Provide a brief description of this project (attach a site plan): The mixed use Parkside Town Commons Development is proposed to be located in the NE quadrant of the NC540 and NC55 interchange in Cary. The Parkside Town Commons Development has already been permitted with NCDWQ (#EXP 07-1503) and USACE (#2005-21328 and 2007-04171) and is considered Phase I of this project. Phase II includes the roadway improvements required to the surrounding infrastructure as a result of this development. NC55, O'Kelley Chapel Road, Morrisville Carpenter Road, Carpenter Fire Station Road, McKrimmon Parkway will all be widened to facilitate new turn lanes. Three stream crossings occur within the roadway project area. A 401 certification was issued on April 30, 2009 and a 404 permit was issued on June 14, 2009 for Phase II. Changes to the crossings were made based on NCDOT comments which resulted in an additional 11 feet of permanent perennial stream impact. In the first iteration of this application, one crossing (O'Kelley Stream Impact #1) had a permanent impact of 11 linear feet associated with a culvert extension. This impact has now been shortened to 8 linear feet. Another crossing (O'Kelley Stream Impacts #2 & #3) had a temporary impact of 35 linear feet related to the installation of a new inlet structure and endwall on either end of the existing pipe. The headwall was not approved by NCDOT so a culvert extension of 14 feet (permanent impact) is required at that location. Temporary impacts are no longer present at Stream Impact Site #3. No other changes were made to the project. A site plan as well as detailed maps of the impacts has been included. 2k. Take the first left onto O'Kelley Chapel Road, the stream impact site is located on the right side of the road before Alston Avenue. Additional roadway work is occurring along NC 55 from Kit Creek to the south side of the intersection with O'Kelley Chapel Road as well as the intersections with McCrimmon Parkway, Carpenter Fire Station Road, and Morrisville Carpenter Road. Wetlands in the right of way on the west side of NC55 near Kit Creek were delineated and are included in this permit application but have not been visited by regulatory personnel during this proiect. 6b.
