constant operation since that time ` Forklifts are serviced onsite by an outside vendor contracted by owner Used fluids are stored in tanks until removed for recycling by vendor Tanks in use at the facility 1 — 6000 Gal Fuel Oil for Boiler (Double Walled) I — 300 Gal Diesel Fuel for Sprinkler Pump (Double Walled & Inside Pump Room) — 10000 Gal Process Water Tank (Secondary Concrete Containment) -- 350 Gal Motor Oil (Inside Building w/Secondary Barrier) 1 — 500 Gal Used Oil Tank (Inside Building w/Secondary Barrier) Series of Totes (Hydraulic Oil, Gear Oil, Motor Oil) for servicing forklifts 2 — Starch Tanks l — Fire Protection Water Tank The facility also has a groundwater well that is available for use of needed to keep production going in time of water crisis The well is currently functional but has not been used Effluent Receiving Waters Effluent from the property drains to an unnamed tributary of Little River, Class C, NSW of the Yadkin -Fee -Dee River Basin Self Monitoring Programs The plant has (11) eleven outfalls the majority of them run along the rear of the facility Mr Houser has a sign labeled at each outfall for ease of locating when monitoring or inspecting Only qualitative monitoring is required under current permit requirements and current operating procedures Monitoring has been conducted as required The facility was
