The Stormwater management plans and design for the project Olivers Trace Apartments have been reviewed and are APPROVED.
Please note that this aaaroval is a termed a "conditional aaaroval" within the IDT system review. This essentially means
that the design phase of the review is concluded. However, before a "full approval" within the system cab be issued and
hence a Stormwater manaaement Dermit is issued the followina supplemental Dermittina items must now be submitted for
review and processin
• Submit one hard copy of the sealed and approved Stormwater management plan set and Stormwater management design
report to me for my file.
• Submit a copy of the approved NCDEQ and US Army Corp of Engineers permits to me for the proposed stream impacts
associated with this project. (Note: I cannot issue my permit until these other permits are verified)
• Submit a sealed engineers estimate of the cost of the Stormwater management system (estimate must include the cost of the
SCM's - both the filter system and the underground pipe storage system, cost of all storm systems and structures that convey
runoff to and from the SCM's and any other pertinent costs as applicable that may be associated with the construction of the
Stormwater management system. Also include a minimum contingency of 10%). Note: This estimate may be scanned and e-
mailed to me if you wish should you prefer that over using regular US mail.
• Upon approval by me of the estimate a non-refundable surety equal to 4% of the cost of the Stormwater management system
must be submitted. This surety can be in the form of a check made payable to "The City of Winston-Salem".
• A signed Operation and Maintenance Agreement with all relevant exhibits attached. Exhibits include: (A) a legal description of
the land the development is taking place on; (B) a legal description of the blanket easement that is to be provided for
maintenance and access. This easement should cover the entire property and ensure it also covers access to the nearest public
right of way (C) you must include all relevant Operation and Maintenance Manuals for the SCM's included in the design. The City
does not have a standard template available for an underground piping system nor a Contech filter system. However, I have
attached a scan of a manual developed for a previous project that was created and submitted by the engineer for that particular
project and that also used a Contech filter and underground piping detention system. You can use this as a go-by for creating
your own such manual. Please read through this example manual carefully and when recreating something similar ensure all pipe
sizes and references in your own manual reflect your own particular situation. Once created then have it signed and notarized by
the owner and include the entire manual as Exhibit C in the agreement. Upon submittal to me of the entire O&M Agreement
containing all the exhibits as just described it will be reviewed, approved if found sufficient and signed by various entities within
the City of Winston-Salem such as the Stormwater Director and the City Attorney. Once its all signed the owner/or owners
engineer shall be notified so that the document can be collected and recorded by them at the Forsyth County Register of Deeds
office. A copy of this recorded agreement must be provided back to the Stormwater Division. I've attached in this e-mail the City's
standard O&M Agreement template for your use in preparing this document. Please note that if any changes to this standard
language are proposed by your client then such changes will first need to be approved by the City Attorneys office and this will
add more time to the process.
