United Plastics has recently received a Notice of Deficiency, from your office, of our
General Storm water Discharge, Permit NCG050069 This was the result of a site visit and
inspection conducted by, Mr Glen White Your notice, called out deficiencies observed when
conducting a compliance inspection of our facility located at 511 Hay Street in Mount Airy We
are working to resolve all of the issues at this time
The current permit has been added to our files, along with an electronic scanned copy in
the record for review during future visits and/or inspections conducted by your office While we
have been performing visual inspection and monitoring of our two outfall locations, we have not
always recorded our findings We now have an electronic version of the inspection which will be
included in our records and kept in the proper location
We intend to comply with all permit requirements therefore, at this time we are updating
our Stomwater Pollution Prevention Plan (SPPP), and a schedule for annual updates and
training, with instructions for training records retention to be maintained for five years

On March 3, 2016 Glen White of the North Carolina Department of Environmental Quality —
Minerals & Land Resource,, met with you at the subject facility located at 511 Hay Street in
Mounty Airy to conduct a compliance mspection for the facilities Industrial Stormwater Permit
Permit
This facility holds General Stormwater Permit NCG050069 to discharge stormwater from
industrial activity classified as Miscellaneous Manufacturing Industries [SIC 31 under the
National Pollutant Discharge Elimination System (NPDES) The permit became effective June
1, 2013 and expires on May 31, 2018 A copy of the current permit is required to be maintained
with the Storrriwater Pollution Prevention Plan (SPPP) documents The current permit was not a
part of the record for review at the time of inspection
Records/Reports
This facility was required to develop and maintain a Stormwater Pollution Prevention Plan
(SPPP) in accordance with Part II, Section A of the permit The inspector reviewed the SPPP
and each of its components I he SPPP has not been updated annually as required and it appears
that nothing has been updated since the initial SPPP was developed in 1998 The file did not
contain the current annual Certificate of Coverage (COC) and annual training records and
qualitative monitoring has not been performed as the permit requires
Facility Site Review
Mr Edwards walked the site with the inspector and reviewed the outfall This facility has one
outfall near the rear of the property A ditch drains into an unnamed tributary located at one of
the side property lines There is a containment area for truck that deliver resins If there are
spills it can easily be contained and cleaned up properly without ever entering the waters of the
State The facility and grounds appear compliant although a large area outside and to the side of
the facility appears unkempt and in need of additional housekeeping

Self -Monitoring Program
This facility is required to provide Qualitative Monitoring at it's one outfall bi-annually No
Analytical Monitoring is required under current operating procedures or permit requirements
Qualitative Monitoring has not be(-n performed as required
The following are Deficiency Issues to be corrected
1 Update all components of the Stormwater Pollution Prevention Plan (SPPP) Must be
reviewed and updated annually This can be done in house Go through the SPPP each
year and note any changes to operating procedures or changes to the facility Add a
revision sheet to sign off on Annual Updating
2 Provide Annual Certificate of Coverage (COC) in the SPPP This is a one page letter that
must be printed at the time annual fee is paid
3 Begin Qualitative Monitoring twice annually Keep records in SPPP for minimum of five
(5) years
4 Establish Required Training and provide annually This can be accomplished by adding
to one of the annual Safety Training sessions Have attendee's sign and date log and
keep in SPPP
Although the facility itself appeared to be well maintained with the exception of the storage area
out back, it was found to be non -compliant with its industrial stormwater permit requirements
due to non-compliance in keeping the SPPP current and by not providing monitoring or
stormwater training as the permit requires

Prevention Plan (SPPP) A copy of the SPPP was available for review The plan contained
all of the items required by the stormwater permit Please note that the permit requires the

clean and well managed

forwarding herewith the subject certificate of coverage to discharge under the subject state -
NPDES general permit This permit is issued pursuant to the requirements of North Carolina
General Statute 143-215 1 and the Memorandum of Agreement between North Carolina and the
US Environmental Protection agency dated December 6, 1983
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual
permit application Unless such demand is made, this certificate of coverage shall be final and
binding
Please take notice that this certificate of coverage is not transferable except after notice to the
Division of Environmental Management The Division of Environmental Management may
require modification or revocation and reissuance of the certificate of coverage
This permit does not affect the legal requirements to obtain other permits which may be
required by the Division of Environmental Management or permits required by the Division of
Land Resources, Coastal Area Management Act or any other Federal or Local governmental
permit that may be required.
