In response to your renewal application for continued coverage under stormwater General Permit NCG030000
the Division of Water Quality (DWQ) is forwarding herewith the reissued General Permit. This permit is
reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October
15, 2007 (or as subsequently amended).
The following information is included with your permit package:
• A new Certificate of Coverage (COC)
• A copy of General Permit NCG030000
• A copy of the Technical Bulletin for the General Permit
• Two copies of the Discharge Monitoring Report (DMR) Form
• Two copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater, and it specifies your obligations for discharge
controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself
with all the changes in the reissued permit. Your facility has six (6) months from the time of receipt of the
permit to update your current SPPP to reflect all new permit requirements.
The first sample period of your permit begins January 1, 2013. Your facility must sample a "measureable
storm event" beginning during the periods beginning January 1 and July 1 of every year (or, if applicable,
report "No Flow," as outlined in Part III, Section E), Also, please note that Tier 3 Actions in Part II of your
permit are triggered by benchmark exceedances on four occasions beginning on the effective date of this
permit and do not count prior exceedances.
The more significant changes in the General Permit since your last COC was issued are noted either in the
Draft Permit Fact Sheet that accompanied the public notice (http://Portal.ncdenr.org/web/wq/ws/su/current-
notices), or in the Response to Comments / Summary of Changes and Technical Bulletin documents that are
posted on the Stormwater Permitting Unit's website with the new General Permit. Please visit
http://Portal.ncdenr.org/web/wq/ws/su/npdessw (click on 'General Permits' tab) to review that information
for your specific General Permit carefully.

Some of the changes include:
« Part II:
• Section A: The Stormwater Pollution Prevention Plan (SPPP) section, if applicable, has been updated
to the most current language of our permits. Additional conditions for specific industry sectors have
been added to the SPPP requirements in some cases.
• Sections B, C: Failure to perform analytical stormwater monitoring may result in the Division requiring
that the permittee begin a monthly sampling scheme.
• Sections B, C: A lower TSS benchmark of 50 mg/I for HQW, ORW, PNA and Tr Waters applies to these
more sensitive waters.
• Sections B, C. The monitoring parameter Oil & Grease (O&G) has been replaced by the parameter
Total Petroleum Hydrocarbons (TPH) for vehicle maintenance areas, and in some cases, other
analytical monitoring requirements.
• Sections B, C, D: Inability to sample due to adverse weather must be recorded in the SPPP, or in
separate on -site records if your General Permit does not require an SPPP. Adverse weather is defined
in the "Definitions" section of the permit.
• Sections B, C. The term "Representative Storm Event" has been replaced by "Measurable Storm
Event." A measurable storm event is defined in the permit.
• Section D: If the permittee fails to respond effectively to problems identified by qualitative
monitoring, DWQ may require the permittee to perform corrective action.
Please review Parts III and IV to understand the Standard Conditions of your new NPDES General Permit,
including Compliance and Liability, Reporting, Monitoring and Records requirements; Operation and
Maintenance obligations; and Definitions. Please note that all samples analyzed in accordance with the terms
of this permit must be submitted to the Division on Discharge Monitoring Report (DMR) forms available on
the Stormwater Permitting Unit's website above. DMR forms must be delivered to the Division no later than
30 days from the date the facility receives the sampling results from the laboratory. Also note that existing
permittees do not need to submit a renewal request prior to expiration unless directed by the Division.
Your coverage under the General Permit is transferable only through the specific action of DWQ. This permit
does not affect the legal requirements to obtain other permits which may be required by DENR, nor does it
relieve the permittee from responsibility for compliance with any other applicable federal, state, or local law,
rule, standard, ordinance, order, judgment, or decree. If you have any questions regarding this permit

I am returning the partial copy of your Integrated Spill and Stormwater Plan transmitted to
us by your letter of October 8, 2003. I direct your attention to footnote 5, Part II page 5 of
7 of your general permit, NCG030000. The elements of.a solvent management plan are
identified there as a) a list of the total toxic organic compounds used, b) the method of
disposal, and c) the procedures for assuring that toxic organics do not routinely get into the
stormwater. It is not clear that your Integrated Spill and Stormwater Plan contains these
elements, and so, it does not meet the permit requirements of a solvent management plan.
Footnote 5 provides that a permit holder may request a waiver of TTO testing based on the
development of a solvent management plan. For administrative reasons, the DWQ
generally considers that signing the TTO statement on the back of the DMR is that request,
that the request is granted by the permit itself, and that another approval by the DWQ is
not required. Additionally, the DWQ does not routinely require submittal of an industry's
solvent management plan. In your case, since your initial efforts did not meet permit
requirements, please send me a copy of your revised and compliant solvent management
plan if you still propose to take advantage of the TTO testing waiver.

