on February 29 2012 The site inspection was in response to your request for assistance
concerning the NPDES General Permit NC6020000 Enclosed is a copy of the compliance
inspection report You must comply with the quarterly monitoring requirements for wastewater
discharges and semi-annual monitoring of stormwater discharges from your pit The Stormwater
Pollution Prevention Plan is due on 04/09/2013
Please continue to work with the Division of Water Quality Wctland Specialist, Chad Coburn, to
ensure the wetlands on site are adequately protected from any dewatering activities and that an
approvable operation and monitoring plan has been developed and implemented

Inspection Summary
The owner of the pit asked for assistance from the Wilmington Regional Office to help him with compliance with this mine
pit The wetland specialist Chad Coburn was asked to provide assistance concerning an approvable operation and
monitoring plan to ensure the wetlands are protected from dewatering activities Although the owner was operating
without the necessary permit the owner voluntarily contacted the Division of Water Quality for assistance in gaining
compliance with permit requirements The owner immediately applied for the permit once he was informed the permit was
required For this reason the inspector did not recommend the issuance of a Notice of Violation

we are forwarding herewith the subject certificate of coverage to discharge under the subject
state — NPDES general permit phis permit is issued pursuant to the requirements of North
Carolina General Statute 143-215 1 and the Memorandum of Agreement between North Carolina
and the US Environmental Protection Agency dated October 15 2007 (or as subsequently
amended)
Please take notice that this certificate of coverage is not transferable except after notice to
the Division of Water Quality The Division of Water Quality may require modification or
revocation and reissuance of the certificate of coverage
Please be aware that mine dewatering at your site has the potential to impact adjacent
wetlands The specific requirements for mine dewatering at facilities near wetlands can be found
in Part III, Section E of your permit You must have an approved Operation and Monitorm�
Plan before mine dewatering discharges occur at your facility Discharging mine dewatering
wastewater without Plan approval will be subject to enforcement action An Operation and
Monitoring Plan for your facility must be submitted to the Wilmington Regional. Office within

2 Mine maps must be accurate and appropriately scaled drawings, aerial photographs or enlarged
topographic maps of the entire mine site All aspects of the mine site must be clearly labeled on the
maps along with their corresponding (approximate) acreage As a reminder, mining permits can
only be issued for up to 10 years, thus, all mine and reclamation maps must only denote those
