Division.

appropriate Regional Office and must be approved prior to operation.
The plan shall include, but is not limited to: (1) Groundwater monitoring strategies to demonstrate the

authority to sign documents has been assigned or delegated to the manager in accordance with corporate procedures.
(2) For a partnership or sole proprietorship: by a general partner or the proprietor. respectively: or
(3) For a municipality. state. federal, or other public agency: by either a principal executive officer or ranking elected offictal.

solid waste disposal.

result in contamination of the surface or ground waters.

This is a sir
ple sand mine covering 10 acres. Maximum depth
is expected
to be 30 feet. A perimeter berm will be established
around the
Mining Area to contain all stormwater within the pit.
Stormwatei
and groundwater will be allowed to collect within the
pit and will
be occasionally pumped out as necessary. The pumped
water will p
iss through the 60 foot by 30 foot basin as shown. All
final side slopes
will be graded to a 3 to 1 slope (or flatter). Runoff
from stockr

material ge
ierated offsite (such as crushed concrete) will be
stored in th