Mr. Steve Grindstaff of the McDowell County Health Department, requested that I send
you information on Crane Resistoflex's permit status, in order to expedite building permits. Crane -
Resistoflex currently has two (2) active permits with the Division of Water Quality. The first is
NPDES Wastewater Permit Number NC0039934. This permit authorizes the plant to discharge
treated wastewater, from their wastewater treatment plant to a tributary of the Catawba River.
This permit also allows the plant -to discharge non -contact cooling water, used in the plant's
processes, to the Catawba River at two (2) separate points or outfalls. The second is the NPDES
Stormwater Permit Number NCG030499. This permit authorizes the plant to discharge
stormwater from the plant property that has been adequately treated and managed in accordance
with their permit. The plant was last inspected on March 7, 2002 and was determined to be in
compliance with its permit conditions.
Finally, the Division has no issue with the Plant adding onto its existing floor drain system,
as long as the system continues to be operated in the manner it is, which precludes the discharge
of any material other than non -contact cooling water.

Your facility is currently covered for stormwater discharge under General Permit, This permit expires on August
31, 2001 The Division staff is currently in the process of rewriting this permit and is scheduled to have the permit
reissued by late'summer of 2002. Once the permit is reissued, your facility would be eligible for continued coverage
under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you in
advance that your permit will he expiring. Enclosed you will find a General Permit Coverage Renewal
Application Form. The application must be completed and returned by March 4, 2002 in order to assure continued
coverage under the general permit.
Failure to request renewal within this time period may result in a civil assessment of at least $250.00. Larger
penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from your facility
without coverage under a valid Stormwater NPDES permit would constitute a violation of NCGS 143-215.1 and
could result in assessments of civil penalties of up to $10,000 per day.
Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial
facilities in any of the 1 1 categories of "storm water discharges associated with industrial activity," (except
construction activities). If you feel your facility can certify a condition of "no exposure". i.e. the facilty industrial
materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion. For additional

If the subject stormwater discharge to waters of the state has been terminated, please, complete the enclosed

CONTINGENCY PLAN & EMERGENCY PROCEDURES
1.0 PURPOSE
The purpose of this Contingency Plan & Emergency Procedures is to provide detailed plans and
procedures that are to be activated during any emergency situation. These may include a fire, a spill of
hazardous or non -hazardous materials, a medical emergency or injury, any weather related incident, or
any other emergency situation that may require some type of response and intervention from plant
response teams or outside local or state resources. The requirements of OSHA 40 CFR 265.50-56
Subpart D and North Carolina RCRA requirements related to hazardous waste emergency response
specific to our facility are also covered.
2.0 SCOPE
This procedure covers all employees, temporary employees, supplies/vendors, and outside contractors
that may be on company property at the time of an emergency incident, and may be required to be
evacuated to an outside location or instructed to move to other locations depending on the situation.
This plan should be used as a guide to insure the safety of our employees and visitors at all times during
an emergency and remembering that "LIFE SAFETY" is our primary goal.

Crane Resistoflex provides pipe fabrication and plastics processing for the chemical
industry. The plant is approximately 250,000 square feet, including office. Primary
materials on -site include iron castings and steel pipe. There are also large quantities of

3.2.4 Emergency Duties and Authority to Commit Resources:
The Emergency Coordinator, while at work or on call will have the responsibility for -
managing all emergency response procedures. He shall remain familiar with all aspects
of the contingency plan, operations and activities of the facility, location and

alarms, make proper notifications, evacuate or isolate in place as necessary, determine
possible imminent or actual immediate danger to employee health and safety, and
environmental damage. The Emergency Coordinator will determine if it is necessary to
shut off power, gas, or water to the entire plant or just isolated areas. In the event outside
resources are called in to assist and upon their arrival, that command and control of plant
functions and resources will be transferred to the primary responding agency command.
The Emergency Coordinator will remain available as a resource for any plant equipment
needed, communication with other plant personnel, and information.
Persons having the authority to sound the emergency evacuation alarm are as follows;

