Matt,
On September 30, 2019, the Division of Water Resources (Division) received your application dated July 1, 2019,
requesting a 401 Water Quality Certification from the Division for the subject project. The Division has determined that
your application is incomplete and cannot be processed. The application is on hold until the following items are
addressed:
1. Please provide an agent authorization letter granting you authorization to sign the Pre -Construction Notification
on behalf of the Village Walk Homeowners. An agent authorization may be found here

2. You provided the attached information to Kevin Mitchell in this office prior to PCN submittal. The attached
information was not included in your PCN submittal to the Division. Please confirm the attached information
may be included as part of your application. [15A NCAC 02H 0502(a)(4)]
3. Per Condition 11.9. of 401 General Water Quality Certification 4134, all work in or adjacent to streams must be
conducted such that the flowing stream does not come in contact with the disturbed area. Please describe how
this condition will be met. [15A NCAC 02H .0506(b)(3)]
Pursuant to Title 15A NCAC 02H .0502(c), the applicant shall furnish all of the above requested information for the
proper consideration of the application. If all of the requested information is not received, the Division will be unable to
approve the application and it will be returned.
