STORMWATER DIVISION NCG PERMI PERMIT NO. / C Ua O DOC TYPE W HISTORICAL FIL LI MONITORING F DOC DATE El aoo) I YYYYMMDD MC®ENR North Carolina Department of Environment and Natural Resources Beverly Eaves Perdue Governor Jerry Davis Construction Co , Inc P O Box 819 Williston, NC 28589 Dear Mr Davis, Division of Water Quality Charles Wakild, P E Director November 15, 2012 Dee Freeman Secretary Subject Technical Assistance Reporting Requirements Smyrna Mine Pit Certificate of Coverage NCG020657 I had received some analytical results in the mail for your quarterly monitoring event in August of this year Thank you for your inquiry concerning the reporting requirements regret it has taken some time to get back to you concerning this matter Enclosed are your analytical results for your files (please keep these records in your files for at least 5 years from the date the samples were collected) I recorded the sample results on an annual summary DMR for your convenience to help you see where and how you should be recording your data I don't know if you collected samples for the first half of the year (January through June), but your monitoring requirements are QUARTERLY, so please be sure to remember to collect samples If you did not pump any wastewater from the pit during a given quarter, you may state as much on the DMR by writing in "no discharge for the quarter" You must also document in your stormwater pollution prevention plan that no pumping had occurred during that particular quarter In the case that no pumping is occurring, the Division would expect the level of water in the pit to reflect no pumping activity Don't forget to collect your fourth quarter samples to remain compliant with your permit Be sure to collect your 41h quarter samples before December 31, 2012 Please review the enclosed DMR If you conducted additional sampling, please include that data (if samples were collected during the first and second quarters of 2012) If not, please ensure in the future that you remain compliant with the quarterly monitoring 1617 Mad Service Center Raleigh North Carofrna 27699 107 Location 512 N Salisbury St Raleigh North Carolina 27604 Phone enclosed a copy of the annual summary DMR without any writing on it so that you will have a template for use in the future The annual summary report must be mailed in to the Division of Water Quality no later than March 1" of each year highlighted that information on the DMR to bring your attention to that requirement Also, before sending in the DMR, it must be signed by the permittee You should compare the limits in your permit (Part 111, table 8 on page 14 of 16) with the analytical data reported for your sample results from the laboratory to determine if your discharge has exceeded any of the limits, if so, you will need to check the box on the front page of the annual DMR, indicating "yes" to limit violations In the case of exceedances, you must send a copy of the DMR to the Wilmington Regional Office within 30 days If you check "no", no further action is required at that time In order to avoid limit violations, it is best to avoid any dewatering within several days of any ram event You can judge the quality of the water in the pit prior to discharge, as well, to avoid pumping when the water is very turbid You should read your permit every year so that you remain familiar with the permit requirements Please do not forget to maintain your stormwater pollution prevention plan which includes annual certifications, employee training requirements, annual review of the stormwater pollution prevention plan and facility inspections Documentation of these events are required by the stormwater pollution prevention plan If you have any questions, you may contact me Facility Location Location of Receiving Waters Latitude and Longitude Coordinates Surrounding waters found on 303d list of impaired waters Watersheds for which a TMDL has been established Site Map Please refer to the attached map of both current and proposed site conditions for the following information - Property Boundaries Stormwater Discharge Outfall Adjacent Surface Waters and Wetlands - Industrial Activity Areas (material storage, disposal areas, processing areas) Site Topography and Drainage Features (including direction of flow) - Outfall Location Existing BMP's and Design Criteria -Spill Prevention and Response - storage of Industrial Chemicals and Required Record -Keeping Any and all hazardous industrial chemicals shall be stored offsite in a properly maintained building suitable for the storage of such chemicals Potential hazardous chemicals may include the following hydraulic fluids, engine oils, and gasoline and diesel fuels All heavy equipment kept on the site is to remain in a properly maintained manner so to prevent any potential leakage Heavy equipment requiring such chemicals will be kept in the designated area (shown on the map) away from the excavated pit, and a vegetated buffer will remain in place surrounding the designated area where the equipment will be kept The fueling of equipment and maintenance of said equipment shall occur within the designated area No longterm storage of any heavy equipment will occur on the site as the site is mostly inactive and the equipment is used at other locations considered active There will not be any storage of industrial chemicals on site **All discharge monitoring data will be recorded on a monthly basis and shown on the Process Mine/Dewaterrng Wastewater = DMR" forms provided by the Division of Water Quality and submitted to the State Central Office by March 1 of each year ** Preventive Maintenance and Good Housekeeping To prevent potential pollution of the surface water and groundwater of the state, the following Preventative Maintenance and Good Housekeeping plan shall be implemented on site - No industrial chemicals/petroleum products are to be discharged into any waters This includes the excavated pit itself which contains groundwater and is ultimately pumped off -site All products will be kept off -site and only brought onto the site for the temporary purpose of fueling/maintenance of any heavy equipment This maintenance will be conducted in the designated area shown on the attached map -Fueling and Maintenance Delivery of fuel to the site will be by approved highway tanks or mobile refueling tanks The transferring and dispensing of fuel will be done with pumping equipment, an approved hose, and