3.3 Implementation of Contingency Plan:
The contingency plan will be implemented in the event of an incident that may threaten human
health or the environment. These may include a fire, a spill of hazardous or non -hazardous
materials, a medical emergency or injury, any weather related incident, or any other emergency
situation that may require some type of response and intervention from plant response teams or
outside local or state resources.
3.4 Emergency Response Procedures:
3.4.1 Notification:
Any employee may begin the notification process for any type of emergency. The
employee should notify his Cell Leader or any other PC that can be found quickly. In
the event no PC is available in the immediate area, pick-up any phone in the plant and
dial "0" for the switchboard operator. Advise the operator of the name of the caller,
nature of problem (fire, spill, injury, etc.), location in plant, and if PC or Emergency
Coordinator has been notified. Operator will notify Emergency Coordinator to verify
emergency and if outside resources are needed. If confirmed, operator will notify
"91 1" (McDowell Emergency Operation Center) of the emergency, answer any
questions the dispatcher has and await their instructions. Emergency Coordinator will
decide if evacuation is required and if so notify Switchboard Operator to activate alarm
system and begin emergency operations activating this plan. Even if no outside agency
response is needed, some notifications may be required to be made to local and state
officials depending on the emergency. If the spill has reached or is likely to reach the
sewer system or the drainage system that empties into the Catawba River, the
Emergency Coordinator shall notify the following agencies:

Any request for outside assistance will automatically activate a notification process thru
the McDowell County Emergency Management Agency of any local or state agency
requiring notification.
3.4.2 Incident Control & Response:
Any control or containment decision must be made based on several factors. The
amount of materials spilled, the danger involved in the materials spilled, location in the
facility, materials involved in fire and its quantity, Bomb Threat, Injury or Medical
Emergency just to name a few. The decision whether to evacuate all or part of the plant
must also be made by the Emergency Coordinator. If the incident happens on the
second shift or on the weekend when few or no trained personnel are at the plant, the
employee should notify the night watchman and he will notify plant management and/or
an Emergency Coordinator, or if needed the employee may notify the "91 1" dispatch
center.
3.4.2.1 Spills:
In the event of a spill or release of hazardous or non -hazardous materials, only
properly trained personnel should respond to the incident location. After
beginning the notification process, then using all necessary personnel protective
equipment needed for employee safety (boots, Tyvek coveralls, SCBA, gloves,
and eye protection), cover floor drains in the immediate area to prevent
discharge into any water source, and prepare any fire protection needed.
Containment and clean up shall begin immediately using the absorbent
materials stored in the plant. Correction of the problem causing the spill shall
begin as soon as possible. Employees shall not attempt to contain a spill unless
they have been properly trained in "Hazardous Materials Response
Procedures", and should remain in a safe area as the emergency dictates.
3.4.2.2 Fire:
Fire within the building that is small and contained in trashcans or confined
areas may be extinguished by employees provided there is no immediate danger
of extension or any other hazards exist. All employees are encouraged to
control these type fires but are not required to do so. Evacuation to a safe area
should begin immediately and the notification process began. All employees
should be aware of the special fire hazards that do exist in this facility.
Naphtha, a highly flammable liquid with a very low flash point and ignition
temperature, Liquid Paints & Thinners, that are flammable depending on type
and what is mixed with them, Methyl Ethel Keytone (MEK), a highly
flammable solvent, and Natural and Propane Gas, piped throughout the building
are just a few. Extreme caution should be exercised when handling and
transporting these and other materials within the facility. In the event of a
major fire, begin notification, and evacuate the building. Release all fire
fighting responsibilities to the local fire department.
3.4.2.3 Injury or Medical Emergency:
Every employee involved in an accident that results in any type of injury, or
anyone needing medical attention would immediately receive the appropriate
care from the in-house First Aid Team. if a determination is made that the

