records and the inspection findings are summarized below 2 The facility is located at 220 East Polo Road, in Winston-Salem, Forsyth County, North Carolina Stormwater related to industrial activity is discharged to Silas Creek, which is currently classified as Class C waters in the Yadkin Pee Dee River basin Documentation & Monitoring Review 3 The Stormwater Pollution Prevention Plan (SP3) is complete, current and has been implemented Documentation of SP3 annual reviews just began, however the outfalls have not been recertified as part of the annual review as required by the permit The SP3 was last reviewed in June, 2009 so it is due for another review/update It is also suggested that you change the WSROs number from 336-771-4600, to 336-771-5000, in the SP3, the former is actually a
