This permit is reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as subsequently amended). The following information is included with your permit package: • A new Certificate of Coverage (CDC) • A copy of General Permit NCG030000 • A copy of the Technical Bulletin for the General Permit • Two copies of the Discharge Monitoring Report (DMR) Form • Two copies of the Qualitative Monitoring Report Form The General Permit authorizes discharges of stormwater, and it specifies your obligations for discharge controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself with all the changes in the reissued permit. Your facility has six (6) months from the time of receipt of the permit to update your current SPPP to reflect all new permit requirements. The first sample period of your permit begins January 1, 2013. Your facility must sample a "measureable storm event" beginning during the periods beginning January 1 and July 1 of every year (or, if applicable, report "No Flow," as outlined in Part III, Section E). Also, please note that Tier 3 Actions in Part II of your permit are triggered by benchmark exceedances on four occasions beginning on the effective date of this permit and do not count prior exceedances. The more significant changes in the General Permit since your last CDC was issued are noted either in the Draft Permit Fact Sheet that accompanied the public notice (http://portal.ncdenr.org/web/wq/ws/su/current- notices), or in the Response to Comments / Summary of Changes and Technical Bulletin documents that are posted on the stormwater Permitting Unit's website with the new General Permit. Additional conditions for specific industry sectors have been added to the SPPP requirements in some cases. • Sections B, C: Failure to perform analytical stormwater monitoring may result in the Division requiring that the permittee begin a monthly sampling scheme. • Sections B, C: A lower TSS benchmark of 50 mg/I for HQW, ORW, PNA and Tr Waters applies to these more sensitive waters. • Sections B, C. The monitoring parameter Oil & Grease (0&G) has been replaced by the parameter Total Petroleum Hydrocarbons (TPH) for vehicle maintenance areas, and in some cases, other analytical monitoring requirements. • Sections B, C, D: Inability to sample due to adverse weather must be recorded in the SPPP, or in separate on -site records if your General Permit does not require an SPPP. Adverse weather is defined in the "Definitions" section of the permit. • Sections B, C: The term "Representative Storm Event" has been replaced by "Measurable Storm Event." A measurable storm event is defined in the permit. • Section D: If the permittee fails to respond effectively to problems identified by qualitative monitoring, DWQ may require the permittee to perform corrective action. Please review Parts III and IV to understand the Standard Conditions of your new NPDES General Permit, including Compliance and liability, Reporting, Monitoring and Records requirements; Operation and Maintenance obligations; and Definitions. Please note that all samples analyzed in accordance with the terms of this permit must be submitted to the Division on Discharge Monitoring Report (DMR) forms available on the Stormwater Permitting Unit's website above. DMR forms must be delivered to the Division no later than 30 days from the date the facility receives the sampling results from the laboratory. Also note that existing permittees do not need to submit a, renewal request prior to expiration unless directed by the Division. Your coverage under the General Permit is transferable only through the specific action of DWQ. This permit does not affect the legal requirements to obtain other permits which may be required by DENR, nor does it relieve the permittee from responsibility for compliance with any other applicable federal, state, or local law, rule, standard, ordinance, order, judgment, or decree. The permit number is COC No. NCG030581 SECTION A: STORMWATER POLLUTION PREVENTION PLAN SITE PLAN This facility is located on the Northeast side of Beulavilie, North Carolina in Duplin County at 196 Hwy 41 North, Beulaville, North Carolina. PHC has operated a manufacturing facility at this site since January 15, 2002. A general site map is attached with storm water drainage areas. 2. This facility manufactures hydraulic cylinders and components for the materials handling industry. The main processes are comprised of machining and painting operations. 3. All interior drains are connected to the sanitary sewer line. No non-stormwater discharges can reach the storm sewers. Stormwater drainage from the building roofs and parking lots are the only areas of drainage from this facility. Drainage is discharged from all sides of the building to open storm drains that flow down gradient in a Northeasterly direction to an unnamed tributary of Limestone Creek. 4. The roof of the manufacturing and maintenance areas can collect particulate from various process stacks. This particulate can potentially be discharged with the stormwater runoff. 5. All hazardous materials are stored inside the manufacturing building. 