patient needs treatment from a Medical Doctor and/or Emergency Room
Treatment, they will be transported to the Plant Physician's Office or the
Emergency Room which ever is appropriate. An in-house First Aid Team
Member may transport any non -emergency patients as needed, or if assistance
in transport is needed the McDowell County Emergency Medical Service will
be called via "91 1 ". In either incident the receiving agency will be notified
about the incoming patient, type & cause of injury, any in-house treatment
given, and any history available. If the patient is contaminated with some type
of material from our facility, MSDS information will be made available to
anyone involved in the response. If transport to any higher level care facility is
required, the information from the MSDS will follow along with the patient as
well as being sent ahead via fax or telephone, allowing the doctors office or
hospital to prepare an isolated area if needed.
3.4.2.4 Confined Space Rescue:
Due to the equipment needed and training requirements Confined Space Rescue
will not handled in-house. An agreement with the McDowell County Rescue
Squad has released any response to a confined space incident to their control
and disposition. In-house personnel will support this outside resource as
needed.
3.4.2.5 Weather:
Weather related decisions will be made based on the current weather
conditions, latest known weather information, number of employees involved,
immediate threat to employee safety, and the facility's structural stability.
Every attempt will be made to isolate employees in-house and allow no one
outside or to leave in a personal vehicle during severe weather.
3.4.2.6 Bomb Threats:
Bomb Threats received by the Switchboard Operator will be turned over to the
McDowell County Sheriffs Department. Notification of the Emergency
Coordinator and evacuation procedures will begin as soon as possible. No
plant personnel should attempt any type of search until the Sheriffs Office is on
site. Every effort to assist the Sheriffs Office will be made, however no
employee will be asked to accompany the officer in a search pattern.
Participation will be strictly on a volunteer basis.
3.4.2.7 Biological Threat:
Biological Threats will be handled as individual responses based on the
circumstances surrounding the incident. The McDowell County Sheriff Office
will be the lead agency, responsible for testing, verification, containment, and
removal. The North Carolina Regional Response Team will assist in the
disposition of the incident. No in-house responders should be exposed to the
threat. Local emergency resources will respond for decontamination purposes
only. Any employee exposed to the threat should follow the CDC Guidelines
listed in Section 6.3 of the Exhibits for initial response guidelines A, B, or C.
The level of exposure will guide that individual as to the response needed.
3.4.3 . Follow up:
After any incident, care must be taken to insure all emergency equipment has been
restored to a state of readiness or replaced as needed. Any waste materials generated
during the incident must be disposed of properly and in a timely manner. The cause of

the incident shall be investigated and determined by the Emergency Coordinator, and
the necessary steps put in place to insure the incident does not repeat itself. A complete
report shall be made to management, detailing dates, time, location, materials involved,
cause. injuries, damage to facility and environment, and steps taken to insure no
repeated incidents.
3.5 Emergency Equipment

3.5.2 Location:
The location of various emergency response equipment is as follows:

1) Four Fire Hydrants on each corner of the facility and one for supplying sprinkler
risers in the front yard.
2) Four Hose Cabinets located one at each corner hydrant.
3) Six Fire Extinguishers at critical points of storage.
Inside:
1) Stand Pipes located on columns throughout plant
2) Fire Extinguishers located throughout the plant and no more then 50 ft. apart in any
direction.
3) Emergency Stretchers located outside MIS doors in safety hallway.

6) Radios are issued to all key personnel. Production Coordinators, Switchboard
Operator, Engineering, Night Watchman, and the Lean Production Manager all
maintain constant contact during operating hours.
7) Tools and supplies are available from the Maintenance Department.
8) Sprinklers cover most of the pipe shop and the west end of the building.
9) Eye Wash Stations are located at all Paint Booths, First Aid Room, Maintenance,
Test Lab, Salt Pots, and Vertical Extrusion Tower..

