Inspection Summary:
Susan Wilson and Jonathan Stepp (DWQ) met James Macomson and Terry McCoy with Wiisonart at the site. The
company makes laminate on site from resin. The site appeared well maintained and the records appeared to be complete
at the time of this inspection.
DWQ suggests that the site map be updated to indicate all drop inlets and the direction of drainages for any outfalls.
Records of any storm water discharges from secondary containment areas should be maintained (esp. the resin storage
area).

In response to your renewal application for continued coverage under stormwater General Permit NCG050000 the Division of
Water Quality (DWQ) is forwarding herewith the reissued stormwaler General Permit. This permit is reissued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North
Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as subsequently amended.)
The following information is included with your permit package:
• A new Certificate of Coverage
• A copy of stormwater General Permit NCG050000
• A copy of a Technical Bulletin for the General Permit
• Five copies of the Discharge Monitoring Report (DMR) Form
• Five copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater only, and it specifies your obligations with respect to stormwater
discharge controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself with all
the changes in the reissued permit.
The more significant changes in the General Permit since the your last Certificate of Coverage include the following:
• Permit cover page -- Coverage is expanded to include industrial activities similar to the categorically identified industries.
• Part I Section A — This part explains that the No Exposure Exclusion from permitting option may be available to the
permittee.
• Part II Section A — Several minor expansions and clarifications of the required content of the Stormwater Pollution
Prevention Plan have been added. Please be sure that your plan is updated to comply with the new requirements.
• Part 11 Section B — Qualitative monitoring must now be conducted during a representative storm event.
• Part II Section B — The permittee is required to keep a written record of his response to problems identified by his
qualitative monitoring observations.
• Part II Section C - For facilities with vehicle maintenance activities using more than 55 gallons of new motor oil per month,
new provisions require that discharges from the vehicle maintenance areas be sampled twice per year, and that the
sampling results be compared to new numerical benchmark values. (The previous cutoff concentrations have been
eliminated.)
• Part 11 Section C — For facilities with vehicle maintenance activities using more than 55 gallons of new motor oil per month,
new provisions require the permittee to execute Tier One and Tier Two response actions, based on the first benchmark
exceedence (Tier One) and the second consecutive benchmark exceedence (Tier Two). Tier Two requires that the
permittee institute monthly monitoring instead of twice -per -year monitoring, until three consecutive monitoring events
show no benchmark exceedences.
• Part II Section C — This part contains a new provision that four exceedences of any particular benchmark will trigger
increased DWQ involvement in the permittee's stormwater management and control actions. DWO may direct the
permittee to apply for an individual permit, or may direct the implementation or installation of specific stormwater control
measures.

In compliance with the provision of North Carolina General Statute 143-215.1, other lawful standards and
regulations promulgated and adopted by the North Carolina Environmental Management Commission, and the
Federal Water Pollution Control Act, as amended,
Ralph Wilson Plastics Company
is hereby authorized to discharge stormwater from a facility located at
Ralph Wilson Plastics Company
Cane Creek Industrial Park
Fletcher
Henderson County
to receiving waters designated as Cane Creek in the French Broad River Basin
in accordance with the effluent limitations, monitoring requirements, and other conditions set forth in Parts I, II, III
and IV of General Permit No. NCG050000 as attached.
This certificate of coverage shall become effective April 23, 1993.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.

GENERAL SAFETY REQUIREMENTS:
Your safety while at our site is very important to us.
The following safety requirements apply to both
employees and visitors for the protection of
everyone on the premises.
All visitors must sign in at the front reception area
and will be issued a badge. Return badge to
receptionist, or escort, upon completion of visit.
1. Badges must be worn and visible at all times.
2. Safety glasses are required in the
manufacturing area.
3. Stay with your designated escort or within your
designated work area.
4. if an emergency alarm sounds, follow your
escort's instructions. Exit facility immediately
if instructed to do so.
5. Material handling equipment may be in use
throughout the facility. Be alert to lift truck traffic,
particularly when approaching blind spots and
intersections. All lifts are equipped with horns
to notify pedestrians, however, it is a good
practice to make eye contact with the operator
before proceeding.
6. Because steel -toed safety shoes are required
in work areas, you should make every effort to
remain in the main aisleways. No open -toed
or open -heeled shoes, or heels above 2 inches
are allowed in these areas.
7. Wilsonart facilities are tobacco free. Tobacco
use is limited to designated outside break
areas.
8. Food and beverages are allowed only in
designated areas.
9. Do not leave an unattended vehicle in the
roadway or blocking access to fire hydrants or
dock areas. Please wear seat belts.
10. Consumption or possession of alcohol or
drugs, or possession of weapons is not allowed
on site, including parking lots.
11. All chemical substances brought on site must
be accompanied by an MSDS and be approved
by Wilsonart supervision for use.
12. Hearing protection will be provided when
necessary.
13. Camera, camera cell phones and recording
devices are not allowed to be used on Wilsonart
premises.
14. Cellular phone use is restricted to office areas.
15. Wilsonart is not responsible for your personal
property or tools.
16. Harassment or violence is not permitted or
tolerated.
17. In emergencies contact a Wilsonart
representative immediately. Emergency
number are posted on all telephones.
18. If you have any questions about the above
requirements, or Wilsonart, please ask your
escort.

inspect and view certain facilities and premises of
Wilsonart, that
I will follow all Wilsonart safety guidelines and
remain in my designated area,
I will not, without prior written approval of an
authorized representative of Wilsonart, disclose or
in any other way make known, reveal, report,
publish, or transfer to any person, firm or
corporation, or myself utilize, any business or
technical information or know-how of Wilsonart (the
"Information"), including but not limited to the
operation, plans, specifications, facilities, drawings,
layouts, blueprints and other materials relating to
the machinery, equipment, processes and products
made in said facilities, except for such information
which is now or which hereafter becomes a part of
the public domain by reason of other than a breach
hereof, and
I hereby release and discharge Wilsonart, its
officers, directors, employees and representatives,
or and from any and all claims of causes of action
I may have, or which my heirs, successors and
others represented by me may have, as a result of
any loss, damage, injury, or death, arising out of or
connected in any way with my visit to, observation
of or work performed in said facilities.