top -fill nozzle When unreeling the fuel transfer hose and nozzle, the nozzle must be in the upright position Ensure that a site -appropriate spill containment kit is readily available -Only fuel tanks with storage of less than 660 gallons are to be allowed onsite and only on a temporary basis If bulk fuel storage of 660 gallons are to be stored, then secondary containment will be required and illustrated to NCDENR in a revision to this plan -Used chemicals/lubricants/fuels will be taken offsite and disposed of according to federal regulation This includes all empty containers of such chemicals -In such cases where any chemicals are spilled onto the ground, the contaminated soil is to be excavated and removed offsite to an appropriate landfill accepting contaminated sod The excavated area will be backfilled with clean soil and re -stabilized with vegetation -Inspections of all heavy equipment will be conducted on a monthly basis All inspections and required maintenance will be recorded Maintenance/inspection Logs will be kept along with annual Summary DMR forms as part of required Operation and Maintenance and in addition to this SWPPP -Reporting of Oil Spills Any spill greater than or equal to 25 gallons must be reported within 24 hours If any spill results in a sheen on surface waters, they must be reported within 24 hours Any spill regardless of amount that occurs within 100' of surface waters must be reported within 24 hours Also, in the event that a spill less than 25 gallons cannot be cleaned up, they too must be reported within 24 hours Employee Training and Yearly Updates Annual All employees associated with Jerry Davis Construction shall be knowledgeable of this SWPPP and will be required to follow all Preventive Maintenance associated with Good Housekeeping Upon hire of new employees, there will be a training session to familiarize the employee with the approved mining permit granted by the Division of Land Resources, and all aspects of the mine map associated with this SWPPP Training will involve the employee becoming knowledgeable of the mine wastewater discharge outfall and the associated BMP's in place The employee should understand maintenance issues regarding these BMP's In addition, the employee shall be familiar with the mine permit boundaries, all stockpile areas within these limits, as well as the designated area for temporary storage of heavy equipment Each employee will be familiar with the proper disposal of all industrial chemicals Employees will be directed by the Designated Individual of the Responsible Party whose signature is in the below paragraph Responsible Party and Agreement to Plan Amendment Jerry Davis Construction agrees to amend this SWPPP at any time that changes occur at the site which may not be addressed in this current plan Jerry Davis Construction also agrees to review and update this SWPPP on an annual basis All record keeping regarding stormwater/wastewater discharge will be kept onsite and submitted to the Division of Water Quality for review as requested This SWPPP will also be accompanied by an Operation and Maintenance plan to follow Responsible Party Jerry Davis Construction PO Box 819 Williston, NC 28589 Designated Individual Responsible for the Overall Implementation of this Plan Debbie Davis _ Debbie Davis _ signature -Stormwater Management Plan - Management of Stormwater Runoff and Runon As excavation exceeds below the water table, it is necessary that dewatering of excess groundwater occurs The excavated pit area is permitted with the Division of Land Resources for 1186 acres Minimal excavation at the pit is the result of a slowing economy with only a few loads of material being excavated each week Therefore, approximately 4 75 acres have been excavated As a result, there is adequate settling efficiency of any turbidity within the pit Dewatering is conducted by a pump skimming the surface of the groundwater Pumping is only to be conducted during working hours as needed For extra settling efficiency and slowed velocity to prevent scouring of wetlands, all pumped water is routed through a vegetated strip and then discharged through the existing series of ditches All effluent is discharged as sheet flow into 404 wetlands located on -site thus recharging them and allowing for nutrient removal Turbidity standards and Total Suspended Solids shall not exceed the standard for HQW (High Quality waters) and PNA (Primary Nursery Areas) Monthly monitoring of all required parameters regarding Stormwater and wastewater discharge will be conducted as required in the Operation and Maintenance Plan as well as monitoring requirements specified in the current IvC6020000 for Quarterly Fecal Cohform and TSS (Total Suspended Solids) All data will be recorded on the Annual Summary DMR forms provided by the Division of water Quality and submitted by March 1 of each year Erosion and Sedimentation Control including associated Best Management Practices Erosion and Sediment Control of the site consists largely of permanently vegetated buffer zones surrounding all disturbed areas as well as added silt fencing where needed The site has little topography with sandy soil allowing for an extremely high infiltration rate All areas drain internal to the excavated pit allowing for any sediment to settle and be excavated for mining purposes At the point of the wastewater discharge outfall, water is skimmed from the surface and pumped at a slow velocity over a vegetated strip and then discharged through the existing series of ditches There has been no observation of turbid water being pumped into the adjacent wetlands * *Please note that this site remains in compliance with the Division of Land Resources and the conditions of its mining permit" Inspection and Maintenance of BMP's (Best Management Practices) This site is to be monitored after every rainfall There will be a rain gauge located onsite and inspection log to record all data Upon inspection of the site after each rainfall by the operator, all said erosion and sediment control measures will be inspected and properly maintained as needed Adequate permanent vegetation of all buffer zones will be maintained As pumping of wastewater is conducted, samples will be taken and sent to a certified laboratory All data will be kept and submitted by March 1 of each year Any limit violation exceedances must be reported to the Wilmington Regional Office as required it (.o:9 .