3.5.3 Capabilities:
In-house capabilities of plant personnel to respond to any kind of emergency are diverse
in nature and critical to being successful. These individuals are well trained employees
that include, Fire Department personnel, State certified Haz-Mat responders trained to
the Awareness Level, BLS First Aid at the EMT Level, First Responders with CPR
Training and AED Qualifications_ Also, we have some Test Lab capabilities. The
equipment available within this facility is plentiful, located at critical points within the
plant, is well maintained, and tested annually or as needed. For a complete inventory
and location see sections 3.5.1 and 3.5.2 in this Contingency Plan,
3.5.4 Equipment available from other resources:
The resources available from outside agencies in McDowell County alone are plentiful
and readily available. The local Fire Department has a Mutual Aid Agreement with
Nine (9) other county departments for equipment and manpower. The county EMS
operates three (3) Advanced Life Support Ambulances 24/7. In the event an employee
should need to be transported by air ambulance, McDowell County is serviced by five
(5) Level 1 Hospital Trauma Centers with flight capabilities. The Rescue Squad
operates a manned facility from 7:00 am to 5:00 pm daily with volunteers covering any
other time. Both EMS and Rescue have Mutual Aid Agreements with surrounding
counties for addition resources as needed. The local Emergency Management
maintains an inventory of absorbent materials, stored at the nearby fire station, loaded
on a response unit and available anytime. Emergency Management also has access to
many State Agencies by request. Even though the response may be delayed, those
resources are numerous and maintain deep inventories of materials and manpower is
plentiful.
3.6 Coordination Agreements
3.6.1 Law Enforcement "91 1"
The McDowell County Sheriffs Office has been provided a copy of this plan and has
agreed to assist Resistoflex as needed. Some Sheriffs Office responsibilities might
include, Bomb Threats, Evacuation, Assault and/or Dispute, and Crowd Control.
3.6.2 Fire Department `=91 P"
The P.G. Volunteer Fire Department, Inc. has been provided a copy of this plan and has
agreed to assist Resistoflex as needed. The local Fire Department will be notified of
any and all fires occurring in this facility. All major fires will be released to the
containment and extinguishment of the Fire Department Officer in charge. Some Fire
Department responsibilities might include, testing Water Supplies, Sprinkler Systems,
and hose. The Fire Department would also assist in Spill Containment. Medical
Emergencies, Helicopter Landing Zones, and any other incident that might require large
amounts of manpower and equipment.

3.6.3 Hospital (828) 659-5000
The McDowell County Hospital has been provided a copy of this plan and has agreed to
assist Resistoflex as needed. The Hospital will be used for Medical Emergencies,
Patient Care, and as a Training Resource.
3.6.4 Rescue Squad "91 l"
The McDowell County Rescue Squad has been provided a copy of this plan and has
agreed to assist Resistoflex as needed. The Squad is contracted to provide Confined
Space Rescue services for this facility. Other possible responsibilities might include
extrication from a machine, High Level Rescue, Swift Water Rescue, Collapse, and as a
back up for EMS Ambulance Transportation.
3.6.5 Emergency Medical Services "91 1"
The McDowell County EMS has been provided a copy of this plan and has agreed to
assist Resistoflex as needed. The EMS will provide ALS patient care at the Paramedic
Level and Patient Transport to where ever necessary. Other responsibilities might
include, Training Resources, and First Aid Supplies for the in-house team.
3.6.6 Emergency Management (828)652-3982
The McDowell County Emergency Management has been provided a copy of this plan
and has agreed to assist Resistoflex as needed. Emergency Management's role in our
plan is primarily a local contact for State Resources. Other responsibilities might
include, Training Resource, Changing Regulations Information Resource, Spill Control

3.7 Evacuation Plan:
3.7.1 Evacuation Guidelines:

These guidelines outline procedures to be followed to insure prompt action in
the event of a Spill, Fire, Bomb Threat, Weather or other incident that would
require the evacuation of this facility, Evacuation routes listed in section 3.7.4
should be reviewed periodically with all employees. Actual Drills should be
held at least semi-annual. All employees transferred to another department
should be retrained. All new employees should be made aware of evacuation
routes on their first day. Employees should be aware of evacuation routes if
they happen to be in another part of the building during an emergency. Cell
Leaders have been designated as evacuation wardens. The Cell Leader will
insure all employees that report to them are aware of the emergency and has
started to the Pavilion Shed for accountability purposes. The Cell Leader will
turn off hydraulic units and any process that cannot be left unattended until the
all -clear signal is given and all employees are allowed to return to the facility.
3.7.2 When to evacuate:
The decision to evacuate either the entire facility or just a certain section will be
made based on the immediate threat to our employee's health & safety. A spill
in the Mixing Room may require employees to move to the other end of the
building while the spill is cleaned up. However, a major fire or Bomb Threat
would require a plant wide evacuation of all employees.
3.7.3 Emergency In -House Response Team (Reference 3.6.7):
In the event an evacuation is sounded, the Emergency Coordinator, persons
trained in fire response, spill containment, and maintenance personnel are to
meet at the front employee entrance door. The Emergency Coordinator will
account for the employees present and notify Personnel Control at the Pavilion
Shed. This team will then proceed to address what ever the emergency may
involve. First Aid Personnel should, if possible, retrieve first Aid Cart and
proceed to the Pavilion and await further instructions.
3.7.4 Signals to evacuate:
A continuous "yeow" horn signal from the fire alarm system will indicate that.
all personnel should evacuate the plant and assemble at the employee picnic
shed, which is located approximately 100 yards north of the plant lobby
entrance. Persons having the authority to sound the emergency evacuation
alarm are as follows;

