3. Facility+ Site Review
This facility provides heat treating of metals to change the material properties to make it harder, etc. to meet
the client's specific need. Profitability has steadily declined at this location over the past few years and the
corporate office has decided to close this location permanently in 2016. Employees has just been given the
news not long before this inspection took place. The inspection covered a review of the Stormwater
Pollution Prevention Plan, monitoring and training records and observation of each of it's outfalls. The site
was clean and well groomed. No obvious deficiencies noted.
4. Eff luent/Receiving Waters
Stormwater from this facility discharges into an unnamed tributary of the Dan River (Class C) waters of the
Roanoke River Basin.
S. Self -Monitoring Program
Analytical Monitoring: This site is required to provide analytical monitoring under current conditions of the
permit. Monitoring has been performed twice annually as required. Results were within parameters.
Overall, the condition if the site is well maintained. The SP3 plan and monitoring records are complete and
compliant. If you have any questions concerning this letter or the attached inspection report, please contact
Glen White at (336) 776-9660.

Subject: NPDES Stormwater Permit Coverage Renewal
CCHT, LLC- Reidsville
COC Number NCG030506
Rockingham County
In response to your renewal application for continued coverage under stormwater General Permit NCG030000
the Division of Water Quality (DWQ) is forwarding herewith the reissued General Permit. This permit is
reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October
15, 2007 (or as subsequently amended).
The following information is included with your permit package:
• A new Certificate of Coverage (COC)
• A copy of General Permit NCG030000
• A copy of the Technical Bulletin for the General Permit
• Two copies of the Discharge Monitoring Report (DMR) Form
• Two copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater, and it specifies your obligations for discharge
controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself
with all the changes in the reissued permit. Your facility has six (6) months from the time of receipt of the
permit to update your current SPPP to reflect all new permit requirements.
The first sample period of your permit begins January 1, 2013. Your facility must sample a "measureable
storm event" beginning during the periods beginning January 1 and July 1 of every year (or, if applicable,
report "No Flow," as outlined in Part III, Section E). Also, please note that Tier 3 Actions in Part II of your
permit are triggered by benchmark exceedances on four occasions beginning on the effective date of this
permit and do not count prior exceedances.
The more significant changes in the General Permit since your last COC was issued are noted either in the
Draft Permit Fact Sheet that accompanied the public notice (http://portal.ncdenr.org/web/wq/ws/su/current-
notices), or in the Response to Comments / Summary of Changes and Technical Bulletin documents that are
posted on the Stormwater Permitting Unit's website with the new General Permit. Please visit
http://Portal.ncdenr.org/web/wq/ws/su/npdessw (click on 'General Permits' tab) to review that information
for your specific General Permit carefully.

Some of the changes include:
Part II:
• Section A: The Stormwater Pollution Prevention Plan (SPPP) section, if applicable, has been updated
to the most current language of our permits. Additional conditions for specific industry sectors have
been added to the SPPP requirements in some cases.
• Sections B, C: Failure to perform analytical stormwater monitoring may result in the Division requiring
that the permittee begin a monthly sampling scheme.
• Sections B, C: A lower TSS benchmark of 50 mg/I for HQW, ORW, PNA and Tr Waters applies to these
more sensitive waters.
• Sections B, C: The monitoring parameter Oil & Grease (O&G) has been replaced by the parameter
Total Petroleum Hydrocarbons (TPH) for vehicle maintenance areas, and in some cases, other
analytical monitoring requirements.
• Sections B, C, D: Inability to sample due to adverse weather must be recorded in the SPPP, or in
separate on -site records if your General Permit does not require an SPPP. Adverse weather is defined
in the "Definitions" section of the permit.
• Sections B, C. The term "Representative Storm Event" has been replaced by "Measurable Storm
Event." A measurable storm event is defined in the permit.
• Section D: If the permittee fails to respond effectively to problems identified by qualitative
monitoring, DWQ may require the permittee to perform corrective action.
Please review Parts III and IV to understand the Standard Conditions of your new NPDES General Permit,
including Compliance and Liability, Reporting, Monitoring and Records requirements; Operation and
Maintenance obligations; and Definitions. Please note that all samples analyzed in accordance with the terms
of this permit must be submitted to the Division on Discharge Monitoring Report (DMR) forms available on
the Stormwater Permitting Unit's website above. DMR forms must be delivered to the Division no later than
30 days from the date the facility receives the sampling results from the laboratory. Also note that existing
permittees do not need to submit a renewal request prior to expiration unless directed by the Division.
Your coverage under the General Permit is transferable only through the specific action of DWQ. This permit
does not affect the legal requirements to obtain other permits which may be required by DENR, nor does it
relieve the permittee from responsibility for compliance with any other applicable federal, state, or local law,
rule, standard, ordinance, order, judgment, or decree. If you have any questions regarding this permit
package, please contact the Division's Stormwater Permitting Unit at (919) 807-6300.