6. Air compressors and allied housings are located on the South side of, and attached to, the manufacturing building. This facility has no history of significant spills or leaks of pollutants having occurred since the inception of operation January 15, 2002. STORMWATER MANAGEMENT PLAN Precision Hydraulic Cylinders Inc. recognizes the importance of good management practices as a means to minimize waste and to promote accident and fire prevention and worker health and safety. Many of the management practices in place reflect Best Management Practices. This section of the plan identifies those practices, which impact upon potential stormwater contamination, and indicates where common practices will be tailored to prevent stormwater pollution. 1. Good Housekeeping is essential to maintaining an orderly and safe work environment. These practices, when extended to include potential sources of stormwater contamination, are the most effective first step toward pollution prevention by limiting the potential exposure of materials for contamination of the stormwater. The basic housekeeping procedure, which will minimize the potential for storm water contamination, is the routine inspections for signs of spills, leakage or other signs of contamination. If a release spill is discovered, it is cleaned up promptly. In addition, al areas are routinely inspected for miscellaneous debris, which is removed and disposed properly. 2. Preventive Maintenance of all machinery assets is a part of this facility's general policies and, by reference, shall become a part of this plan. 3. A Feasibility Study has been completed to change the method of handling machining chips for removal by a steel scrap company. Currently, the machining chips are conveyed into a scrap hopper as they are generated from the machining processes. The cutting and coolant fluids, used by the machining processes, are collected in these hoppers. These hoppers are then carried out to a large chip container that is located under a shelter to protect the contents from stormwater pollution. The concrete floor under this shelter has a slope to the center for gravity drainage to a sump of any fluids that may be remaining in the containers. The fluids then drain by gravity to a storage tank, where the stored fluids will be picked and hauled off by a licensed carrier. The results of this Feasibility Study will further improve upon Best Management Practices and minimize pollution from stormwater from our machining chip disposal. In addition, it is the plan of this facility to place all outside areas under shelter that are exposed to stormwater. These areas include the solid steel containers, the scrap wood containers, and the materials stored at the East end of the building. 4. A Visual Inspection of all stormwater devices will be conducted quarterly by the Industrial Engineering Manager or site Maintenance Manager. Where a leak or other condition is discovered that may result in significant releases of pollutants to the drainage system, corrective action shall be taken immediately or the process will be shut down until corrective action can be taken. When a leak or non -containment has occurred, contaminated soil, debris or other material will be promptly removed and disposed of in accordance with Local, State and Federal regulations. This facility maintains spill control materials including spill absorbent and/or spill pads (pillows) to absorb the spill and spill socks for containment. The absorbed materials, under this contingent plan, will be disposed of in accordance with Local, State and Federal regulations. 2. Employee Training Employees will be trained in the essential elements of housekeeping, the objective of the plan, and other appropriate safety and health topics that would support this plan. All employees, at the time of employment, are trained in the plan and its provisions. At the initial implementation of the plan, all employees will be informed of its provisions and objectives. All employees who work with hazardous materials or the waste oil/coolant storage tank or who might have reason to come in contact with materials in these areas, will be trained specifically on the following: • Spill prevention and response and preventive maintenance for that specific area of the facility. • Applicable pollution control laws and regulations. • The facility's Stormwater Pollution Prevention Plan • Feature and operations of the facility that are designated to minimize discharges of hazardous materials. Coverage under this general permit is applicable to all owners and operators of storm water point source discharges associated with activities classified as establishments primarily engaged in the rolling, drawing, and extruding of nonferrous metals [standard industrial classification (SIC)335], fabricating of metal products (SIC 34), manufacturing of industrial and commercial machinery (SIC 35), manufacturing of electronic equipment (SIC 36), manufacturing of transportation equipment (SIC 37), and manufacturing of measuring and analyzing instruments (SIC 38). The following activities are specifically excluded from coverage under this General Permit: Ship and boat building and repairing (SIC 373), which is covered by general storm water permit NCG190000. Permit is valid through October 31, 2012. SECTION A: STORMWATER POLLUTION PREVENTION PLAN SITE PLAN 1. This facility is located on the Northwest of Wallace, North Carolina in Duplin County at 145 SouthPark Drive, Wallace, North Carolina. PHC has operated a manufacturing facility at this site since January 2005. A general site map is attached with storm water drainage areas. 