electronic crystals, or manufacture cathode ray tubes. For purposes of this permit the definition of Total Toxic Organics is that definition contained in the EPA
Effluent Guidelines for the facility subject to the requirement to sample (for metal finishing use the definition as found in 40 CFR 433,11; for semiconductor
manufacture use the definition as found in 40 CFR 469.12; for electronic crystal manufacture use the definition as found in 40 CFR 469.22; and for cathode ray
tube manufacture use the definition found in 40 CFR 469,31).

Facilities that incorporate a solvent management plan into the Storm water Pollution Prevention Plan may so certify, and the requirement for TTO monitoring
may bewaived. The solvent management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General
Permit. For those facilities electing to employ the TTO monitoring waiver, the discharger shall sign the following certification statement:
"Based upon my inquiry of the person or persons directly responsible for managing compliance with the permit monitoring requirement for total toxic organics
(TTO), 1 certify that to the best of my knowledge and belief, no dumping of concentrated toxic organics into the Stormwater or areas which are exposed to rainfall or
stormwater runoff has occurred since filing the last discharge monitoring report. I further certify that this facility is implementing the all the provisions of the
solvent management plan included in the Stormwater Pollution Prevention Plan."

Facilities that incorporate a solvent management plan into the Stormwater Pollution Prevention Plan may so certify, and the requirement for TTO monitoring
may be waived. The solvent management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General `
Permit. For those facilities electing to employ the TTO monitoring waiver, the discharger shall sign the following certification statement:
"Based upon my inquiry of the person or persons directly responsible for managing compliance with the permit monitoring requirement for total toxic organics
(TTO), I certify that to the best of my knowledge and belief, no dumping of concentrated toxic organics into the stormwater or areas which are exposed to rainfall or
stormwater runoff has occurred since filing the last discharge monitoring report. I further certify that this facility is implementing the all the provisions of the
solvent management plan included in the Stormwater Pollution Prevention Plan."

may be waived. The solvent management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General

Dear Mrs. Smith,
Mickey Truck Bodies Inc. is a manufacturer of beverage delivery bodies, trailers and also build dry and
refrigeration van bodies. Primary operations at the facility include welding, cutting, and the bending/painting of
aluminum and steel for truck bodies. The facility includes offices, paint booths, dry booths and fabrication
plants.
According to a storm water sample that was taken on 6/30/15 and received on 7/28/15, we were noncompliant
on copper, zinc and total suspended solids. Copper tested at 0.042 mg/L, with the allowable limit being 0.007
mg/L. Zinc tested at 1.28 mg/L, with the allowable limit being 0.067 mg/L. Total Suspended Solids tested at
160 mg/L, with the allowable limit being 100mg/L. These allowable limits are according to our storm water
permit NCG030517.
On 7/30/15, we had an inspection of the facility to identify any factors that would have contributed to the
noncompliant results. This inspection was conducted by Joshua Soto, EHS Manager. Findings were that the
overages could have come from multiple locations throughout the facility.
Below are the items that we believe contributed to the high levels of Zinc, Copper and Total Suspended Solids:
• We use aluminum, steel, stainless steel, galvenneal and various other types of metals to manufacture our
product. Due to this, we have multiple units made from this material on the yard. Rain water washes
zinc and copper off these units and into the storm water drains.
• We use welding to build our truck bodies. This welding wire contains copper that could be washed off
of a unit's exterior welds and into the storm drains.
• We have multiple scrap dumpsters out on the yard that contain steel and aluminum. These metals all
contain zinc and copper that can be washed off by rain and into the storm drains.
• We have vehicles that are driving inside our facility throughout the day. These vehicles create brake
dust from the brake pads that are used to stop the unit, and this dust contains large amounts of zinc and
copper. Our facility is also surrounded on three sides by busy streets. When it rains, brake dust from

our trucks and other cars driving down the road can be washed into our storm water drains, adding to .
this issue.
Dirt and sediment from the surrounding roads and the ground contain zinc and copper that could be
washed into our storm water drains.
• We store new aluminum and steel in racks that are out on the yard. When it rains zinc and copper could
be washed off and into the storm drain.
• Weeks of dry weather prior to the qualifying rain event may have allowed solids to buildup resulting in
higher levels during the rain event.
Mickey Truck Bodies will sample again no later than August 30, 2015 and provide these results to your office
showing all levels to be within compliance.
Following the site inspection Mickey Truck Bodies will incorporate the following action items to help reduce
the total suspended solids washed into the storm drain:
• Utilize an outdoor floor sweeper to collect large debris on the grounds that could be washed into the
storm drains.
• Install large drain filters at the drains closest to the trash compactors to catch any debris being washed
into the storm drain.
Mickey Truck Bodies would also like to request a change of our Authority Contact in our permit
NCG030517 from Dean Sink to Matt Sink.

5 Total Toxic Organics sampling is applicable only for those facilities which perform metal finishing operations, manufacture semiconductors, manufacture
electronic crystals, or manufacture cathode ray tubes. For purposes of this permit the definition of Total Toxic Organics is that definition contained in the EPA
Effluent Guidelines for the facility subject to the requirement to sample (for metal finishing use the definition as found in 40 CFR 433.11; for semiconductor
manufacture use the definition as found in 40 CFR 469.12; for electronic crystal manufacture use the definition as found in 40 CFR 469.22; and for cathode ray
tube manufacture use the definition found in 40 CFR 469.31.).
Facilities that incorporate a solvent management plan into the Stormwater Pollution Prevention Plan may so certify, and the requirement for TTO monitoring
may be waived. The solvent management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General
Permit. For those facilities electing to employ the TTO monitoring waiver, the discharger shall sign the following certification statement:
"Based upon my inquiry of the person or persons directly responsible for managing compliance with the permit monitoring requirement for total toxic organics
(TTO), I certify that to the best of my knowledge and belief, no dumping of concentrated toxic organics into the stormwater or areas which are exposed to rainfall or
stormwater runoff has occurred since filing the last discharge monitoring report. I further certify that this facility is implementing the all the provisions of the
solvent management plan included in the Stormwater Pollution Prevention Plan."
