On August 2, 2016 Glen White of the North Carolina Department of Environmental Quality,
Department of Energy, Minerals, and Land Resources met with you at the Teknor Apex facility
located at 3518 Dillon Rd in Jamestown, to conduct a compliance inspection per the Industrial
Stormwater Permit requirements This inspection consists of review of the Stormwater Pollution
Prevention Plan, training, monitoring and spills records as well as observation of the general
condition of the site, the outfalls and any possible exposures to downstream stormwatf r Your
assistance with the inspection was greatly appreciated
Permit
This facility holds General Stormwater Permit NCG050081 to discharge stormwater from
industrial activity associated with Apparel, Printing, Leather, Rubber, & Miscellaneous
Manufacturing under the National Pollutant Discharge Elimination System (NPDES) The
permit became effective June 1, 2013 and expires on May 31, 2018 A copy of the current
permit is required to be maintained with the Stormwater Pollution Prevention Plan (SPPP)
documents The current permit was available for review and was included in the SPPP
Records/Reports
This facility was required to develop and maintain a (Stormwater Pollution Prevention Plan)
SPPP in accordance with Part I1, Section A of the permit The SPPP as well as the EAP
(Emergency Action Plan) and all other documents were well prepared and maintained Annual
training, spill prevention reporting, corrective actions/response are recorded and well
documented All qualitative, monitoring records are required to be maintained with the SPPP for
a minimum of five (5) years Records are maintained in the Mr West computer system as well
as in printed form

Facility Site Review
Teknor Apex produces the product used by their customer's to create PVC (polyvinyl chloride)
pipe and molded plastic products Teknor's product is in the form of rigid and flexible
compound and pellets The facility has nine (9) silo's that contain PVC resin and a central
vacuum collection system for dust collection One (1) tank contains DOTP in liquid form The
tank is maintained inside a concrete diked area to prevent spills No vehicle maintenance is
performed onsite other than facilities maintenance for propane and electric forklifts No gasoline
or diesel forklifts are used Waste products are disposed of in one (1) trash compactor
Batteries, used oil and light bulbs are collected and picked up for recycling
The building is approximately 80,000 SF under cover The plant operates three (3), eight (8)
hour shifts, five (5) days per week with approximately 70 employees total Employee training
for stormwater in provided to all employees Pen/ink signatures are provided The facility has a
Safety Committee that includes managers and employees
Effluent Receiving Waters
Effluent from this facility discharges to an unnamed tributary of Deep River, Class WS-IV
waters of the Cape Fear River Basin via three (3) outfalls All were observed to be free of
debris and easily reached for monitoring at the time of inspection
Self Monitor Program
Part I1, Section C of the permit requires semi-annual qualitative monitoring No analytical
monitoring is required under current operating procedures and permit requirements Monitoring
has been conducted as required

I am writing in response to your letter dated 3/29/12 with regards to our Stormwater Permit No
NCGO50081
On your recent inspection of the Viking Polymers facility, we were found deficient on a couple of items
and this letter is to advise you that the required changes have been completed They are as follows
1 We do have a site map in our position and did have at the time of the inspection Unfortunately, I
was just unable to produce it at the time
2 The two drain valves on our silo containment system have been changed so that they ale of a
locking type and are indeed kept locked Keys are in a lock box which is maintained in our
maintenance department as they are in charge of opening for periodic draining A record is now
being kept as to when the releases are performed
3 The dumpster area has been cleaned up and is now cleaned during each change -out or emptying
of the compactor These instructions have been add to our Stormwater Pollution Prevention Plan
4 Semi-annual monitoring and documentation has resumed with this morning's rain event
I believe this covers all our stated deficiencies mentioned in your letter Should you require additional