2. This facility manufactures hydraulic cylinders and components for the materials handling industry. The main processes are comprised of machining and painting operations. 3. All interior drains are connected to the sanitary sewer line. No non -storm water discharges can reach the storm sewers. Storm water drainage from the building roofs and parking lots are the only areas of drainage from this facility. Drainage is discharged from all sides of the building to open storm drains that flow down gradient in a Northeasterly direction to an unnamed tributary of Island Creek.. The roof of the manufacturing and maintenance areas can collect particulate from various process stacks. This particulate can potentially be discharged with the storm water runoff. 5. All hazardous materials are stored inside the manufacturing building. 6. Air compressors and allied housings are located on the west side of, and attached to, the manufacturing building. 7. This facility has no history of significant spills or leaks of pollutants having occurred since the inception of operation January 2005. STORM WATER MANAGEMENT PLAN Precision Hydraulic Cylinders Inc. recognizes the importance of good management practices as a means to minimize waste and to promote accident and fire prevention and worker health and safety. Many of the management practices in place reflect Best Management Practices. This section of the plan identifies those practices, which impact upon potential storm water contamination, and indicates where common practices will be tailored to prevent storm water pollution. 1. Good Housekeeping is essential to maintaining an orderly and safe work environment. These practices, when extended to include potential sources of storm water contamination, are the most effective first step toward pollution prevention by limiting the potential exposure of materials for contamination of the storm water. The basic housekeeping procedure, which will minimize the potential for storm water contamination, is the routine inspections for signs of spills, leafage or other signs of contamination. If a release spill is discovered, it is cleaned up promptly. In addition, all areas are routinely inspected for miscellaneous debris, which is removed and disposed properly. 2. Preventive Maintenance of all machinery assets is a part of this facility's general policies and, by reference, shall become a part of this plan. 3. A Visual Inspection of all storm water devices will be conducted quarterly by the Industrial Engineering Manager or site Manager. A completed inspection form will be filed with the Compliance Manager as a part of this plan. Where a leak or other condition is discovered that may result in significant releases of pollutants to the drainage system, corrective action shall be taken immediately or the process will be shut down until corrective action can be taken. When a leak or non -containment has occurred, contaminated soil, debris or other material will be promptly removed and disposed of in accordance with Local, State and Federal regulations. This facility maintains spill control materials including spill absorbent and/or spill pads (pillows) to absorb the spill and spill socks for containment. The absorbed materials, under this contingent plan, will be disposed of in accordance with Local, State and Federal regulations. 2. Employee Training: Employees will be trained in the essential elements of housekeeping, the objective of the plan, and other appropriate safety and health topics that would support this plan. Training will be done after new employees probationary period and may be included in annual or specialized training events. At the initial implementation of the plan, all employees will be informed of its provisions and objectives. DATE PRINTED: October 09 2W4 1.0 Purpose PHC has committed to comply with the prevention guidelines of the oil or waste paint solvents Pollution Prevention Regulation, the Spill Prevention Control Plan will describe in detail the procedure for prevention of an accidental discharge of oil or waste paint solvents into the environment. The Plan will meet the requirements of 40 CFR Chapter 1, Part 112 and will be drafted using good engineering practices. PHC will make revisions to the Plan, as necessary, to provide a Plan that reflects current conditions at the facility in accordance with 40 CFR, Chapter 1 Part 112.5 2.0 Overview A Spill Prevention Control Plan is a facility -specific description of a facility's containment and countermeasures that would prevent an oil or waste paint solvents spill from occurring as well as procedures to respond to and clean up an oil or waste paint solvents spill that does occur. The Plan addresses the following