Weather related scenarios shall be included as well as Spill & Control Incidents
as a part of annual training. A Log of the Evacuation Drills (Exhibit 6.5) will
be maintained by Human Resources.
Monthly testing of alarms shall be conducted on the following schedule, which
does not include evacuation or any action requirements. This test is to insure
that all plant personnel can hear the alarm no matter where they may be in the
facility, knows what the alarm means, knows the evacuation routes available to
him/her, and knows where to meet after the alarm sounds. The PC's shall be
responsible to verify with each employee his/her knowledge of these
procedures.

3.7.6 Primary & Secondary Evacuation Routes
• All office personnel will evacuate the building through one of the three
front doors to the office area.
• Maintenance, Tool Room, and R&D Test Lab departments: Primary route
is through the walk-through door on the northeast corner of the plant near
the maintenance area. Secondary route is the employee entrance next to the
canteen.
• Small elbow/tee, receiving inspection, and raw material stockroom:
Primary route is through the walk-through door at the west end of the plant
in the raw material stockroom. Secondary route is through the walk-
through door near the salt pot area.
• Paste Extrusion, Mixing Room, Polyflon, and Isostatic: Primary route is
through the walk-through door near the Isostatic Pipe Liner Molding area at
the west end of the plant. Secondary route is through the walk-through
door at the rear of the plant near the pipe liner sintering ovens.
• Preform area, horizontal and vertical extruder: Primary route will be
through the walk-through door located near the pipe liner sintering ovens.
Secondary route will be the walk-through door located near the Isostatic
pipe liner molding area at the west end of the plant.
• Shipping department: Primary route will be through the walk-through door
next to the shipping office. Secondary route will be through the walk-
through door at the back of the pipe shop in the rear of the building.
• Pipe shop: Primary route will be through the walk-through door at the rear
of the pipe shop. Secondary route will be the walk-through door next to the
shipping office.
• Salt -pot departments: Primary route will be through the walk-through door
near the salt pot area & thru Test Lab to outside. Secondary route will be
through the door in raw material stockroom.
• Chemical Hose: Primary route through walk in door on north wall.
Secondary route will be back thru door into salt pot area and out door in
raw material stockroom.
• Pipe Swaging & Thermoplastic Extrusion. Primary route will walk-
through door at front of building, next to large roll up door, to outside.

5.1 Job Assignments:
Specific assignments for Response Team members are as documented below.
Fire Chief
I . Insure that all In House Response team members are furnished with the current
Contingency plan and are familiar with their job responsibilities.
2. Insure that the Sprinkler Control Valve operator is thoroughly informed and trained
concerning the operation of the sprinkler control valve and dispatched in the event of a
fire.
3. Insure that the Electrician, Plumber and gas operators are trained and dispatched in case
of a fire.
4. Assign In House Response team members to positions a required for the particular
circumstances at the time of the fire and insure that fans, equipment, or machinery
involved are shut off as required.
5. If the situation warrants, dispatch a runner to evacuate employees from the area.
6. In conjunction with the Emergency Coordinator, conduct the appropriate drills as
spelled out in the Contingency Plan.
Control Valve Operator
1. Upon notification of the location of a fire, proceed immediately to the Sprinkler -Control
Valve for the affected area and try the valve to make sure it si fully open.
2. Remain at Sprinkler Control Valve to make sure if remains open until instructed to
close it by the Fire Chief.
3. Stand by the Sprinkler Control Valve after it has been closed and be ready to re -open it
promptly if instructed to do so by the Fire Chief. Constant radio contact with the Fire
Chief shall be maintained.
4. After all clear by the Fire Chief, check valve for fully open position, seal and make flow
drain test to insure protection is restored.
Electrician
1. Secure all circuits, machines, panels, fans, etc. as directed by the Fire Chief.
2. Stand by to restore power as directed by the Fire Chief.
Plumber
1. Secure all air and water circuits as directed by the Fire Chief.
2. Stand by to restore air and water service as directed by the Fire Chief.

