Corey,
The permittee never requested their renewal COC on-line, and so one was never generated and filed in Laserfiche. They
need to do that. Attached is the issuance letter with those instructions that we sent out in August 2018. (Just for
reference, I've also attached the initial renewal information letter that went out earlier that summer.)
The link to obtain the renewal COC is here:
https://edocs.deg.nc.gov/Forms/SW-COC-RenewaI
Even though most requests are for the recent renewals, it should still work for this one too. If any information is
incorrect for this permit, though, he'll have to contact us. Permittees were asked to review that info ahead of the COC
issuance in our June 2018 letter. It looks like this COC is in Mulch Masters of NC, Inc's name; his email address suggests
that may have changed since then. If there's been a change of ownership, they will have to fill out this form:
https://edocs.deg.nc.gov/Forms/SW-Name-Change
I don't know why the original historical file is not in Laserfiche, though. Hopefully it means it was in the last batch of
scanned NCGs that just came back from Star Point. I have not yet imported those, but I have filed all the other NCG21's
that were scanned.
I hope that helps! I will put this correspondence (along with the letters) into our repository this morning.

Subject: Missing COC
Bethany,
I just spoke to someone who is looking for a copy of their COC. I looked in Laserfiche and there's no folder for the
permit. It's NCG210370. How can I go about getting a copy of that? Do we only have a paper copy?
I'll reach back out to him, but I'm going to put his contact info here because I have too much paper on my desk and
saving it in this email will ensure I don't lose it!
