I really don't see any benefit from increased monitoring for pH only. Continue monitoring both Outfalls 2 & 3 under Tier
I, Semi-annual sampling for all required parameters. We may need to re-evaluate or make some suggestions at the next
inspection but for now continue in Tier 1.

Gilbarco continues to have low pH issues in Outfall 002 and 003 depending on the rain volume. See attached sampling report for Jan -
June 2017. 1 completed a roof top sampling event of pH in residual rain puddles across the roof and the pH is consistently below 5. In
researching this, there appears to be several potential contributors including the roof fabric (PVC constituency), the copper HVAC coil
condensate discharge, and the uric acid from geese, Based on the attached study both copper material and PVC roofing can cause
low pH levels.
Gilbarco is continuing its best management techniques to minimize potential impacts but does not feel monthly sampling will provide
any additional value for Outfall 002. 1 am requesting that the waiver of monthly monitoring requirement be extended from outfall 003 to
outfall 002. Please feel free to contact me regarding this issue.

As we discussed in our phone conversation NCDEQ will grant you a waiver from the Tier II monthly monitoring
requirements of your Stormwater Permit for the pH parameter at Outfall 003. Unless other required parameters are
exceeded consecutively, you may return to Tier I, bi-annual sampling.

Thank you for taking time to speak with me today,
As discussed, Gilbarco has been completing the outfall sampling required as part of our NPDES permit and have had occasional low
pH --5,5 in our Outfall 003 when sampling occurs during low volume rain events. We have completed a review of potential
contributors. The major source appears to be condensate from our rooftop HVAC systems. Gilbarco continues to implement BMPs to
minimize impacts to stormwater but is limited in its ability to control pH of condensate discharge from HVAC system.
We will continue best management practices but are requesting a waiver of Tier II requirements for this parameter in our outfalls.

industrial compliance inspection of Gilbarco Veeder-Root. Local regulations permit an
inspection of industrial facilities under authority granted by City of Greensboro Code of
Ordinance Chapter 30, Article 12, and Section 7.1 (C-S). 'rbis inspection consisted of two parts:
a review of the facility files/records and an on -site facility inspection.

On December 8, 2016 Glen White and Sue White, inspectors with the North Carolina
Department of Environmental Quality — Department of Energy, Minerals & Land Resources met
with John Burke at the subject facility, located at 7300 W Friendly Avenue, to conduct a
compliance inspection for the facilities Industrial Stormwater Permit,
Permit:
This facility'holds General Stormwater Permit NCG030056 to discharge stormwater from
industrial activity classified as Heat Treating of Metal (SIC 3398), Fabricating of Metal
Products (SIC 3398), Manufacturing of Measuring and Analyzing Instruments (SIC 38)
under the National Pollutant Discharge Elimination, System (NPDES). The permit became
effective November 1, 2012 and expires on October 31, 2017. A copy of the current permit is
required to be maintained with the Stormwater Pollution Prevention Plan (SPPP) documents. A
copy of the current permit was available for review at the time of inspection.
Records/Reports:
This facility was required to develop and maintain a Stormwater Pollution Prevention Plan
(SPPP) in accordance with Part II, Section A of the permit. Inspector reviewed the SPPP and
each of its components. The SPPP has been updated annually as required. Training is conducted
monthly along with other required training and emergency response training. All employees
sign off on training log with pen and ink signatures. Monitoring is conducted according to the
permit. Records are well maintained in a 3-ring binder as permit suggest. No deficiencies noted.
Facility Site Review:
This facility is a metal fabricator that builds a large variety of gas pumps and equipment that is in
use throughout the world. The grounds of the facility appear to be in good conditon and are well
maintained.

The site has a groundwater remediation system in place to filter out groundwater and soil
contamination. The system is managed by a third party environmental group. There are (6) six -
stormwater outfalls and (3) three - detention ponds. The facility has a history of elevated
monitoring results for zinc, copper and low pH. NCDEQ recently allowed Gilbarco to reset the
monitoring to Tier 1, bi-annual monitoring. Zinc, copper and low pH may or may not be a result
of natural processes not associated with this facility.
Inspectors suggest changes to the monitoring process. Suggest adding a concrete v-notch at the
location of sample collection so a clean -catch can be made and add rip -rap on the upstream. side
of sample location to assist with filtering water before capture.
Effluent Receiving Waters:
Effluent from this facility drains to Horsepen Creek, Category 5 — Impaired Waters of the Cape
Fear River Basin.
Self-Monitorinjz Prollram:
Part II, Sections B & D of the permit require this facility to perform qualitative and analytical
monitoring on a semi-annual basis unless a Tiered Response is triggered. Monitoring is
performed as required and reports are well prepared by a third party designated Gilbarco.
This facility was found to be compliant with the requirements of its General Stormwater Permit.