Secure the main Gas Shut Off Valve and stand by for instructions from the Fire Chief to
shut off valve.

Personnel Control
1. When evacuation alarm sounds, immediately proceed to the Pavilion for assembly.
Personnel control will assemble everyone in the Pavilion by the appropriate supervisor
and account for each employee.
2. Personnel Control will bring with them a bullhorn, two-way radio, visitor's book and a

3. Report final headcount to the Emergency Coordinator. Once all clear is received from

First Aid
I. When the evacuation alarm is sounded, the First Aid personnel should report to the
Pavilion to be accounted for. The First Aid personnel should bring with them, if
possible, the First Aid Cart and AED.
2. After being accounted for, proceed to the affected location to administer First Aid as
directed by the Emergency Coordinator.

Dear Mr. Daggett:
On May 1, 2013, a compliance sampling inspection was conducted at the Crane
Resistoflex facility. The facility was found to be non -compliant with permit NCG500616.
You were present during a portion of the inspection, as was Jerry Freeman and Manny
Mojica with Crane Resistoflex.
There was some confusion during the inspection regarding the permits Crane Resistoflex
holds. Crane Resistoflex currently holds two Division of Water Quality (DWQ) permits; a
Stormwater General Permit (NCG030499) and a Wastewater General Permit
(NCG500616). Attached is a letter to DWQ dated June 26, 2003 from Todd Poteat with
Crane Resistoflex rescinding an old Wastewater Individual Permit (NC039934). My
inspection was for the Wastewater General permit NCG500616, which is associated with
non -contact cooling waters etc. However, because of the permit confusion we did
discuss the Stormwater General Permit at length as well. Therefore there are important
comments in the inspection reported related to that permit which needs your attention.
Also attached is letter from DWQ to Crane Resistoflex regarding Total Toxic Organics
(TTO) sampling. Total Toxic Organic sampling is required for the Stormwater General
Permit. This parameter is not currently being sampled; you will need to immediately
include this in your stormwater sampling.
Regarding the Wastewater General Permit there are several items that need to be
addressed for compliance. It is important that you review the attached Water Compliance
Inspection Report for findings and required actions. Items requiring immediate attention
include initiating sampling of your wastewater discharges according to the permit for pH,
temperature and chlorine, as well as upstream and downstream sampling. Chlorine levels
during the inspection were elevated and a concern for aquatic organisms.

You mentioned during the inspection that you may hire a consultant to assist you with
compliance, if you do hire someone please have them contact me. You are required to
report back to me in writing by July 31, 2013 the actions you plan to undertake for
compliance. In your response you need to address how you plan to obtain and maintain
compliance with your wastewater permit. Also include in your response all wastewater
sampling results obtained from the date of this letter through July. It is requested that you
sample your effluent and receiving stream on several occasions (weekly is suggested, at
a minimum monthly) to assist you in determining if your actions are achieving compliance
with your wastewater permit.
Please refer to the enclosed inspection report for inspection findings and actions Crane
Resistoflex will need to take. If you or your staff have any questions, please call me at