On March 23, 2012, Mike Mickey of this office met with you to perform a NPDES Stormwater
Permit Inspection of Viking Polymers The facility was found to be noncompliant vath the permit
recordkeepmg requirements as outlined in Items 2 and 5 below
1 Permit
Viking Polymers currently holds NPDES Stormwater Permit No NCG050081 for the discharge of
stormwater from the facility The permit expires next on May 31, 2013
2 Records/Reports
Part II, Section A of the permit requires the development of a Stormwater Pollution Prevention Plan
(SPPP) A copy of the 2008 SPPP was available for review during the inspection Please note that Item
3(7) of Section A of the permit requires that the SPPP be reviewed and updated on an annual basis In
addition, the SPPP did not contain a site map Item 1(c) of Section A of the permit requires that a site map
be developed of the property showing the stormwater discharge points, industrial activity areas, direction
of Stormwater flow, etc
3 Facility Site Review
A tour of the property found the following items that need to be addressed, 1) the release valve on the
plasticizer containment structure was unlocked Item 2(b) of Section A of the permit requires that
secondary containment release valves be secured with a locking mechanism In addition, records of any
release must be maintained to document the date, time and individual making the release a,, well as visual
observation of the released stormwater, and, 2) runoff was occurring at the compactor dtlmpster from
material that was spilled underneath The spilled material should be removed and this area should be
listed in the Preventative Maintenance and Good Housekeeping section of the SPPP per Item 4 of Section
A of the permit

4 Effluent/Receiving Waters
Stormwater from the site is discharged to the Deep River, Class "WS-IV,CA *" waters in the Cape Fear
River Basin Observations of the outfalls did not reveal any concerns
5 Monitoring Program
Part II, Section B of the permit requires that qualitative (visual) monitoring be performed and documented
semi-annually during a representative storm event (> 0 finches of rainfall) A review of the facility files
found that this monitoring was not being performed and documented as required The semi-annual
monitoring should be performed once at each outfall between January and June and between July and
December of each year
The analytical monitoring for on -site vehicle maintenance referenced in Part II, Section C of the permit is
not required since the facility uses less than 55 gallons of new motor oil when averaged over the calendar
year
Please be aware that the failure to comply with any permit conditions is a violation of the General
Permit Per the permit text in Part III, Section A (2), "The permittee must comply with all conditions of
this general permit Any permit noncompliance constitutes a violation of the Clean Water Act and is
grounds for enforcement action " Additionally, per the permit text in Part III, Section A (2)(c), "Under
state law, a daily civil penalty of not more than ten thousand dollars ($10,000) per violation may be
assessed against any person who violates or fails to act in accordance with the terms, conditions, or
requirements of a permit [Ref NC General Statute 143-215 6A]"
In summary, the SPPP for Viking Polymers needs to be updated to incorporate all Items listed in Part
II, Section A of the permit (i e , site map) and the qualitative monitoring needs to be performed per Part 11,
Section B of the permit The locking mechanism on the containment structure and the issues at the
compactor dumpster also need addressing You are requested to respond in writing to this office on or
before April 27, 2012, providing a status of the items listed above

Implement a Stormwater Pollution Prevention Plan (SPPP) A copy of a SPPP from
2008 was found in the facility files Please note that Item 3(7) of Section A of the permit
requires that the SPPP be updated annually In addition, Item 1(c) of Section A requires
that a site map be developed of the property showing stormwater discharge points,
industrial activity areas, direction of stormwater flow, etc

Comment The analytical monitoring specified in Part 11, Section C of the permit is not
required since on -site vehicle maintenance is not performed at the facility and less than
55 gallons of new motor oil used each month when averaged over the calendar year

Comment The facility has three outfalls that need to be numbered and specifically
identified on the new site map The only issues noted during the outside Inspection of
the facility involved the following
1) The release valve on the plasticizer containment structure was unlocked Item 2(b)
of Section A requires that secondary containment release values be secured with a
locking mechanism In addition, records shall be maintained that document the date,
time and individual making the release from the containment structure as well as visual
observations of the released stormwater
2) Runoff was occuring at the compactor dumpster from material that was spilled
underneath The spilled material should be removed This area should be listed in the
Preventative Maintenance and Good Housekeepping section of the SPPP per Item 4 of
Section A of the permit

PART IV - EVALUATION AND RECOMMENDATIONS
The Division conducted a review of the facility on February" 2,
1993 Fisher Nut Inc roast and packages peanut that have been
blanched The nuts are roasted and coated with sugar water and
salt, then packaged Tne company packages 45,000 to 50,000 lbs per
shaft At the time of the review there was only one shift
operating
An air compressor is used to clean off equipment and surrounding
area City water flows through the compressor to keep it cool, and
the effluent from the compressor discharges into a drainage ditch
that is tributary to Queen Anne's Creek The compressor operates
continuously Please note there are no chemical additives and no
treatment There is a meter on the compressor which is used to
determine the amount of discharged Presently, the facility
discharges an averages of 3700 gpd ( with a high of 5600 gpd and a
low of 2000 gpd)
It is recommended to issue a general permit to the facility for
non -contact cooling water discharge