Inspection Summary:
Inspection with John Burke, Sr. Env. Engineer with Gilbarco, was conducted by Glen White and Sue White, inspectors with
NCDEQ out of the Winston-Salem office on 1218116. Records were complete, current and well maintained. History of
elevated zinc, copper and pH found in samples. NCDEQ has allowed the facility to reset monitoring to Tier I, bi-annual
sampling. All monitored out€ails were observed during inspection. Inspectors suggest installing a v-notch with rip -rap on
the upstream side of the v-notch to capture clean -catch samples. This may help with reducing exceedance of monitoring
parameters.
Overall, site was in very good condition and Mr. Burke has done a very good job of maintaining the Stormwater Pollution
Prevention Plan and records.

This letter is a follow-up to the previous letter dated 28 October 2013, In that letter, it was determined that
more frequent monitoring of copper and zinc in Outfalls 001 and 002 would be ineffective. Based on recent
analytical monitoring results, the Division also relieves you of the additional monitoring frequencies required in
the Tiered Response section of this permit for copper and zinc at Outfall 003. You must continue to monitor semi-
annually as required by the permit, but you will not be required to institute monthly monitoring for any future
exceedences of the copper and zinc parameters at Outfalls 001, 002, and/or 003. Tiered response requirements
still apply for all other parameters.
We appreciate your efforts to comply with the stormwater permit and the proactive steps taken to identify and
eliminate potential sources of pollution. No additional response to this letter is required, but please append this
letter to your SPPP to document that your facility has been relieved of monthly monitoring requirements for the
specified parameters. Should you have any questions regarding the NCG030000 permit, please do not hesitate to

Aana Taylor -Smith of the Winston-Salem Regional Office of the NC Division of Energy, Mineral, and Land
Resources (DEMLR) conducted a compliance evaluation inspection at the subject facility on 23 October 2013. Your
assistance and cooperation during the inspection were greatly appreciated. An inspection checklist is attached for
your records and the inspection findings are summarized below.
This facility is located at 7300 West Friendly Avenue in Greensboro, Guilford County, North Carolina. Stormwater
related to industrial activity is discharged from the site to Horsepen Creek, this segment of which is currently
classified as Class WS-III, Nutrient Sensitive Waters, in the Cape Fear River Basin.
Documentation & Monitoring Review
The facility's Stormwater Pollution Prevention Plan (SPPP) contains all of the required elements and is up to date
and fully implemented. Both qualitative and analytical monitoring have been completed as required. The facility
has three Stormwater Discharge Outfalls (SDOs) that discharge Stormwater related to industrial activity. A tiered
response has been instituted for all three SDOs at this time.
Sample results from outfalls 001 and 002 exceeded benchmarks for copper and zinc and those taken at Outfall
003 exceeded the pH range to trigger a tiered response. In all cases, Gilbarco Veeder-Root has complied with
permit requirements to increase sampling, identify potential sources of pollution, and attempt to eliminate them.
As a result, the pH exceedences have been resolved at Outfall 003.
Site Review
The facility is clean and well -maintained. Secondary containment is implemented as required. All three outfalls
were inspected and found to be in good order. No evidence of erosion or instability at the SDOs was observed and
the receiving stream was free of pollution indicators. During the inspection, it was noted that this facility would
only need to make a few adjustments to qualify for No Exposure Certification, which would eliminate the need for

Tiered Response Require ents
Currently, monthly monitoring is required in response to two consecutive exceedences of the benchmark values
or range for any specific parameter at a specific SDO. Gilbarco Veeder-Root has completed monthly monitoring
for all outfalls as well as conducting extensive studies to identify and eliminate potential sources of
contamination. As previously stated, the pH issue at Outfall b03 has been resolved through implementation of
best management practices. Exceedences of copper and zinc benchmarks have continued at Outfalls 001 and 002
despite the facility's proactive attempts to identify and eliminate any source of pollutants, including installation of
a mist eliminator and other measures.
At this time, the Division concurs that more frequent monitoring at this site would be ineffective. The Division
relieves you of the additional monitoring frequencies required in the Tiered Response section of this permit for
the parameters specified. You must continue to monitor semi-annually as required by the permit, but you will not
be required to institute monthly monitoring for any future exceedences of the copper and zinc parameters at
Outfalls 001 and 002, or the pH parameter at Outfall 003. Tiered response requirements still apply for all other
parameters.
Additionally, the Division highly recommends that Gilbarco Veeder-Root consider options for making
modifications to the facility to become eligible for No Exposure Certification. Coverage under the No Exposure
Certification would eliminate the need for analytical monitoring altogether as long as the facility abides by No
Exposure requirements.
We appreciate your efforts to comply with the stormwater permit and the proactive steps taken to identify and
eliminate potential sources of pollution. No additional response to this letter is required, but please append this
letter to your SPPP to document that your facility has been relieved of monthly monitoring requirements for the

