ENGINEERS-PLAN NERS-SURVEYORS PROJECT NARRATIVE for Bug-N-A-Rug Bolivia, North Carolina October 15, 2019 Bug-N-A-Rug is located at 1704 Southport Supply Road, approximately 0.7 miles West from the intersection of NC211 & Clemmons Rd SE in Bolivia. The Tax ID number for the property is 1850002206. The total project area is 1.27 acres, and is currently home to a commercial office for Bug-N-A-Rug. WASTEWATER COLLECTION (SEWER) The proposed commercial extermination service facility will have 20 employees. The total flow allocation has been determined at 500 gpd. The sewer system will consist of a duplex grinder station and force main as well as the gravity lines for the facility. The duplex grinder station will receive wastewater from the building via a 4" gravity service line. Upon satisfactory completion of construction, all portions of the sewer system will be owned and maintained by the developer. The duplex grinder station will pump wastewater via 200 LF of 1.25-inch force main to the point of connection on NC-211 in Bolivia, North Carolina. The point of connection is a permitted 8-inch low pressure line along NC-211 that is owned by Brunswick County. A portable emergency generator will be utilized to supply power to the lift station in the event of a power failure. 4918 MAIN STREET*POST OFFICE BOX 2469•SHALLOTTE,NORTH CAROLINA 28459•TEL:910-754-8029•Fax:910-754-8049 FIRM LICENSE NUMBER: C-3024 R State of North Carolina Department of Environmental Quality Division of Water Resources elvislon of Water Resources 15A NCAC 02T.0300—FAST TRACK SEWER SYSTEM EXTENSION APPLICATION INSTRUCTIONS FOR FORM:FTA 04-16&SUPPORTING DOCUMENTATION This application is for sewer extensions involving gravity sewers, pump stations and force mains, or any combination that has been certified by a professional engineer and the applicant that the project meets the requirements of 15A NCAC 02T and the Division's Minimum Design Criteria and that plans, specifications and supporting documents have been prepared in accordance with, 15A NCAC 02T, 15A NCAC 02T.0300,Division policies and good eneineerine practices. While no upfront engineering design documents are required for submittal, in accordance with 15A NCAC 02T .0305(b), design documents must be prepared prior to submittal of a fast track permit application to the Division.