Dear Ms.Revel:
Mike Thomas of the Winston-Salem Regional Office of the NC Division of Water Quality
(DWQ) conducted a compliance evaluation inspection (CEI) at the subject facility on 12
October 2010. Your assistance and cooperation during the inspection was greatly
appreciated. An inspection checklist is attached for your records and the inspection findings
are summarized below.
The facility is located at 628 Grooms Road, in Reidsville, Rockingham County, North
Carolina. Stormwater related to industrial activity is discharged to an unnamed tributary to the
Dan River, which is currently classified as Cass C waters in the Roanoke River Basin.
Documentation and Monitoring Review
The Stormwater Pollution Prevention Plan (SP3) is complete and well implemented.
Training sessions are conducted annually to keep staff informed and updated. You also
indicated To Mr. Thomas that you often work with employees one-to-one to ensure that the
SP3 is being followed.
Bluewater Thermal has five stormwater discharge outfalls (SDO) and all were well
marked and easily accessible for inspection. Records and sampling results from qualifying
events were made available for inspection and appeared to be complete. Sample results for
the last two qualifying events at SDO 004 indicated that the bench mark of 100 mg1L for Total
Suspended Solids (TSS) was exceeded. You indicated that the source was determined to be
wastewater from a floor sweeper being dumped into a dumpster at the loading dock, upstream
of SDO 004. You have corrected the situation by no longer allowing employees to dispose of
the waste in that manner and that the loading area is now cleaned on a weekly basis. As you

and Mr. Thomas discussed during the inspection, exceeding a benchmark value on two
successive samples now requires that SDO 004 be monitored for TSS under the Tier Two
schedule as outlined on page 7 of Permit No. NCG030000. Tier Two requires three
successive test results for TSS below the benchmark before you can return to your regular
sampling routine.
Site Review
The site was very clean and well maintained at the time of inspection. Exposed
industrial activity at the facility was minimal. Only minimal maintenance was suggested at
SDO 004 to help curb erosion. The containment dike around the process oil storage area is in
need of maintenance. It did not appear to be leaking at the time of the inspection. You
indicated to Mr. Thomas that plans were underway to have the dike system totally replaced.
We greatly appreciate your efforts to develop and implement an effective stormwater
pollution prevention plan. Please continue your efforts to help safeguard the waters of our
State. Should you have any questions, please feel free to contact Mr. Thomas or me at (336)-

Subject: NPDES Stormwater Permit Application Fees
Carolina Heat Treating
Reidsville and Arden Plants
A) C C? O 30507
Your applications for individual permits for the stormwater discharges from the Subject plants has been received. The fee for
the individual permits is $715 each. We received your two checks last week for $715 each to cover each of the applications.
We have recently reissued our General Stormwater NPDES permit, NCG030000, and included heat treating facilities in the
new version of NCG030000. This means that instead of individual permits (fee of $715 application and each year) that you
can qualify for General Permit coverage (fee of $80 application and each year). Based upon your discussion with Valery
Stephens today, we are returning herein the two $715 checks and you will issued two new checks for $80 each to us.
Please attach a copy of this letter with the new checks, loose checks that come to the agency without some attached
identifying letter become quickly lost and never seem to get to the right place.
Thank you for your cooperation and we are pleased to be able to now offer the General Permit for these facilities.
If you have any questions, please contact me at 919-733-5083x548 or email at bill.mills@ncmail.net.
Sincerely.