There appears to be confusion between the stormwater permit (NCG030499) requirements and the
wastewater permit (NCG500616) requirements.
STORMWATER PERMIT:
Sampling results presented to the inspector from Pace Labs were for the Stormwater Permit. Although this
inspection was not a stormwater inspection, I will offer comments regarding the stormwater permit
compliance issues in order to assist the facility.
1. Contact Tim Fox with Division of Water Quality (DWQ) at 828-296-4500 for follow up compliance
assistance.
2. The stormwater samples are not being obtained correctly. Please review Part it Section B of Stormwater
Permit for sampling requirements. The frequency should be semi-annual unless in a tiered response mode.
The stormwater samples must be obtained during a storm event and collected within the first 30 minutes of
the stormwater discharge. This sampling requirement is important because the first flush of stormwater is a
representative sample of the pollution load since it carries the most pollutants to the stream. Sample results
reviewed indicated one sample set was obtained without any rainfall- Samples are obtained by a consultant
in Asheville, so it is unlikely the 30 minute requirement can be met. Personnel at the facility should be
gathering the samples and notifying consultant to come and pick up the samples for delivery to the lab. The
Consultant can train Crane Resistoflex personnel in proper sampling and handling techniques. See Table 1
in stormwater permit for required parameters.
3. A Stormwater Pollution Prevention Plan has not been developed and implemented. Review Part 1,
Section A of Stormwater Permit for Plan requirements. Pieces of the Stormwater Pollution Prevention Plan
may be at the facility (i.e. other program requirements; Contingency Plan and Emergency Procedures) but
have not been specifically developed and properly implemented for compliance with the Stormwater Permit.
4. Illicit discharges need to be evaluated and removed. This is an old industrial building, therefore the
inspector discussed illicit discharges with staff.
WASTEWATER PERMIT:
The facility was not compliant with the. Wastewater Permit. They were unaware of the NCG500616 permit
requirements (General Permit No. NCG500000 Non -Contact Cooling Water, Cooling Tower and Boiler
Blowdown, Condensate, etc). Required actions listed below are to assist facility with their General
Wastewater Permit compliance.
1. Contact the local municipality to determine if discharging wastewater into the sewer system is a feasible
alternative. Contact Larry Carver with the City of Marion at 652-8843. If you chose to tie onto the City sewer
system contact me at 296-4500 to assist you with rescinding your NCG500616 General Wastewater permit.
2. You have to begin sampling your Wastewater Discharge (small boiler in lab and Ram tower extruder).
Two wastewater discharges were discussed during the inspection. One discharge was sampled during the
inspection (Ram Tower). Results indicated non-compliance with the permit (Part I A(1)).
3. If Crane Resistoflex wishes to retain the Wastewater permit, the chlorine residual in the wastewater
discharge will have to be reduced. Chlorine is higly toxic to aquatic organisms especially at the levels
indicated during the inspection. There are options to reduce/remove chlorine from your wastewater
discharge; if you have the gradient it is possible to aerate the wastewater discharge enough to volitalize the
chlorine. There are also dechlorination devices that can be installed.

Comment: There is a fence around the property, however there is an area by the
stream where the fence is hanging loose because of an eroded streambank.
Restoration of the streambank should be considered otherwise the erosion will continue.

Comment: The Inspector visited two of the three outfalls. Three outfalls are reported
on the site map for this facility. Outfall #4 was evaluated during the inspection. This
outfall carries wastewater associated with the NCG500616 permit. During the
inspection this was discharging wastewater from the Ram tower. The facility has a small
boiler in tab that also has a wastewater discharge, it is unclear where this discharges.
There was an area with several pipes at outfall #4, all of the pipes origins were
unknown. This has to be determined by facility.
The other two outfalls are believed to be associated with the stormwater permit
NCG030499.
Note: Outfall #4 also carries stormwater during a rain event, therefore outfall #4 should
never be sampled for wastewater associated with permit NCG500616 during a rain
event.

with sample results by Pace tabs but these results are from the samples obtained for

Comment. Analysis of wastewater effluent (outfall #4, Ram Tower) was performed
during the inspection. Results indicate that temperature may not be an issue in the
receiving stream, however chlorine is likely an issue. Chlorine levels obatined during the
inspection were 1.78 mg/l or 1780 ug/l. The DWQ considers values less than 50 ug/I
compliant with the permit.
Chlorine is toxic to aquatic organisms.
Crane Resistoflex needs to evaluate chlorine levels in their effluent as well as upstream
and downstream where their wastewater discharge enters the receiving stream
(Unnamed Tributary to Catawba). This needs to be done at an increased rate to
determine if the actions taker! by Crane Resistoflex are effective for compliance with
permit limits. Contact inspector for proper upstream and downstream sampling
locations.

The purpose of this letter is to request that Crane Resistoflex Individual Permit No. NC0039934,
be rescinded. Out fall 001 is now piped into the new Pleasant Garden Sewer Extension and the
Package Treatment Plant has been removed from service. I am also requesting that our non -
contact cooling water discharges 002, 003, and 004 (004 has been in existence, but was not used
as non contact cooling. It will be used as such with our new expansion) be changed to a General
Wastewater Discharge Permit, along with our already existing General Stormwater Permit No.
NCG030499. Discharge 002, 003, and 004 are pre-existing discharges presently in use at this
facility. Attached is a completed Notice of Intent.
