On August 15, 2017 Glen White of the North Carolina Department of Environmental Quality —
Department of Energy, Minerals & Land Resources met with you and James Frei (Consulting
Environmental Engineer) of SwSG, LLC at the subject Facility, located at 1266 N Scales Street
in Reidsville, to conduct a compliance inspection for the facilities Industrial Stormwater Permit.
Your assistance with inspection was greatly appreciated.
Permit:
This facility holds General Stormwater Permit NCG030033 to discharge stormwater from
industrial activity classified as Heat Treating of Metal (SIC 3398), Fabricating of Metal
Products (SIC 3398), Manufacturing of Measuring and Analyzing Instruments (SIC 38)
under the National Pollutant Discharge Elimination System (NPDES). The permit became
effective November 1, 2012 and expires on October 31, 2017. If a permit revision is not issued
prior to the expiration date, the facility will continue to operate under the expired permit until a
new permit is issued. A copy of the current permit is required to be maintained with the
Stormwater Pollution Prevention Plan (SPPP) documents. A copy of the current permit was
available for review at the time of inspection.
Records/Reports;
This facility was required to develop and maintain a Stormwater Pollution Prevention Plan
(SPPP) in accordance with Part II, Section A of the permit. Inspector reviewed the SPPP and
each of its components. The SPPP has been updated annually as required. A training workshop
that includes emergency response training is conducted annually by Mr. Frei. All employees
sign off on training log with pen and ink signatures. Monitoring is conducted according to the
permit. No deficiencies noted.

Facility_ Site Review:
This facility manufactures tank trucks and specialty trucks for a range of uses that vary from
carrying drinking water to hauling various types of fuels including jet fuel. The facility has both
private and government contract clients with large multi -year contracts for military equipment.
Vehicles and tanks are built to customer specifications and tested using the government and
customer required methods.
Effluent Receiving Waters:
Effluent from this facility drains to from an unnamed tributary to Wolfe Island Creek, a part of
the Roanoke River Basin.
Self -Monitoring Program:
Part II, Sec#ions B & D of the permit require this facility to perform qualitative and analytical
monitoring on a semi-annual basis unless a Tiered Response is triggered. The last monitoring
samples were taken on August 10, 2017. The plant has undergone significant
renovation/expansion over the last 12 month period. Outfalls have increased to a total of five
(5). Outfall 1 is at the front of the property. Outfall 2 at the rear of the facility is monitored
representative of that outfall. All other outfalls are located at the rear of the facility and adjacent,
to the new fence line. Those outfalls are required to be monitored until R.O.S. (Representative
Outfall Status) can be achieved.
The condition of the facility itself appears to have significantly improved since the last
inspection was conducted in 2015. The -production area and laydown yard have been expanded
and much of the production area is now paved with concrete. New covered production areas
have been installed and there is a new covered area for metal stock. General housekeeping at the
site has improved significantly. All storage appears to be neatly stored in its respective place.
The SPPP and siteplan will need to be revised to include all site renovations, additions, and
current records once the revised/new general permit has been issued. If you have further

Inspection Summary:
Many additions and renovations have'occured over the past year at the facility. The majority of the operations area has been
paved with concrete and the laydown yard has been expanded significantly. The overall appearance of the site has improved
and housekeeping appears to have improved as well. Everything appears to be neat and in in place. There are new covered
production areas and there is a new covered area for metal stock. Most containment is under roof and all appear to have
secondary containment. Siteplan and SPPP will require updating as construction nears completion and after the new
revision of the general permit has been issued. The old permit expires on October 31, 2017. If the new permit has not been
issued by that date, the facility will continue to operate in compliance under the expired permit until a new revised permit
has been issued.

(DEMLR) conducted a compliance evaluation inspection at the subject facility on November 04, 2015. Mr.
James Frei of SwSG, LLC ad Mr. Mike Leffew of Isometrics Engineering, assisted the inspector with review of
the SP3 (Stormwater Pollution Prevention Plan) documents and with site review. The facility was found to
be compliant with its Stormwater permit requirements. An inspection report is attached for your records
and the inspection findings are summarized below.
1. Permit
This facility holds General Stormwater Permit No. NCG030033 to discharge Stormwater from activities
associated with Heat Treating of Metal Products and Metal Fabrication under the National Pollutant Discharge
Elimination System (NPDES). The permit became effective November 1, 2012 and expires October 31, 2017.
Your permit renewal package must be received in the Central Office in Ra leigh no later than 180 days prior to
the expiration date of the permit.
2. Records/Reports
Part II, Section A of the permit requires the development of a Stormwater Pollution Prevention Plan (SPPP).
The plan must include all 9 sections of Part II, Section A requirements of the permit. This site is required to
complete analytical and qualitative monitoring twice yearly as defined in Part ll, Section C of the permit. The
plan must be updated annually. The SPPP had been updated and was current at the time of inspection.

Dear Sirs;
Thank you for the opportunity to respond to your evaluation of our facilities conducted on
27 September of this year. Now, to address your concerns:
1. The used oil tank will be moved to the old hazardous waste shed. Here the tank will have
secondary containment and will be under a roof. We will also secure the outlet valve with a locking
mechanism.
2. The two smaller diesel tanks are being emptied and will no longer be used for storage.
3. We, in conjunction with our storm water consultant, Jim Frei of Stormwater Services Group, LLC
8916 Oregon Inlet Ct., Raleigh, North Carolina 27603, are planning to name a new storm water
pollution prevention team and schedule a training session once the new permits are issued.
Items 1 and 2 above will be completed by the end of the month, 31 October 2012.

Mike, thanks for the quick response. Yes, the permit does allow certain other discharges but part of what we do is to
identify and eliminate illicit wastewater discharges. With this information, we have shown that this process is not a point
source wastewater discharge to waters of the state. I've run this by my boss and he agrees.
However, I must convey to you to ensure the process doesn't change. Let's keep the water uncontaminated and keep
the operation where it is on the site. Any changes to the process may merit a reevaluation in the future. Call if you have
any questions or concerns and thanks for all your work on this.

Ron,
We had a sample of the water analyzed. SafetyKleen took a sample directly from the outlet
port of one of the tanks after testing. This was sent to TestAmerica Laboratories, inc. in Pittsburg. Results are attached
I discussed this with our consultant, Jim Frei of Stormwater Services Group, Raleigh; you met him when you were here
last. He said that our permit allows discharge of potable water and does not state that all process waters are, by default,
non -potable- He also said that, if discharged, the water would run through a grass swale and be dispersed through a
wooded buffer for at least 150 feet. The votume of water from the tank would not discharge directly into the intermittent
stream, but rather would infiltrate.
Do you think, based on Mr. Frei's statements and the attached analysis, that it would be satisfactory for us to discharge
this water? Thanks for your help with this, I await your response.
Meanwhile, we are pumping from one tank to the next to test these units and have not discharged any of the waters used
in testing_

permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215 .1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated
December 6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual permit
