This plant manufacturers soldering wire. Mr. Luckard walked the grounds of the plant with the inspector. A few areas were identified that will need attention prior to issuance of a No Exposure Exclusion for this facility. 1. There were a couple of boneyard areas where metal waste and metal that still may be of use to the plant are being stored outside. Some storage is on concrete as will be required while others metals are being stored directly on the ground. Neither are undercover. 2. Barrels are being stored directly on the ground and not undercover. Items 1 and 2 will both be required to be stored either in doors or under cover and on a solid surface pad such as concrete or asphalt. Cover may be in the form of roof type structure, building, shed, trailer, tarps, etc... Please address these items before applying for No Exposure Exclusion to the General Stormwater Permit NCG030200. VAN DER VAART Secretary This letter is in response to your acknowledgement and proper reporting of several exceedances of stormwater monitoring results for copper and zinc at the Lowery Street facility. This facility is currently covered under an NC General Stormiwater Permit as required by the Federal Environmental Protection Agency. You have been conducted analytical monitoring as the permit requires. You have made significant attempts to identify sources of contamination that may be causing exceedance. You've had Peter DeVries of GeoScience & Technology test samples of the rainwater directly from the downspouts and sampled where rainwater has made no contact with any surface. Results showed significant concentrations of zinc and copper in both samples. Since the monitoring results did not reveal significant variances following implementation of source control measures by the facility, we are granting regulatory relief in the form of a collapsed Tier 2 - Tier 3 response. Upon receipt of this letter, Worthington Cylinders at 1690 Lowery Street may resume the permit specified semi-annual analytical monitoring for the remainder of the current permit term. Your current permit is set to expire on October 31, 2017. This decision applies only to the copper and zinc benchmarks. (GeoSci) is pleased to submit this document in response to a Compliance Evaluation Inspection Letter dated June 16, 2014 from your office; hereafter referred to as the "Letter". The facility is covered under North Carolina General Stormwater Permit #NCG030000, Certificate of Coverage #NC(3030200. This response will address the issues noted as a result of a compliance evaluation inspection by you on May 28, 2014. The responses below correspond to the eleven required actions listed in Part 4 of the Letter. 1) Clean up oil leak at the loading dock in the employee parking lot. The oil `spots' on the asphalt pavement noted in Picture #1 in your Letter were cleaned on September 11. 2014. Worthington purchased product to put down in the event of fresh oil drips and the product will be kept it at both ends of the building. One of those locations is in proximity to the door leading out to the drainage box (see Item 3, below). . 2) Prevent and eliminate erosion at `French Drains' noted in photographs attached to the Letter. As of September 11, 2014, the `French drain' noted in Picture #3 in your Letter was rerouted by the addition of pipe sufficient to connect to the stormwater drain. This will effectively eliminate erosion at this location. Clean #57 stone has been placed around the outlet of the `French drain' noted in Picture #2 in your Letter to mitigate erosion at that location. 3) Place a spill kit near the drainage box to the sanitary se"ver. Spill absorbing materials have been placed at both ends of the building in proximity to the Ioading docks (see Item #1, above). 4) Update and revise the Stormwater Pollution Prevention flan (SP3) with current information, including potential stormwater pollutants. The SP3 has been revised as of the writing of this document to reflect the current management team responsible for implementation of the SP3. This includes contact information for the facility management team and environmental contacts in case of a spill or incident related to stormwater pollution. ' Currently the. only materials with the potential to impact stormwater are metals contained in dust from the filter `bag' house located on the roof and petroleum drips and spills that may occur in the parking areas and loading docks. The majority of the metals used in the manufacturing of_solder at this facility are in a solid form and are not considered potential stormwater pollutants. Molten metal in the meIt kettles within the building could be potential pollutants, However, these, materials would solidify very quickly once outside of the kettles and the potential for them to be exposed to stormwater is considered very minimal due to the distance from the kettles to the closest point of possible exposure. 5) Evaluate the stormwater sampling point with the drop box at SDQ-3. Stormwater samples are currently collected at the stormwater intake/drop box located on the southeast edge of the employee parking lot at the east end of the facility. Stormwater from the building roof and the loading dock area located at the west end of the facility is conveyed to this intake via a pipe located oil the south side, of the building. Runoff from the employee parking area at the east end of the facility also discharges to this intake via sheetwash from the paved parking Iot. The stormwater discharges from all sources combine at the bottom of the drop box at this intake. Stormwater samples are collected at the bottom of the drop box using a telescoping Mr. Ladonta Clark September 2014 Worthington Cylinders Slormwater Cornpliance Page 2 I pole with an attached sample container after the discharges have combined. Collecting samples from the distal end of the pipe, located at the base of the steep slope on the north bank of Brushy Fork Creek is considered logistically impractical due to the extreme slope and slippery conditions typically encountered during rain events. It is GeoSci's opinion that the current method of collection provides a storrnwater sample that is representative of all of the sources of stormwater runoff from this facility. 6) Create a general location map. A General Site Location Map based on the portion of the "Winston Salem East" USGS quadrangle is attached to this document. An updated Detailed Site Map is also attached to this document. 7) .Document inspection of material handling areas. A monthly site inspection form has been added to the SP3. 8) Update the Stormwater Division's contact information. Environmental Contacts have been updated in the SP3 as of the date of this document. 9) Provide good housekeeping and preventive maintenance training. Good Housekeeping and Preventive Maintenance training will be provided to employees and documented annually. The documentation will be inserted into the SP3. 10) Document the start and end time of the measurable rain event for analytical and qualitative sampling. The sampler/inspector will endeavor to document the start and end time of sampled rain events; however, exact start and end times of rain events at a particular location are not always known. GeoSci employees typically collect stormwater samples for this facility and the time of sample collection is noted in their field logbooks, on the quantitative monitoring report form and on the laboratory Chain of Custody documents. When possible, the approximate time the rainfall began is also noted in the field Iogbook. Furthermore, there is no mandate on either of the North Carolina reporting forms for this information; however, whenever possible, the precipitation event start and end times will be recorded in the field log books. We were also told that if the exceedances in the stormwater above the benchmark levels for Copper and Zinc were determined to be LIle fatiility meta roof, vve may be told tc go back to norma! sampling schedule.