The General Permit authorizes discharges of stormwater, and it specifies your obligations for discharge
controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself
with all the changes in the reissued permit. Your facility has six (6) months from the time of receipt of the
permit to update your current SPPP to reflect all new permit requirements.
The first sample period of your permit begins January 1, 2013. Your facility must sample a "measureable
storm event" beginning during the periods beginning January 1 and July 1 of every year (or, if applicable,
report "No Flow," as outlined in Part III, Section E). Also, please note that Tier 3 Actions in Part II of your
permit are triggered by benchmark exceedances on four occasions beginning on the effective date of this
permit and do not count prior exceedances.
The more significant changes in the General Permit since your last COC was issued are noted either in the
Draft Permit Fact Sheet that accompanied the public notice (http_/Zpartal.ncdenr.org/web/wg/ws/su/current-
notices), or in the Response to Comments / Summary of Changes and Technical Bulletin documents that are

Some of the changes include:
Part II:
• Section A: The Stormwater Pollution Prevention Plan (SPPP) section, if applicable, has been updated
to the most current language of our permits. Additional conditions for specific industry sectors have
been added to the SPPP requirements in some cases.
• Sections B, C: Failure to perform analytical stormwater monitoring may result in the Division requiring
that the permittee begin a monthly sampling scheme.
• Sections B, C: A lower TSS benchmark of 50 mg/I for HQW, ORW, PNA and Tr Waters applies to these
more sensitive waters.
• Sections B, C. The monitoring parameter Oil & Grease (O&G) has been replaced by the parameter
Total Petroleum Hydrocarbons (TPH) for vehicle maintenance areas, and in some cases, other
analytical monitoring requirements.
• Sections B, C, D: Inability to sample due to adverse weather must be recorded in the SPPP, or in
separate on -site records if your General Permit does not require an SPPP. Adverse weather is defined
in the "Definitions" section of the permit.
• Sections B, C. The term "Representative Storm Event" has been replaced by "Measurable Storm
Event." A measurable storm event is defined in the permit.
• Section D: If the permittee fails to respond effectively to problems identified by qualitative
monitoring, DWQ may require the permittee to perform corrective action.
Please review Parts III and IV to understand the Standard Conditions of your new NPDES General Permit,
including Compliance and liability, Reporting, Monitoring and Records requirements; Operation and
Maintenance obligations; and Definitions. Please note that all samples analyzed in accordance with the terms
of this permit must be submitted to the Division on Discharge Monitoring Report (DMR) forms available on
the Stormwater Permitting Unit's website above. DMR forms must be delivered to the Division no later than
30 days from the date the facility receives the sampling results from the laboratory. Also note that existing
permittees do not need to submit a renewal request prior to expiration unless directed by the Division.
Your coverage under the General Permit is transferable only through the specific action of DWQ. This permit
does not affect the legal requirements to obtain other permits which may be required by DENR, nor does it
relieve the permittee from responsibility for compliance with any other applicable federal, state, or local law,
rule, standard, ordinance, order, judgment, or decree. If you have any questions regarding this permit

up as to what's coming their way.
That's from an immediate standpoint, but I'd also say yes so we can note it in their NPDES file and keep it in mind during

on August 31, 2002. The Division slaff is currently in the process of rewriting this permit and is scheduled to have
the permit reissued by late summer ol' 2002. Once the permit is reissued, your facility would be eligible for
continued coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the Division of Waler
Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you in
advance that your permit will be expiring. Enclosed you will find a General Permit Coverage Renewal
Application Form. The application must be completed and returned by March 4, 2002 in order to assure continued
coverage under the general permit.
Failure to request renewal within this time period [nay result in a civil assessment of at least $250.00. Larger
penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from your facility
without coverage under a valid storniwaler NPDES permit would constitute a violation of NCGS 143-215.1 and
could result in assessments of eivil penalties of up to $10,000 per clay.
Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial
facilities in any of the I t categories of "storm water discharges associated with industrial activity," (except
construction activities). if you feel your facility can certify a condition of "no exposure", i.e. the facilty industrial
materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion. For additional

In accordance with your application for discharge permit received on September 28, 1992, we are
forwarding herewith the subject certificate of coverage to discharge under the subject state - NPDES general
permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215 .1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated
December 6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual permit
application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division
of Environmental Management. The Division of Environmental Management may require modification or
revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Environmental Management or permits required by the Division of Land Resources, Coastal Area
Management Act or any other Federal or Local governmental permit that may be required.