areas: o Operating procedures that prevent oil or waste paint solvents spills. These procedures are to be adhered to in the event of catastrophic failure of any oil or waste paint solvents storage facility including, but not limited to sumps, tanks, drums, reservoirs and transformers and/or a reasonable potential for equipment overflow or leakage o Control measures installed to prevent a spill from reaching the environment o Countermeasures to contain, clean up, and mitigate the effects of an oil or waste paint solvents spill that reaches the environment o Appropriate containment and/or diversionary structures or equipment to prevent spilled oil or waste paint solvents from reaching navigable waters o Notification protocol for the appropriate personnel o Investigation and reporting requirements of the suspected event. 3.0 Applicability of SPCC Requirements The Clean Water Act requires "facilities that store, transport, or handle oil or waste paint solvents and could reasonably be expected to discharge oil or waste paint solvents in harmful quantities to navigable waters" to prepare spill prevention, control, and contingency plans. At PHC there is no reasonable expectation that any spilled material(s) in harmful quantities would ever reach navigable waters. PHC has however made the commitment to follow the guidelines of 40 CFR Chapter 1, Part 112 to the extent to manage and report any spills that enter the environment. Many other substances are considered oil or waste paint solvents, including mineral oil or waste paint solvents, vegetable and animal oil or waste paint solvents origins, and other non -petroleum oil or waste paint solvents. The Spill Prevention Control Plan procedure outlined below in section 7.0 will include: o Where and how oil or waste paint solvents is used and stored o Preventative maintenance procedures o Inspection procedures o Emergency response procedures, including who would be contacted if a spill were to occur and how it would be managed. 4.0 Facility Drainage and Secondary Containment The drainage area for the site is shown on the attached site map. Secondary containment will be utilized in the form of dikes, curbing, trenches, catch basins, and spill pallets. 5.0 Facility oil or waste paint solvents Sources and Control measures There are several different oil or waste paint solvents sources at the facility that will be considered for the Plan. Attached, along with each source, is the capacity and secondary containment, if applicable. See attachment. 6.0 Control To ensure control of stored oil or waste paint solvents, both new and waste, the containers will be in secure locations within the plant and/or in protected locations with limited access by plant maintenance personnel. Containers shall be identified clearly has to what they contain. There shall be no oil or waste paint solvents containers placed in a manner that will be directly open to the environment. 7.0 Procedure 7.1 General Overview Any transfer, operation, or testing involving oil or waste paint solvents must have a procedure with this Plan attached. All leaks will be addressed as follows: 0 IF PRINTED, THIS DOCUMENT IS VALID ONLY ON THE DATE PRINTED. DATE PRINTED: October 09 2014 o Any leaks that develop in an area or equipment must be addressed immediately o Proper absorbent must be readily available for rapid clean up. o If the leak cannot be corrected immediately, use an appropriate collection container to prevent further spillage. As schedule and system operations permit, the leak will be fixed in a controlled manner. o All material used for clean up shall be disposed of properly. All waste on site will be stored in proper containers. Persons managing any waste transfer will be responsible for ensuring that the hazardous water removal company employed conforms to appropriate regulations. o A significant oil or waste paint solvents spill is defined as a spill on permeable ground greater than one gallon of oil or waste paint solvents. 7.2 Spill Response and Countermeasures PHC's policy for reacting to an oil or waste paint solvents spill is as follows: o Take all necessary action to stop the source of the spill. o If the spill is minimal, located on the shop floor (concrete) and can be cleaned up by normal housekeeping methods; then mop up the spill and dispose of the mop water by normal procedures into the wastewater holding tank. o Notify maintenance personnel of the spill. Use appropriate absorbent materials to soak up and detain the flow and spread of oil or waste paint solvents o All materials used for cleanup shall be disposed of properly. The waste removal company shall handle any major cleanup effort and transfer of oil or waste paint solvents waste from the site. o Notify the NCDNR of the spill status and ask for a letter of "no further action" o It will be the Industrial Engineering Manager's responsibility to ensure an Incident Report is correctly and completely filled out and Section 8.0, of this procedure, Spill History is updated. The US EPA (40 CFR 110) mandates that facilities report discharges and submit SPCC Plans after having 2 discharges over 42 gallons in any 12-month period.
