Industrial Connections & Solutions LLC — Stormwater General Permit NCG030000
Dear Sir or Madam:
Per our NPDES Stormwater Permit NCG030000, Industrial Connections & Solutions LLC collected the
required benchmark samples from our facility outfalls on March 21, 2019. The following documents are
enclosed:
1. Stormwater Discharge Monitoring Report
2 2"d Copy of Discharge Monitoring Report
3. Lab Results from Sampling (Meritech, Inc.)
4. Qualitative Monitoring Reports (SDOs) —Blue Ridge Geological Services, Inc.
In accordance with our NPDES Stormwater Permit, Industrial Connections & Solutions LLC is notifying
the Division of Energy, Mineral, and Land Resources (DEMLR) that the sample results of all outfalls were
within the benchmark values indicated in the permit. We will continue to conduct monthly sampling while
looking for solutions to these benchmark exceedances.
If you have any questions about this report, please contact me at (919) 563-7521.
Sincerely,

' Monthly sampling (instead of semi-annual) must begin with the second consecutive benchmark exceedance for the same parameter at the same outfall.
z For sampling periods with no discharge at any single outfall, you must still submit this discharge monitoring report with a checkmark here.
3The total precipitation must be recorded using data from an on -site rain gauge. Unattended sites may be eligible for a waiver of the rain gauge requirement.
° See General Permit, Section B, Table 1 to identify the especially sensitive receiving water classifications where the more protective benchmark applies.
5 Total Toxic Organics sampling is applicable only for those facilities which perform metal finishing operations, manufacture semiconductors, manufacture
electronic crystals, or manufacture cathode ray tubes. For purposes of this permit the definition of Total Toxic Organics is that definition contained in the EPA
Effluent Guidelines for the facility subject to the requirement to sample (for metal finishing use the definition as found in 40 CFR 433.11; for semiconductor
manufacture use the definition as found in 40 CFR 469.12; for electronic crystal manufacture use the definition as found in 40 CFR 469.22; and for cathode ray
tube manufacture use the definition found in 40 CFR 469.31).

Facilities that incorporate a solvent management plan into the Stormwater Pollution Prevention Plan may so certify, and the requirement for TTO monitoring
may be waived. The solvent management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General
Permit. For those facilities electing to employ the TTO monitoring waiver, the discharger shall sign the following certification statement:
"Based upon my inquiry of the person or persons directly responsible for managing compliance with the permit monitoring requirement for total toxic organics
(TTO), I certify that to the best of my knowledge and belief, no dumping of concentrated toxic organics into the stormwater or areas which are exposed to rainfall or
stormwater runoff has occurred since filing the last discharge monitoring report. I further certify that this facility is implementing the all the provisions of the
solvent management plan included in the Stormwater Pollution Prevention Plan."

Subject: Report of Stormwater Inspection and Sampling - March 2019
]industrial Connections & Solutions 1 ABB
6801 Industrial Drive, Mebane, North Carolina
Dear Ms. Stennis:
As authorized by acceptance of our proposal dated September 2016 and your requests to continue
sampling, Blue Ridge Geologieal Services, Inc. (Blue Ridge) personnel performed additional
environmental services at the subject site. The work was performed in general accordance with the
Stormwater Pollution Prevention Plan (SWPPP) prepared for the subject site as well as the requirements of
NCDEQ NPDES General Permit. On March 21, 2019 Blue Ridge personnel performed a stormwater
inspection and sampling event at the subject site during a measureable rainfall event. Rainfall data was
obtained from a nearby weather station using Weather Underground. According to the website, the area in
the vicinity of the subject site received approximately 0.58 inches of rain during the storm event.
Blue Ridge personnel observed the stormwater discharge at outfalls SDO-1 through SDO-7 and completed
stormwater discharge outfall (SDO) qualitative monitoring report forms for each of the outfalls. The SDO
forms are attached. The pH of the samples was measured in the field using an Oakton pH meter and
recorded on the SDO forms. No environmental issues were observed at the outfalls during this inspection.
Field personnel collected samples from each of the stormwater outfalls, placed the samples into labeled
containers, completed a chain of custody (COC) form, and submitted the samples and COC form to Meritech
laboratory personnel.
This inspection report and the SDO forms should be placed in the SWPP Plan or a file on site and they do not
need to be submitted to the NCDEQ. We will attempt to perform the next stormwater inspection/sampling
event at the site before April 30, 2019. We appreciate the opportunity to continue to assist you on this
project. Please contact the undersigned at if you have any questions regarding this report or if you need
additional assistance.
Sincerely,
Jeffrey L. Gerlock, L.G.

All permits require qualitative monitoring to be performed during a "measwable storm event."
A "measurable storm event' is a storm event that results in an actual discharge from the permitted site
oWfall. The previous measurable storm evertl must have been at least 72 hours prior. The 72-hour storm
interval does not apply if the permitlee is able to docu rnent that a shorter interval is representative for
local storm events during the sampling period and the permirtee obtains &WovW from the local DEMLR
Regional Office.
